    Abstract: This study is aimed at exploring the expression pattern and methylation level of G0S2 in the peripheral blood mononuclear cells (PBMCs) of myasthenia gravis (MG) patients with positive acetylcholine receptor (AChR) autoantibodies and revealing the relationship between the G0S2 methylation pattern and MG. The relationship between the NFAT family members and G0S2 was explored to reveal the regulatory mechanism of G0S2 in the pathogenesis and treatment of AChR MG. Moreover, we attempted to demonstrate the potential therapeutic mechanism of tacrolimus in AChR MG. The relative G0S2 expression level in the PBMCs of healthy people was compared with that in the PBMCs of AChR MG patients with quantitative real-time PCR (qRT-PCR). The methylation frequency of the G0S2 promoter was detected by bisulfite sequencing PCR (BSP) and pyrosequencing. A dual-luciferase reporter system was used to reveal the relationship between the G0S2 promoter and nuclear factor of activated T cells 5 (NFAT5). The qRT-PCR results showed that G0S2 expression was significantly upregulated in the B cells and CD8+ T cells of AChR MG patients but not in the CD4+ T cells, and these expression differences were significantly associated with a decrease in G0S2 methylation. NFAT5, which was speculated to bind to island 1 (p1) in the G0S2 promoter, may regulate the lymphocyte balance by regulating G0S2 gene expression but failed to affect the methylation of the G0S2 promoter. Tacrolimus therapy-induced methylation and overexpression of NFAT5 could significantly reduce the expression of G0S2 in AChR MG patients. The G0S2 gene was remarkably upregulated in the PBMCs of MG patients. NFAT5 may affect transcription initiation and downregulate G0S2 expression through p1 in the promoter, thus controlling G0S2 gene expression and regulating the lymphocyte balance. Therefore, G0S2 could be an immune regulatory factor in both AChR MG occurrence and treatment with tacrolimus.

    Abstract: BRCA1 and BRCA2 as important DNA repair genes have been thoroughly investigated in abundant studies. The potential relationships of BRCA1/2 pathogenic variants between multicancers have been verified in Caucasians but few in Chinese. In this study, we performed a two-stage study to screen BRCA1/2 pathogenic variants or variants of uncertain significance (VUS) with 7580 cancer cases and 4874 cancer-free controls, consisting of a discovery stage with 70 familial breast cancer cases and a subsequent validation stage with 7510 cases (3217 breast cancer, 1133 cervical cancer, 2044 hepatocellular carcinoma, and 1116 colorectal cancer). 48 variants were obtained from 70 familial breast cancer cases after BRCA1/2 exon detection, and finally, 20 pathogenic variants or VUS were selected for subsequent validation. Four recurrent variants in sporadic cases ( BRCA1 c.4801A 〉 T, BRCA1 c.3257del, BRCA1 c.440del, and BRCA2 c.7409dup) were identified and three of them were labeled Class 5 by ENIGMA. Two variants ( BRCA1 c.3257del and c.440del) were specific in breast cancer cases, while BRCA2 c.7409dup and c.4307T 〉 C were detected in two hepatocellular carcinoma patients and the BRCA1 c.4801A 〉 T variant in one cervical cancer patient, respectively. Moreover, BRCA1 c.3257del was the most frequent variant observed in Chinese sporadic breast cancer and showed increased proliferation of BRCA1 c.3257del -overexpressing triple-negative breast cancer cell lines (MDA-MB-231) in vitro. In addition to the known founder deleterious mutations, our findings highlight that the recurrently pathogenic variants in breast cancer cases could be taken as candidate genetic screening loci for a more efficient genetic screening of the Chinese population.

    Abstract: Special protective clothing should have a certain thermal insulation and protective effect in the production in front of the blast furnace. In order to study the design of special protective clothing for blast furnace foreman, its temperature distribution and internal thickness structure are deeply analyzed. First, based on the existing experimental data, combined with heat transfer and the Fourier law theory, a one-dimensional unsteady heat conduction equation is established. Using the explicit finite difference solution algorithm, the distribution law of the temperature of each fabric layer with time is obtained and the error analysis is performed. The error result is 0.42877%, which shows that the numerical solution is stable and convergent. Then, through the intelligent optimization model of a simulated annealing ant colony (SA-ACO), compared with the single ant colony algorithm (ACO) and a simulated annealing algorithm (SA), it avoids falling into the local optimal solution, reversely solves the optimal thickness of two layers of protective clothing in front of the blast furnace, improves the convergence speed of optimization, and verifies the reliability of the model with a sensitivity test. Finally, this paper actually solves the problem of the protective clothing design and mathematical intelligent model in front of the blast furnace and combines it with numerical simulation software to realize the numerical simulation of temperature distribution and thickness optimization of special protective clothing in front of the blast furnace, so as to provide theoretical guidance for enterprise blast furnace production and development of special protective clothing.

    Abstract: The filling material of the karst collapse column (KCC) is easy to be activated by mining. During this process, the mechanical properties of KCC fillings change, and its water resisting capacity constantly deteriorates and thus often leads to water inrush disaster. In this study, the samples of KCC fillings were taken on-site and then were remolded by the consolidation drainage method. The variation laws of the compressive strength, tensile strength, cohesive stress, internal friction angle, and permeability of the filling samples with respect to the consolidation pressure and moisture content were tested and analyzed. Based on an engineering example, the yield and activation and particle loss of the filling material of the KCC are analyzed. A mechanism for the lagging water inrush of KCC in the process of mining is proposed. The main results of the present study can be concluded concisely as follows. (1) The KCC fillings show obvious soft rock characteristics in the process of uniaxial compression and Brazilian split. The ratio of the uniaxial compressive strength to splitting tensile strength is between 12 : 1 and 8 : 1. The larger the consolidation pressure or the smaller the moisture content, the larger the ratio. (2) With the increase of consolidation pressure or the decrease of moisture content, the uniaxial compressive strength, elastic modulus, splitting tensile strength, cohesive stress, and internal friction angle of the filling material of the KCC increase linearly, while its permeability increases exponentially. (3) When the crack field of the surrounding rocks of the stope is connected with the KCC, its filling material will continue to yield, activate, and migrate under the fluid-solid coupling effect and finally result in the lagging water inrush from the KCC.

    Abstract: This retrospective cohort study was conducted to explore the effect of traditional Chinese medicine (TCM) therapy on the long-term trends in CD4+ T-cell count among patients with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) who were treated with combined antiretroviral therapy (cART) over a 14-year period. A total of 721 individuals were treated with cART alone (cART group), and 307 individuals were treated with both cART and TCM (TCM + cART group). Among all enrolled patients with HIV/AIDS, 99.5% were farmers, 71.1% had more than 6 years of education, and 96.8% were infected with HIV via a paid blood donation. For those patients with HIV/AIDS who had a baseline CD4+ T-cell count of 〈 350 cells/mL, the CD4+ T-cell count tended to increase to approximately 350 cells/mL more rapidly in the TCM + cART group than in the cART group, but when the baseline CD4+ T-cell count was ≥350 cells/mL, there was no difference between the cART and TCM + cART groups. For other patients with HIV/AIDS who had a baseline CD4+ T-cell count of 350–500 cells/mL, the CD4+ T-cell counts tended to increase slightly, but there was no difference between the two groups. For patients with HIV/AIDS who had a baseline CD4+ T-cell count of ≥500 cells/mL, the CD4+ T-cell counts tended to be maintained at a particular level, with no difference between the two groups. The results show that the effect of TCM on the CD4+ T-cell counts of patients with HIV/AIDS is related to the CD4+ T-cell level at the time of initial treatment. TCM can increase the CD4+ T-cell count among patients with HIV/AIDS who have a baseline CD4+ T-cell count of 〈 350 cells/mL. Sex and age have a slight influence on the therapeutic effect of TCM.

    Abstract: Conceptual models are built to depict and analyze complex systems. They are made of concepts and relationships among these concepts. In a particular domain, conceptual models are helpful for different stakeholders to reach a clear and unified view of domain problems. However, the process of building conceptual models is time-consuming, tedious, and expertise required. To improve the efficiency of the building process, this paper proposes a configurable semantic-based (semi-) automatic conceptual model transformation methodology (SbACMT) that tries to reuse existing conceptual models to generate new models. SbACMT contains three parts: (i) a configurable semantic relatedness computing method building on the structured linguistic knowledge base “ConceptNet” (SRCM-CNet), (ii) a specific meta-model, which follows the Ecore standard, defines the rules of applying SRCM-CNet to different conceptual models to automatically detect transformation mappings, and (iii) a multistep matching and transformation process that employs SRCM-CNet. A case study is carried out to detail the working mechanism of SbACMT. Furthermore, through a systematically analysis of this case study, we validate the performance of SbACMT. We prove that SbACMT can support the automatic transformation process of conceptual models (e.g., class diagrams). The scalability of SbACMT can be improved by adapting the meta-model and predefined syntax transformation rules.

    Abstract: Background . Aidi injection (ADI) is being used widely for breast cancer in China. However, the efficacy and safety of it need to be summarized. We conducted a systematic review and meta-analysis to compare ADI and non-ADI treatment for advanced breast cancer. Methods . We searched PubMed, EMBASE, CNKI, SinoMed, and CENTRAL from inception to Jan 2020 for randomized controlled trials (RCTs) with diagnosis of advanced breast cancer that compared the efficacy of ADI with non-ADI treatment. Two researchers screened the literature, extracted data, and evaluated risk of bias separately. The primary outcomes were overall response rate (ORR) and disease control rate (DCR). The secondary outcomes included the QOL, immune cells, and adverse events. Review Manager software was used for estimating risks of bias of included studies, data analysis, and plotting. The sensitivity analysis and the publication bias test were performed using the R language. I 2 and chi-square tests were used to estimate heterogeneity. If P 〉 0.1 or I 2   〈  40%, the fixed-effect model meta-analysis was performed. A random or fixed-effect analysis was used depending on the heterogeneity testing. Weighted mean difference (WMD) or standard mean difference (SMD) was used for analysis of continuous data, and the rate ratio (RR) was calculated for the dichotomous variable, respectively. Results . We included 14 studies with 1006 patients diagnosed as advanced breast cancer in total. The pooled effect showed that ADI increased ORR in advanced BC patients as an add-on therapy with little heterogeneity ( RR  = 1.14, 95% CI 1.03–1.27). DCR in BC patients could not be improved by ADI. ADI improved the KPS score in BC patients compared with chemotherapy alone (MD = 3.26, 95% CI 1.74–4.78). There were no improvements on immune markers except CD4/CD8 and NK%. Serum tumor markers CEA and CA153 were decreased while treated with ADI, but only one trial was involved. ADI decreased the numbers of myelosuppression in advanced BC patients, and AST, ALT, γ -GT, and CK-MB were all decreased. The sensitivity evaluation indicated that the result of the pooled effect size had good stability. Conclusion . This meta-analysis suggested that based on the existing evidence, treatment with ADI significantly changed the ORR of patients with advanced BC and improved their quality of life with few side effects. However, more randomized trials involving larger samples should be considered, and detailed mechanisms are needed to be uncovered.

    Abstract: An effect of guide-vane numbers on pressure fluctuations and structural vibroacoustics induced by unsteady flow is performed by a hybrid numerical method. A 3D flow field is simulated in axial-flow pump with four impeller blades, in which three diffuser models with 5, 7, and 9 vanes are devised to match, respectively. A full scale structural vibroacoustics coupled model is solved using LMS acoustics software. The results show that the blade-passing frequency (BPF) is dominated frequency of the vibration acceleration of pump, which is consistent with frequency spectral characteristics of pressure pulsation. The unsteady pressure fluctuating becomes strong as the flow discharge decreases from 1.0Qv to 0.6Qv, the circumferential unsteady behavior of which is more severe due to flow nonuniformity induced by the suction elbow at partial operation. Generally, the pressure fluctuating increases slightly when the flow discharge increases from 1.0Qv to 1.3Qv. Moreover, pressure fluctuations amplitude on the pump with 9-vane diffuser is small relative to other two models and the vibrating accelerating and radiation sound field at BPF are also slight relatively, which indicates that appropriate guide-vane numbers contribute to suppress pressure fluctuations and vibroacoustics in axial-flow pump. The conclusions in the present paper can provide theoretical guidance for low vibration pump design.
