    Abstract: The consolidation of soil is one of the most common phenomena in geotechnical engineering. Previous studies for the axisymmetric consolidation of unsaturated soil have usually idealized the boundary conditions as fully drained and absolutely undrained, but the boundaries of unsaturated soil are actually impeded drainage in most practical situations. In this study, we present a general analytical solution for predicting the axisymmetric consolidation behavior of unsaturated soil that incorporates impeded drainage boundary conditions in both the radial and vertical directions simultaneously. The impeded drainage boundary is modeled using the third kind boundary, and it can also realize fully drained and absolutely undrained ones by changing the drainage parameter. A general analytical solution is developed to predict the excess pore-air and pore-water pressures as well as the average degree of consolidation in an unsaturated soil stratum using the common methods of eigenfunction expansion and Laplace transform. The newly developed solution is expressed in the product of the terms of time, depth, and radius, which are derived using Laplace transform, usual Fourier, and Fourier-Bessel series, respectively. The eigenfunctions and eigenvalues are evaluated from the impeded drainage boundaries in both radial and depth dimensions. Then, the correctness of the proposed analytical solution is verified against the existing analytical solution for the case of traditional boundaries and against the finite difference solution for the case of general impeded drainage boundaries, and excellent agreements are obtained. Finally, the axisymmetric consolidation behavior of unsaturated soil involving impeded drainage boundaries is demonstrated and analyzed, and the effects of the drainage parameters are discussed. The results indicate that the larger drainage parameter generally expedites the dissipations of the excess pore pressures and further promotes the soil settling process. As the drainage parameter increases, its influence gradually diminishes and even can be neglected when it is larger than 100. The general analytical solution and findings of this study can help for better understanding the axisymmetric consolidation behavior of the unsaturated soil stratum in the ground improvement project with vertical drains as well as the gas-oil gravity drainage mechanism in the naturally fractured reservoirs.

    Abstract: Objective . DC-CIK therapy included DC-CIK cells and Ag-DC-CIK cells. To further confirm whether DC-CIK reconstructs the antitumor immunity and improves the tumor responses and reveals its optimal usage and combination with chemotherapy, we systematically reevaluated all the related studies. Materials and Methods . All studies about DC-CIK plus chemotherapy for NSCLC were collected from the published and ongoing database as CBM, CNKI, VIP, Wanfang, ISI, Embase, MEDLINE, CENTRAL, WHO-ICTRP, Chi-CTR, and US clinical trials (established on June 2017). We evaluated their methodological bias risk according to the Cochrane evaluation handbook of RCTs (5.1.0), extracted data following the predesigned data extraction form, and synthesized the data using meta-analysis. Results . We included 28 RCTs (phase IV) with 2242 patients, but most trials had unclear bias risk. The SMD and 95% CI of meta-analysis for CD3 + T cells, CD3 + CD4 + T cells, CD3 + CD8 + T cells, CD4 + /CD8 + T cell ratio, CIK cells, NK cells, and Treg cells were as follows: 1.85 (1.39 to 2.31), 0.87 (0.65 to 1.10), 1.04 (0.58 to 1.50), 0.75 (0.27 to 1.22), 3.87 (2.48 to 5.25), 1.51 (0.99 to 2.03), and −2.31(−3.84 to −0.79). The RR and 95% CI of meta-analysis for ORR and DCR were as follows: 1.38 (1.24 to 1.54) and 1.27 (1.20 to 1.34). All differences were statistically significant between DC-CIK plus chemotherapy and chemotherapy alone. Subgroup analysis showed that only DC-CIK cells could increase the CD3 + T cells, CD3 + CD4 + T cells, CD3 + CD8 + T cells, and CD4 + /CD8 + T cell ratio. In treatment with one cycle or two cycles and combination with NP or GP, DC-CIK could increase the CD4 + /CD8 + T cell ratio. All results had good stability. Conclusions . DC-CIK therapy can simultaneously improve the antitumor immunity and tumor responses. DC-CIK therapy, especially DC-CIK cells, can improve antitumor immunity through increasing the T lymphocyte subsets, CIK cell, and NK cells in peripheral blood. The one cycle to two cycles may be optimal cycle, and the NP or GP may be optimal combination.

    Abstract: Phyllanthus niruri L., a well-known medicinal plant, has been used as a folk antitumor remedy in the worldwide scale. However, the antitumor components in P. niruri have not been reported. In order to verify the antitumor components of P. niruri and the plants which have the high content of these components, we isolated the antitumor components with bioguided fraction and isolation, by different chromatographic methods from the ethyl acetate fraction of P. niruri. , and identified them as ethyl brevifolincarboxylate and corilagin by 1 H-NMR, 13 C-NMR, 2D-NMR, and mass spectrometric analyses. Cell cytotoxicity assays showed that corilagin has broad-spectrum antitumor activity, a better antitumor potential, and lower toxicity in normal cells. Besides, the coefficient of drug interaction (CDI) of 10  μ M corilagin and 20  μ M cDDP reached up to 0.77, which means corilagin can promote the antitumor activity of cDDP. Furthermore, by the extensive screening among 10 species of plants reported to contain corilagin, we found that Dimocarpus longan Lour. has the maximum content of corilagin. In conclusion, corilagin is the major active antitumor composition in P. niruri. L. on HCC cells and has high content in D. longan .

    Abstract: Objective. This study aims to evaluate the efficacy of various conventional synthetic DMARDs, including Tripterygium wilfordii Hook F (TwHF) for treating rheumatoid arthritis (RA) by network meta-analysis. Methods. We retrieved the related literature from online databases and supplemented it by using a manual retrieval method. Data was extracted from the literature and analyzed with STATA software. Results. A total of 21 trials (5,039 participants) were identified. Assessment of ACR20 response found that TwHF combined with methotrexate (MTX) had the greatest probability for being the best treatment option among the treatments involved, while TwHF used singly was second only to TwHF combined with MTX. Assessment of ACR50 response found that TwHF combined with MTX ranked second in all treatment options after cyclosporine A (CsA) combined with leflunomide (LEF) and TwHF alone, followed by TwHF combined with MTX. Assessment of ACR70 response found that CsA combined with LEF ranked first, TwHF combined with LEF ranked second, TwHF combined with MTX ranked third, and TwHF used singly ranked fourth. In the safety analysis, TwHF had the least probability of adverse event occurrence, followed by TwHF combined with MTX, which ranked first and second, respectively. Conclusion. Compared with the current csDMARDs for treating RA, the efficacy of TwHF was clear, and TwHF combined with MTX performed well under various endpoints. In the future, large, rigorous, and high-quality RCTs are still needed to confirm the benefits of TwHF therapy on RA.

    Abstract: Background. Ascites is a common clinical finding caused by many different diseases, so we developed a technique termed single orifice percutaneous endoscopic surgery (SOPES) which can access peritoneal cavity through the contralateral McBurney’s point or umbilicus to seek the underlying causes. In this study, we describe the initial clinical experience of SOPES and compare the application of two accesses. Methods. This is a retrospective study performed between 2007 and 2018. Patients with ascites of unknown origin who underwent these two kinds of SOPES were included. Main outcomes were measured by diagnostic accuracy, complication rate, procedure time, time till stitches removal, length of hospital stay, and hospital cost. Results. 148 patients successfully undergone SOPES via the contralateral McBurney’s point (IM group, n = 70 ) or the umbilicus (UM group, n = 78 ). 63 patients in the IM group and 71 patients in the UM group reached clear diagnosis (90.0% vs. 91.0%, p = 0.831 ). The overall complication rate was 5.4%, while the UM group was higher than the IM group (10.3% vs. 0%, p = 0.017 ). All complications were resolved after medical treatment, and no mortality resulted from this procedure. The procedure time and the time until stitches removal in the UM group were longer than that in the IM group. There were no significant differences in length of hospital stay and hospital cost between the two groups. Conclusions. SOPES, which combines the strength of minimally invasive single orifice incision and flexible angles of examination and instrumentation, is a newly developed flexible endoscopic surgical modality that provides new important clinical valuable in evaluation of ascites of unknown origin. Moreover, SOPES via the contralateral McBurney’s point was safer than the umbilicus approach.

    Abstract: Hierarchical MnO 2 microfibers were prepared by using cotton as the template and KMnO 4 as the precursor via an ultrasonic assistance route. The results of scanning electron microscope characterization showed that the concentration of KMnO 4 had a significant effect on the morphology of MnO 2 microfiber. At low concentration of KMnO 4 , the microfiber was composed of MnO 2 nanorods with single crystal structure. With increasing the concentration of KMnO 4 , the secondary nanostructure of MnO 2 microfibers had a transformation from nanorod to nanoparticle. The results of N 2 adsorption-desorption analysis indicated that MnO 2 microfibers had BET surface area of about 25  m 2 /g. This synthesis provides a new way to control the secondary nanostructure of MnO 2 microfiber by adjusting the concentration of precursor. Furthermore, the mechanism for the replication was proposed and discussed.

    Abstract: Background. It is controversial and unclear how N-stage would increase the risk of incidence of hypothyroidism (HT) for patients with nasopharyngeal carcinoma (NPC) after radiotherapy. Our study aimed to explore the correlation between cervical lymph node metastasis and the incidence of HT in NPC. Materials and Methods. A total of 206 patients with NPC treated at the Cancer Hospital of University of Chinese Academy of Sciences, and their clinical information were retrospectively collected. A series of univariate logistic regression models were performed to explore the association of clinical and lymph node indices with the development of HT. Significant features in univariate analysis were then used to construct three prediction models, for HT prediction using multivariate logistic regression based on Bayesian information criterion. Prediction performance of those models was measured by area under the receiver operating characteristic curve (AUC) using 10-fold cross-validation. Results. A total of 111 patients developed HT, and the incidence of HT in N2–3 and N0–1 patients was 58.82% and 44.29%, respectively. Compared to Model 1 (consisted of pretreatment TSH concentration, thyroid volume, and N-stage) whose AUCs were 0.801 and 0.766 in training and validation sets, with N-stage be replaced by shortest distance from thyroid, Model 2 achieved more stable AUCs of 0.824 and 0.801. While with numbers of positive lymph nodes in Level IIb additionally added, Model 3 improved its AUCs to 0.841 and 0.813. Conclusion. The shortest distance between the lymph nodes and thyroid gland and the number of lymph nodes in IIb are better predictors of radiation-induced HT than the N-stage.

    Abstract: Background . Alanine aminotransferase (ALT) is referred as liver transaminase and predominantly expressed by hepatocytes. Previous evidences showed that high levels of ALT were reversely associated with short- and long-term outcomes in patients with myocardial infarction. Besides, low lymphocyte has been demonstrated to be significantly correlated with adverse clinical outcomes in coronary artery disease (CAD). However, evidences about the relationship between ALT-to-lymphocyte ratio (ALR) and outcomes in CAD patients with normal liver function are limited. The aim of this study was to assess the relationship between ALR and clinical outcomes in patients with CAD. Methods . This is a retrospective cohort study, and a total of 3561 patients were enrolled in Clinical Outcomes and Risk Factors of Patients with CAD after percutaneous coronary intervention (PCI), from January 2013 to December 2017. After excluding patients with liver dysfunction, we finally enrolled 2714 patients. These patients were divided into two groups according to ALR value: the lower group (ALR  〈  14.06, n  = 1804) and the higher group (ALR ≥ 14.06, n  = 910). The average follow-up time was 37.59 ± 22.24 months. Results . We found that there were significant differences between the two groups in the incidence of all-cause mortality (ACM) ( P 〈 0.001 ) and cardiac mortality (CM) ( P = 0.010 ). Kaplan–Meier survival analysis suggested that CAD patients with higher ALR tended to have an increased accumulated risk of ACM and CM (log rank P 〈 0.001 and P = 0.006 , respectively). Multivariate Cox regression analysis showed that ALR was an independent predictor of ACM (hazard ratio (HR) = 2.017 (95% confidence interval (CI): 1.289–3.158), P = 0.002 ) and CM (HR = 1.862 (95% CI: 1.047–3.313), P = 0.034 ). We did not find significant difference in the incidence of major adverse cardiovascular events (MACEs) and major adverse cardiovascular and cerebrovascular events (MACCEs) between the two groups after adjustments of confounders. Conclusion . Our results indicate that ALR is an independent predictor of long-term adverse outcomes in CAD patients who underwent PCI.

    Abstract: Background. The aim of the study was to investigate the association between single nucleotide polymorphism (SNP) of vitamin D receptor (VDR) gene and clinical progress of benign prostatic hyperplasia (BPH) in Chinese men. Methods. The DNA was extracted from blood of 200 BPH patients with operation (progression group) and 200 patients without operation (control group), respectively. The genotypes of VDR gene FokI SNP represented by “F/f” were identified by PCR-restriction fragment length polymorphism. The odds ratio (OR) of having progression of BPH for having the genotype were calculated. Results . Our date indicated that the f alleles of the VDR gene FokI SNP associated with the progression of BPH ( P = 0.009 ). Conclusion . For the first time, our study demonstrated that VDR gene FokI SNP may be associated with the risk of BPH progress.
