Abstract: Background: Drug-resistant Tuberculosis (DR-TB) is challenging public health problem in countries with high tuberculosis prevalence and limited resources. Developing and applying the most appropriate and effective methods for diagnosing DR-TB from clinical samples is necessary, allowing a more rapid detection method for large-scale screening. Methods: Applying real-time PCR Taqman allelic discrimination with a PCR Taqman probes panel to identifying the DR-TB associated mutations in rpoB and katG of Mycobacterium Tuberculosis from isolates and clinical samples. Results: Comparing results of the real-time PCR allelic and DNA sequencing results, the sensitivity and specificity for Isoniazid resistance detection by analysing katG were found 95%(75.1 - 99.8) and 100%, Rifampicin resistance determining region (RRDR) of rpoB were found 95.5(77.16 - 99.88) and 100%, respectively. The real-time PCR TaqMan allelic discrimination also showed the sensitivities 100% for both katG and rpoB, and the specificities were 93.55% (78.58 - 99.21) for the rpoB and 93.94% (79.77 - 99.26) for the katG from clinical samples. Conclusions: This study showed that the real-time PCR taqman allelic discrimination assay is useful for detection of TB and DR-TB because of an accurate and rapid diagnosis in the early stages. Abstract: Background: Sepsis has a high risk of death from septic shock and dysfunction of many organs. Diagnosis and prognosis of patients in the early stage play a very important role, helping to reduce mortality and shorten the hospital stay of patients. Aims: Study some factors for severity and mortality of sepsis adult patients. Subjects and methods: 84 patients over 15 years old were diagnosed with sepsis, treated at the Department of Tropical Diseases and Intensive Care Unit, Hue Central Hospital during the period from April 2018 to June 2019. A cross-sectional study. Results: Lactate concentrations within 24 hours of admission had a good severe prognosis value with an AUC of 0.866 (p 〈 0.001). PCT concentrations within 24 hours of admission had a good prognosis value with an AUC of 0.866 (p 〈 0.001). Comorbidities, hospital infections, creatinine, PCT, blood lactate, and SOFA and APACHE II scores were factors associated with a severe prognosis in patients sepsis with an OR of 3.33 respectively; 5.39; 1.01; 1.05; 2.52; 4.16, and 1.36. SOFA and APACHE II scores at 24 hours post-admission had AUC of 0.982 and 0.876, respectively. The factors related to mortality are comorbidities, nosocomial infections and SOFA score at 24 hours after admission with OR 99.07, respectively; 7.66 and 2.02. Conclusion: Comorbidities, nosocomial infections, an increase in serum creatinine levels, an increase in PCT, blood lactate as well as increased SOFA and APACHE II scores are risk factors associated with severe prognosis in sepsis patients. The factors related to mortality are comorbidities, nosocomial infections and SOFA score at 24 hours after admission with OR 99.07, respectively; 7.66 and 2.02. Key words: Sepsis, prognostic factor.

Abstract: Introduction: Prenatal diagnosis of trisomy 21, 18 and 13 plays a very important role in the improving population quality. This study was aimed at (1) Identifying the prevalence of trisomy 21, 18 and 13 by QFPCR from amniotic cells of high-risk pregnancies; and (2) Evaluating the association between diagnosed trisomies and some characteristics of mother and fetus. Objectives and methods: 170 pregnant women with high risk of having trisomy 21, 18 or 13 fetuses during first trimester screening (gestation age from 11 weeks to 13 weeks 6 days). DNA was extracted from amniocytes for prenatal diagnosis using QF-PCR. Results: The prevalence of trisomies was 9.4%, among which trisomy 21 and trisomy 18 accounted for 68.8% and 31.2%, respectively; none of them was trisomy 13. There was the significant association between diagnosed trisomies and maternal age (cut-off 30.5 years old) and nuchal translucency thickness (cut-off 1.95 mm). MoM median of free β-hCG increased in trisomy 21 group (4.35, p = 0.021) and decreased in trisomy 18 group (0.13, p 〈 0.001) as compared to the non-trisomy group (2.28). MoM median of serum PAPP-A decreased in trisomy 18 group (0.14, p = 0.004) as compared to the non-trisomy group (0.54). Conclusion: Prenatal diagnosis by QF-PCR detected remarkable prevalence of fetuses with trisomy 21 và 18. There was the significant association between diagnosed trisomies and maternal age, nuchal translucency thickness, free β-hCG and serum PAPP-A. Key words: prenatal diagnosis, trisomy, QF-PCR

Abstract: Sepsis is a serious bacterial infection. The main treatment is using antibiotics. However, the rate of antibiotic resistance is very high and this resistance is related to the outcome of treatment. Objectives: To evaluate the situation of antibiotic resistance of some isolated bacteria in sepsis patients treated at Hue Central Hospital; to evaluate the relationship of antibiotic resistance to the treatment results in patients with sepsis. Subjects and methods: prospective study of 60 sepsis patients diagnosed according to the criteria of the 3rd International Consensus-Sepsis 3 and its susceptibility patterns from April 2017 to August 2018. Results and Conclusions: The current agents of sepsis are mainly S. suis, Burkhoderiae spp. and E. coli. E. coli is resistant to cephalosporins 3rd, 4th generation and quinolone group is over 75%; resistance to imipenem 11.1%; the ESBL rate is 60%. S. suis resistant to ampicilline 11.1%; no resistance has been recorded to ceftriaxone and vancomycine. Resistance of Burkholderiae spp. to cefepime and amoxicillin/clavulanic acid was 42.9% and 55.6%, resistant to imipenem and meropenem is 20%, resistance to ceftazidime was not recorded. The deaths were mostly dued to E. coli and K. pneumoniae. The mortality for patients infected with antibiotic-resistant bacteria are higher than for sensitive groups.
