    Abstract: Objective: To determine the effectiveness of therapeutic plasma exchange (TPE) in management of critically ill Covid-19 patients Materials and Methods: In this study non-randomized controlled trial, a total of 47 patients were included after thorough screening of the admitted patients in Covid ITC CMH, Multan from July-2020 to May-2021.Single administration of TPE was done to determine the results. The parameters assessed in this study included the efficacy of TPE in terms of improvement of critical end points such as Norepinephrine dose to maintain MAP of more than 65mmHg, 6-hour balance of fluid, MAP, CRP (C reactive protein), WBC count (white blood cell), platelets, INR (international normalized ratio), IL-6. Results: The variations before and after TPE in clinical and biochemical parameters shown in table. II. In clinical parameters and in gas exchange parameters no significant difference was found. Inflammatory biomarkers, before and after TPE, the parameters had not significant different, (p 〉 0.005). The differences before and after TPE between acid base balance, cytokines and vasoactive substances were also statistically insignificant, (p 〉 0.005). Conclusion: Even though there is evidence of slight improvement in clinical endpoints of Covid-19 patients with TPE, overall efficacy of TPE is still a question that needs an answer as no significant improvement could be seen in outcome values after TPE.     Abstract: Objective: To assess the efficacy of neutrophil-lymphocyte count ratio in predicting the severity of covid-19 Materials and Methods: The patients with positive results for COVID-19 were shifted to COVID ITC, CMH, Multan. The clinical histories of confirmed patients of COVID-19 during February 2020 to May 2021 were reviewed. The patients were divided into 4 classes, mild, common, severe, and fatal, according to guidelines of COVID-19 i.e., trial version 7. Data was collected regarding age, sex, smoking history, temperature, epidemiological history, clinical symptoms, and laboratory findings of all included patients including NLCR. Results: The mean NLCR of Mild and Severe Group was 2.77±1.23 and 20.31±4.45, respectively, (p=0.000). (Table. II). The area under the curves of NLCR was larger than neut, suggesting the optimal performance of NLCR with cut-off value 5.89. The sensitivity and specificity of NLCR was 85% and 95%, respectively. (Table. III). Conclusion: It can be concluded that an increase in NLCR levels can indicate that the covid-19 disease is moving towards exacerbation. NLCR can be recommended as a novel and highly sensitive and specific indicator for severity prediction in Covid-19 patients.     Abstract: Aim: To evaluates the EQ level in facilitators of government and private medical colleges of Karachi. Study design: Cross sectional Place and duration: Ziauddin Medical College, January-June 2022 Methodology:Facilitators of government and private sector medical institutes particularly lecturers were targeted population. The calculated sample; (n=35/institute) was recruited by simple random technique. The questionnaire regarding demographic data was self-developed however, emotional quotient was assessed by EQ scale. Chi-square test was applied as test of significance. Results: The maturity scale score of females showed that statistically (p-value=0.011) they are emotionally more mature than males. When sensitivity and maturity score were compared between the two institutes (Government or private) their scores were similar and there was no significant difference between sensitivity and maturity scores of government and private sector employees. According to EQ compatibility scoring system scale ,100% male and 100% female scored in the range of extremely high competency category. Conclusion: The emotional quotient analysis showed that females are more sensitive and mature emotionally than males though the emotional intelligence of male and female participants was equal. Emotional sensitivity, maturity and intelligence scores of Government and private sector employees was comparable to each other.     Abstract: Introduction: Abdominal tuberculosis has higher incidence particularly in developing countries. The challenge to Surgeon is to diagnose it as its presentation is very vague and diagnostic tools are dubious.1 Biochemical analysis helps a little bit in this regard. The abdominal tuberculosis which is not so commonly seen as pulmonary TB can be a source of significant morbidity and mortality and is usually diagnosed as late due to its non specific clinical presentation. About 15 to 25% of cases with abdominal tuberculosis here concomitant pulmonary TB.2 Any part of GIT can be affected but it is usually small bowel and sometimes a colon which is affected barium study has some role in diagnosing the ileal tuberculosis and then is on laparoscopic examination.3 Patients and Methods: This is a cross sectional study conducted at People Medical College Hospital Nawabshah. It was conducted in Surgical Unit 1 from January 2019 to December 2021. All the patients admitted in the ward came through OPD or through emergency. History was taken and proper examination of abdomen apart from systemic examination was done. Routine investigations were done. specific investigations like Liver function test (LFT) and serum electrolytes were done. Radiological investigations like ultrasound and x ray chest abdomen in erect and supine posture were done. Diagnosis was made and surgical intervention was done. Results: Total 30 patients were included in this study. Age of patients was between 20-50 years. 9(30%) patients aged between 20-3- years. 10 (33.3%) were of age between 31-40 years and 11 were aged between 41-50 years. Higher prevalence was in 4th and 5th decade of life. Patients with acute emergency were treated by Exploratory Laparotomy. Per operative findings showed stricture in 14 (47%) patients that were treated with resection and end to end anstomosis. 13 (43%) patients had single as well as multiple perforations of more than 4-5 cm in sizes with contamination of contents in abdominal cavity. Covering ileostomy was made in these patients. 3 (10%) patients’ perforations were of .5 to 1 cm with lesser contamination of abdominal cavity. Primary repair was made in these patients. Conclusion: It is concluded that Higher prevalence was in 4th and 5th decade of life. Its prevalence in female is higher as compared to male. .Majority of the patients with Abdominal Tb were treated by resection and end to end anastomisis.     Abstract: Introduction: Epididymo-orchitis is common condition in scrotum found in surgical practice. It is commonly seen in young males. It can occur at any age. The common age is between 2nd to 4th decades of life. Its duration of illness is 6 weeks in acute condition and 12 weeks in in chronic conditions. It mostly occurs associated with sexually transmitted diseases. Chlamydia trachomatis and Neisseria gonorrhea are the common causative organisms. After age 40, the common organisms causing this disease are Escherichia Coli and Coliform bacteria found on G.I.T. There are also other causes of this disease. It is mostly managed conservatively but it can be operated it the condition develops complications. Even Orchidectomy is also done in some conditions. Objective: To evaluate the management options in epididymo-orchitis . Material methods: This is a cross sectional study done at surgical Unit 1 of Peoples Medical Hospital (PMCH) Nawabshah. The duration of study was one year from 10th August 2019 to 9th July 2020. All the patients were admitted through Surgical out Patient Department (SOPD) and emergency department of Peoples Medical College Hospital (PMCH) Nawabshah. Approval of study was taken from Ethical committee. Detailed history and thorough clinical examination was done. Routine investigations were done apart from viral markers including HIV. Urine DR and Culture ,Ultrasound of Scrotum and CT Scan were done. Management was planned accordingly. Results: Total 65 patients were included in this study. Of them, 20 (31%) had left sided pathology, 40 (61%) had right sided and 5(8%) had bilateral. Testicular abscess formation was seen in 8 (12.3%) patients. In 2 (3.07%) patients, testicular infarction was noted. 1 (1.5%) patient became infertile due to bilateral involvement. 3 (4.6%) patients had developed psychosis. Conclusion: It is concluded that most of the patients were treated conservatively and in a few patients, incision drainage and orchidectomy was performed. Keywords: Epididymo-orchitis.     Abstract: Background: Stevia is nontoxic and non-caloric sweetens nearly 300 times more sweetener than any other source. Numerous health benefits for chronic patients and its reach source of essential nutrients pooling the world. Objective: This study tried analyzed the effect of black cumin added in stevia apple jam in place of sugar as functional ingredient due to health promoting effects in case of hypertension. Method: A stevia-sweetened apple jam sample was mixed with black cumin. And the effect was measured and recorded by sensory evaluation and proximate analyses over a six-month period. Practical implication Conclusion: Finding revealed that black cumin contained stevia apple jam significantly increased the total suspended solid level while, decreased moisture contents in sampled experiment. Unlikely, it increased nutritional components such as proteins, carbohydrates and fat due to black cumin addition. Thus, black cumin in stevia apple jam paramount important with magnifying essential nutrients for current novel chronic disease remedy.     Abstract: Background: Procalcitonin has been used globally as indicator marker of sepsis to define bacteremia or blood stream infection for early patient management or antibiotic therapy. Blood culture refers to a microbiological culture of a peripheral blood sample. The blood cultures help to determine the presence of systemic infections, such as septicaemia. If the culture is positive, the causative micro-organism can usually be identified, and antibiotic sensitivity testing performed. On the other hand, the sensitivity of procalcitonin in comparison to its specificity to certain bacterial infections is still in question. Methodology: In this study, we selected 309 patients without restriction of age gender with pre-diagnosed sepsis or septic shock syndrome. Blood culture samples were collected in bactAlert media plus culture bottles and were incubated in BactAlert 3D semi-automated blood culture system; blood sample was collected for serum procalcitonin on immunoassay principle. Results: Out of 309, a total 87 blood cultures were positive and PCT was positive in 134 whereas 179 were PCT negative. In positive PCT, 63 were having bacterial growth in their blood culture while 71 were with negative blood culture. Although in total, 179 PCT were negative, only 16 were having bacterial growth in blood culture samples whereas all others revealed no growth that showed maximum number of positive blood cultures were having PCT positive but not all PCT positive demonstrated bacterial growth in blood culture suggesting that the bacteremia or sepsis can only be suspected with positive PCT. In addition, blood culture positive or negative results cannot be predicted by PCT value to make diagnostic confirmation of sepsis.     Abstract: Objective: To evaluate the outcome of patients in term of range of motion after total knee arthroplasty with all poly implants. Study Design: Retrospective study Place and Duration of Study: Department of Orthopaedics, Sahara Medical College, Narowal from 1st April 2021 to 31st March 2022. Methodology: Sixty various patients who came for total knee arthroplasty were selected on the basis of their conditions. Patients were divided into two inserts groups for understanding the better outcome results in terms of range of motion (ROM). Thirty patients having fixed bearing and other 30 patients having mobile bearing inserts will all poly implants. A standardized medial-parapatellar approach was taken for surgical procedure and resection of primary tibia was done. Pain function and evaluation was performed through knee society scoring (KSS) as well as VAS score and goniometer was used for analyzing range of motion which was defined as degree of flexion of knee subtracting from number of extension-defect. Results: There was no significant difference in either the gender or ages of both implants selected patients However the mobile bearing group patients has a better BMI value than the fixed bearing implant patients. In term of range of motion mobile bearing group patients had a higher ROM value than the fixed bearing operated patients in case of overall change in range of motion analysis. The Western Ontario and McMaster Universities Osteoarthritis (WOMAC) Index, score showed a significant change with an increased value observed in mobile bearing implant patients. Conclusion: Visual analogue pain score show significant difference in mobile and fixed bearing group. In terms of range of motion, mobile implants show better outcome in contrast to fixed bearing group. Keywords: Knee implants, Mobile, Fixed bearing, Outcomes, Osteolysis

    Abstract: Late-life depression is considered as one of the features of older age instead of a treatable disease. This under recognition of this major disorder leads to multiple visits of patients to different medical consultants with different clinical presentations. This research aimed to find out the relevance of age of onset of depression with its serious life-threatening output like brain hemorrhage, suicide etc. Methods: Then if divide it according to its classifications, there will be two types of depression, first one is early-onset depression that happens before 60 years of age and second one is late-life depression that occurs in elderly 60 years of age or above. Age of onset matters alot while making diagnosis or treating depression because of the differences in treatment response and complications. Depression is directly linked with brain and start effecting whole body including. As the day’s passes, it effects more our brain and body as compared to previous day. 196 patients are taken as sample and divide them into different groups according to early and late life depression. Results: After the research, according to the results that are generated as patients with late life depression show more harmful effects and leads to brain hemorrhage as compared to those who was diagnosed during early-onset of depression. Conclusion: It is concluded that these hemorrhage and nervous disorder does not occur with early onset of depression but with late onset depression.

    Abstract: Objective: To assess the outcome of dynamic hip screw fixation in intertrochanteric femur fractures. Study Design: Prospective cohort study. Place and Duration of Study: Department of Orthopaedics, Pak Red Crescent Medical & Dental College, Dina Nath from 1st October 2020 to 31st March 2022. Methodology: Fifty Patients suffering from intertrochanteric femur fractures were enrolled. The operational time as well as incision length, measurement of blood loss during surgery was recorded. The chance of re operating on a patient and post-operative infection was also observed and documented. A complete follow up for assessing the outcomes of dynamic hip screws were continued until 18 months. Results: The mean age of the patients was 58.05±7.58 year. There were 64% males in this study while only 36% females. The ASA grade presented 32% cases within grade II while grade I and IV had equal number of cases in it. The Harris hip score presented outcome scores at 1 then 2 followed by 12 then 15 and 18 months as mean 75.37, 85.40, 88.25, 88.76 and 87.53 respectively. Conclusion: Dynamic hip screw fixation is reliable surgical procedure for treating intertrochanteric femur fractures with decreased complication rate and improved Harris hip scoring.
