    Abstract: Our aim was to quantitatively assess the relationship of smile esthetics variables with various types of anterior overjet (OJ) malocclusion, and identify the cephalometric factors affecting smile measurements in different types of anterior overjet malocclusion. 90 patients undergoing orthodontic treatment in the Department of Orthodontics were selected for this retrospective study based upon the inclusion criteria. The patients were divided into the following groups according to their OJ: Group 1 (0-4mm), Group 2 ( & #62;4mm), Group 3 ( & #60;0mm). The upper lip height, and inter-labial gap differed significantly among the groups, whereas arc ratio, tooth number, upper midline, buccal corridor, smile index, arch form index and lower tooth exposure did not significantly among the groups. Some smile variables (upper lip height, inter-labial gap) differed significantly among different types of anterior overjet malocclusion. This study confirmed that the smile pattern varies between different types of malocclusion.

    Abstract: Aims of this study to determine the predisposing factors and common presenting complaints of ROCM patients, the knowledge of which will assist in formulation of effective preventive measures at our local level. A retrospective, hospital record-based study was conducted comprising 30 patients that presented to our tertiary care centre from 1st February to 31st July, 2021, admitted under a Multidisciplinary mucormycosis team. Detailed history and examination was supplemented with histopathological and radiological investigations. Preponderance for middle aged to elderly was seen. Only one patient had bilateral presentation. Diabetes mellitus was found to be the primary associated comorbidity. Duration of COVID infection emerged as a significant factor with 70% of cases observed to have a duration & #62; 28 days. Primary presenting complaint was facial edema followed by proptosis and Diminution of vision. Corticosteroid use was the prevailing predisposing factor. All patients received parenteral therapy with Amphotericin B. Of the 30 cases, 76.67% required primary functional endoscopic debridement, and four reported mortality as the final outcome. 10% patients required re-debridement with retrobulbar amphotericin B injection, and only two ended in Orbital exenteration. High index of clinical suspicion in elderly and diabetics especially in corticosteroids and/or oxygen therapy is required. Due attention to warning symptoms and signs, early diagnosis and apt treatment may help optimize the outcome of ROCM in the setting of COVID-19.

    Abstract: Maxillary and mandibular skeletal discrepancies are one of the commonly prevailing malocclusion in the childhood. Many a time it is ignored by the parents as well as the clinicians. It can present itself with aesthetic, functional and psychological problems of the child. Proper identification, diagnosis and treatment planning at the proper age can lead to correction and improvement in the overall facial balance and psychological esteem of the child. Over the years orthopedic and myofunctional appliances has been used to correct Class II and Class III skeletal malocclusion in a growing child. Face mask therapy with RME has been used as treatment choice to treat a patient with skeletal Class III malocclusion. Orthopedic correction in the early stage of life can eliminate the severity and further complex treatment modalities in the later stages of life. This case report describes the treatment of male child aged 12 years who had skeletal Class III malocclusion with maxillary deficiency. The treatment was done in two phases; orthopedic correction was done initially using RME Facemask followed by fixed orthodontic correction. There was overall improvement in the facial profile and occlusion of the patient post treatment.

    Abstract: : The measurements for assessing the antero-posterior discrepancy between the apical bases are many with their individual benefits and shortcomings. So, it is very important to categorize the reliability of the major diagnostic criteria for successful planning of treatment and to fore see the outcome. The present study assesses the predictability of Wits appraisal, ANB, Beta, Yen, W, µ and Pi angles as an indicators of anteroposterior dysplasia. A total of 120 lateral cephalograms of skeletal Class I, II and III patients were selected based on Down’s facial angle and then tracing was carried out manually to measure Wits appraisal, ANB, Beta, Yen, W, µ and Pi angles. Analysis of variance and correlation coefficient analysis were done to assess the significance of association between these variables. Yen angle is highly predictable and a homogenously distributed angular parameter used to assess sagittal discrepancy. The most desirable differentiation results among the three skeletal relationship were represented by Beta angle followed by Yen angle. Statistically significant correlation among the seven parameters for skeletal Class I relationship were shown by Yen and W angle, for skeletal Class II relationship by Beta angle and µ angle and for skeletal Class III relationship by Beta angle and W angle. No statistically significant correlation existed between Yen Angle and Beta Angle in all the three groups. Yen angle was found to have the most homogenous and predictable parameter. A statistically significant correlation for skeletal Class I relationship were observed between Yen and W angle, for skeletal Class II relationship between Beta and µ angle and for skeletal Class III relationship between Beta angle and W angle and they can be used interchangeably in assessing skeletal jaw relationship.

    Abstract: Background and aim: An accurate placement of spinal needle is crucial while injecting drugs to avoid a failed sub arachnoid block (SAB). A pre puncture estimation of skin to subarachnoid depth (SSD) may guide accurate spinal needle placement. This study is aimed to predict the depth from skin to subarachnoid space based on pre-operative anthropometric assessment and to find out formula for predicting SSD, best suited in terms of accuracy when compared with observed depth. Method: In this hospital based, prospective, observational study, a total of 120 patients, of which 68 were men, and 52 were women (non-pregnant) with a mean age of 59.75 ±14.1 kilograms. The SSD was measured after SAB was performed. This SSD was compared with the predicted SSD calculated using the Abe’s, Bonadio’s, Craig’s, Stocker’s, and Chong’s modified formulae. Analysis was done using unpaired t test for quantitative data to determine best suited formula to predict SSD in our population in terms of both accuracy and ease of application. Pearson correlation test was also done. Results: The observed SSD in the overall study population was 4.96 ± 0.67 cm. Stocker’s formula is closest with a mean difference of 0.17cm. In males the mean observed depth was (5.06±0.60 cm) more than that in the non-pregnant female population (4.83±0.74cm). These anthropometric variables were statistically significant with a presentation value of 〈 0.001. Conclusion: Among various formulae Stocker’s formula can most accurately predict the SSD when applied to Indian population.

    Abstract: Background and Aims: Laparoscopic cholecystectomy has emerged as a gold standard technique for gall bladder stones. The aim of the present study was to compare the analgesic effect of intravenous (IV) vs intraperitoneal (IP) dexmedetomidine as an adjuvant to intraperitoneal (IP) bupivacaine in laparoscopy. Methods: A prospective, randomized, double blind, interventional study was conducted on 100 patients undergoing laparoscopic cholecystectomy where they were divided into following 2 groups: Group A: Patients received IV 1µg/kg dexmedetomidine diluted to 30 ml with normal saline over 10 min and 40 ml of 0.125% bupivacaine IP after removal of gall bladder. Group B: Patients received IV 30 ml of normal saline and 1µg/kg IP dexmedetomidine in 40 ml of 0.125% IP bupivacaine after removal of gall bladder. The primacy outcome was noted as a difference in mean duration for need of first rescue analgesia. The total consumption of analgesic in first 24hours was recorded and compared between the two groups. Results: Both the groups were comparable in terms of demographic profile and intraoperative hemodynamic parameters with no statistical difference. Comparison of time to first analgesic requirement between the two groups showed statistically significant results with unpaired t test The time of first rescue analgesia in Group A was 151.80 min ± 76.624. and in Group B was 94.80min ± 21.499. The total analgesic requirement in 24 hours in Group A was 136.64 ± 31.251 and in Group B was 144.12 ± 21.49. Conclusion: In our study we concluded that intravenous dexmetomidine provided superior analgesia as compared to intraperitoneal dexmetomidine when used as an adjuvant with Bupivacaine intraperitoneally.
