    Abstract: An increasing number of patients and a lack of awareness about obstructive sleep apnea is a point of concern for the healthcare industry. Polysomnography is recommended by health experts to detect obstructive sleep apnea. The patient is paired up with devices that track patterns and activities during their sleep. Polysomnography, being a complex and expensive process, cannot be adopted by the majority of patients. Therefore, an alternative is required. The researchers devised various machine learning algorithms using single lead signals such as electrocardiogram, oxygen saturation, etc., for the detection of obstructive sleep apnea. These methods have low accuracy, less reliability, and high computation time. Thus, the authors introduced two different paradigms for the detection of obstructive sleep apnea. The first is MobileNet V1, and the other is the convergence of MobileNet V1 with two separate recurrent neural networks, Long-Short Term Memory and Gated Recurrent Unit. They evaluate the efficacy of their proposed method using authentic medical cases from the PhysioNet Apnea-Electrocardiogram database. The model MobileNet V1 achieves an accuracy of 89.5%, a convergence of MobileNet V1 with LSTM achieves an accuracy of 90%, and a convergence of MobileNet V1 with GRU achieves an accuracy of 90.29%. The obtained results prove the supremacy of the proposed approach in comparison to the state-of-the-art methods. To showcase the implementation of devised methods in a real-life scenario, the authors design a wearable device that monitors ECG signals and classifies them into apnea and normal. The device employs a security mechanism to transmit the ECG signals securely over the cloud with the consent of patients.

    Abstract: The purpose of this pilot evaluation was to assess the impact of a university culinary medicine program on participating medical students and community members, which included individuals managing chronic illness and public middle school students. A total of 59 program participants enrolled in the study. Data were obtained using pre- and post-course surveys and qualitative interviews from September 2021–July 2023. Results show increased confidence in medical students’ ability to provide nutrition counseling, with a high significance in their ability to provide counseling regarding chronic conditions. Participants managing chronic conditions demonstrated significant increases in self-reported confidence in their understanding of overall chronic disease management and care and in their kitchen skills, with participants who attended five or more classes having significantly higher means. Qualitative feedback from middle school students highlights their knowledge and willingness to try new foods after engaging with the curriculum. Findings add to the growing literature on culinary medicine and provide insight into the effectiveness of culinary medicine programming to increase knowledge and promote positive changes among future healthcare professionals and community members. However, more extensive research across a longer time span is needed to confirm the potential for sustained change.

    Abstract: The emergence of vaccine-derived polioviruses (VDPVs) in patients with Primary Immunodeficiency (PID) is a threat to the polio-eradication program. In a first of its kind pilot study for successful screening and identification of VDPV excretion among patients with PID in India, enteroviruses were assessed in stool specimens of 154 PID patients across India in a period of two years. A total of 21.42% of patients were tested positive for enteroviruses, 2.59% tested positive for polioviruses (PV), whereas 18.83% of patients were positive for non-polio enteroviruses (NPEV). A male child of 3 years and 6 months of age diagnosed with Hyper IgM syndrome was detected positive for type1 VDPV (iVDPV1) with 1.6% nucleotide divergence from the parent Sabin strain. E21 (19.4%), E14 (9%), E11 (9%), E16 (7.5%), and CVA2 (7.5%) were the five most frequently observed NPEV types in PID patients. Patients with combined immunodeficiency were at a higher risk for enterovirus infection as compared to antibody deficiency. The high susceptibility of PID patients to enterovirus infection emphasizes the need for enhanced surveillance of these patients until the use of OPV is stopped. The expansion of PID surveillance and integration with a national program will facilitate early detection and follow-up of iVDPV excretion to mitigate the risk for iVDPV spread.
