Abstract:
Zusammenfassung. Lumbale Rückenschmerzen sind sehr häufig und in rund 90 % der Fälle unspezifisch. Ein einseitiger Fokus auf biomechanische Schmerzursachen führt zu unnötigen Abklärungen und Behandlungen, während die Relevanz des individuellen Umgangs mit Schmerzen unterschätzt wird. Liegt ein maladaptives Coping mit Angst und Vermeideverhalten vor, resultiert dies in Fehlbelastung, Dekonditionierung und einem Circulus vitiosus, der ein negatives Schmerzverhalten unterstützt. Konzepte, die psychische Faktoren wie Angst und Stress suchten und gezielt behandelten, waren effektiv. Es ist wichtig, dies in der hausärztlichen Praxis und auf Notfallstationen bei Patienten mit Rückenschmerzen ohne klare Radikulopathie oder andere Alarmzeichen zu berücksichtigen.
