Abstract: Objective: To determine the frequency of post-operative complications of transurethral resection of prostate (TURP), in benign prostatic hyperplasia (BPH) patients, using the Modified Clavien Classification System (MCCS). Study Design: Descriptive study. Setting: Urology Department, Sindh Institute of Urology and Transplantation, Karachi. Period: 26th May, 2019 to 25th Nov, 2019. Material & Methods: A total number of 162 patients with benign prostatic hyperplasia planned for TURP were included in this study and Post-operative complications data was collected, and classified according to the Modified Clavien Classification System (MCCS). Patient’s demographics and other parameters like prostate volume, operative time, mean prostatic tissue resected and hospital stay was collected. Results: Mean age was 63.32±8.36 years. Mean prostate volume was 56.99±13.25 grams. Mean operative time was 26.55±9.46 mins. Mean prostate tissue resected was 16.75±12.09 grams. Mean hospital stay was 1.27±0.60 mins. Grade I complication was occurred in 06 (3.70%) patients, grade II in 03 (1.85%) patients, grade III in 00 patients, grade IV in 01 (0.62%) patients and grade V in no patient. While there were no complications in remaining 152 (93.83%) patients. Conclusion: Clavien–Dindo classification system can be easily applied by urologists to grade the post-operative transurethral resection of prostate (TURP) complications. We observed that TURP is a very safe procedure for surgical management of benign prostatic hyperplasia, and is having low morbidity and mortality.

Abstract: Objective: To determine post procedure recurrence and incontinence rate for high fistula-in-ano cases, who underwent procedure of fistulectomy and Seton placement. Study Design: Descriptive study. Setting: Department of General Surgery, Khyber Teaching Hospital Peshawar. Period: January to December, 2018. Material & Methods: This study was conducted over 50 patients with diagnosis of high fistula-in-ano, treated with combined procedure of fistulectomy and Seton placement. Patients follow up was done for one year for recurrence and incontinence. Results: Out of 50 patients, 28(56%) were male and 22(44%) were female. Overall male to female ratio was 1.27:1. Average age of patient was 36.88 years+6.63 SD. Flatus Incontinence was observed in single case (2%), no patient reported stool Incontinence. Recurrence of disease was found in 2 patients (4%). All patients were followed for one year. Conclusion: This technique for treating high Fistula-in-ano showed overall low recurrence and incontinence rates during follow up, thus reducing morbidity and costs associated with recurrent surgery for Fistula-in-ano.

Abstract: Objective: To compare stone clearance rate in patients with distal ureteric stones, undergoing URS with safety guide wire and without safety guide wire. Study Design: Randomized Clinical Trial study. Setting: Department of Urology, Pir Abdul Qadir Shah Institute of Medical sciences. Period: January, 2020 to December 2020. Material & Methods: The study patients fulfilling the inclusion criteria were divided into two groups using the lottery method. Group A: Patients underwent URS with insertion of safety guidewire (SGW). Group B: These patients underwent URS without safety guidewire (SGW). Patients in both groups were compared for stone clearance. Results: Mean age in Group A was 37.79±12.44 years and in Group B were 33.31±11.48 years. Group wise distribution of gender showed 79 (73.83%) male and 28 (26.17%) females in Group A and 70(65.42%) male and 37 female (34.57%) in Group B. In comparison of both groups, 65.42% stone clearance was noted in Group A while 78.50% stone clearance was found in Group B and p-value found to be significant (P=0.033). Conclusion: Our study concluded that stone clearance in patients with distal ureteral stones undergoing URS without SGW is higher in comparison to URS with SGW, without any added advantage.

Abstract: Objective: To correlate the prevalence of COVID-19 and the rhinopathological conditions simultaneously in the population visiting the hospital of Mirpur, Azad Jammu and Kashmir. Study Design: Case-control study. Setting: District Head Quarter (DHQ) Teaching Hospital Located in Mirpur, Azad Jammu and Kashmir. Period: September 2020 to February 2021. Material & Methods: Patients visiting the DHQ teaching hospital, Mirpur, Azad Jammu and Kashmir for their COVID-19 related symptoms were included in the study. Demographic data was obtained. Prevalence of DNS and nasal polyps was recorded for further statistical analysis of the symptoms in the population. The obtained patient data was evaluated for the rhinopathological conditions that exists with the prevailing COVID-19 or may even aggravate the situation. Although nasal polyps and DNS are not among the typical symptoms of the COVID-19 but they may add to the results of PCR testing following swab testing. Conclusion: Despite being a significant relationship, there lies a chance of misleading results of swab testing as these rhinopathies obstruct the airway hindering the path. Therefore, while carrying out a confirmation test for the infection it is important to consider the chances of rhinopathies in patients beforehand.

Abstract: Background: Retrograde nailing for fracture of distal shaft of femur is commonprocedure but data about post-operative functional outcome in terms of knee range of motion, union and pain at 6 weeks, 6 months and 1 year follow-up is lacking. Objectives: The mainaim of this study was to determine post-operative functional outcome after performing nailingin a retrograde fashion for distal disphyseal femoral fracture in terms of knee range of motion, pain and union at 6 weeks, 6 months and at least 1 year follow-up. Study Design: Descriptivecase series. Setting: Orthopedics and Spine Centre, Ghurki Trust Hospital Lahore, Pakistan.Period: 1st July 2015 to Dec. 2016. Materials and Methodology: 140 patients of either sex andage with fractures of distal disphyseal femoral fracture and were operated using retrogradenailing were included in the study. All the data were analyzed using SPSS 17.0 version. Results: 140 patients were included with mean age of 31.81± 7.117 ranged from 16 to 75years of age.97 patients (69.3%) were male and remaining 43 patients (30.7%) were female.Pain was significantly decrease in all patients on Visual analogue scale. Mean union were 32.3± 5.3 weeks, Knee range of motion after last follow up were more than 1200 in 126 patients. Conclusion: Retrograde Nailing for distal diaphyseal fractures is an excellent option with goodfunctional outcome. It should always be considered while managing such fractures.
