    Abstract: Objective. To investigate the effect of intensive management and achieving the target control more than 3 times on endpoint events during 9 consecutive years’ annual assessment in type 2 diabetes (T2DM) patients in the Sanlitun Community Health Service Center in Beijing, including blood glucose, blood pressure, lipids profiles, and the joint target control. Methods. In Beijing Community Diabetes Study (BCDS), 224 patients with T2DM from the Sanlitun Community Health Service Center were enrolled in 2008. All patients were randomly assigned to the intensive management group ( n  = 113) and the standard management group ( n  = 111). All patients were followed up for nine consecutive years from January 2009 to December 2017. Systolic blood pressure (SBP), diastolic blood pressure (DBP), glycosylated hemoglobin (HbA1c), and low-density lipoprotein cholesterol (LDL-C) were detected as the main indexes, and the endpoint events were also carried out at the same time. The endpoint events were analyzed by using survival analysis (Kaplan–Meier method) based on management grouping and whether achieving the target control more than 3 times or not. Results. During the nine-year follow-up, the abscission number was 35 (14.29%), among which 14 (12.39%) was in the intensive management group and 21 (18.92%) was in the standard management group. The incidence of diabetic retinopathy (6 cases, 5.41%) and diabetic nephropathy (13 cases, 11.71%) in the standard management group was significantly higher than that in the intensive management group (1 case, 0.88%; 5 cases, 4.42%), respectively ( P 〈 0.05 ). However, there were no significant differences on the other endpoint events between the two groups ( P 〉 0.05 ). All-cause death was 23 cases, in which patients who achieved the target control (HbA1c and LDL-C) and the joint target control more than 3 times were significantly lower than that of less than 3 times ( P 〈 0.05 ). As far as death caused by cardiovascular events, cerebrovascular events, and newly onset coronary heart disease are concerned, there were no significant differences on the aforementioned endpoint events between the two groups based on target control more than 3 times or not ( P 〉 0.05 ). There were less incidence of new onset cerebrovascular events, stenosis or occlusion of large arteries, and diabetic microvascular complications in patients who achieved target control (HbA1c and LDL-C) and the joint target control more than 3 times than those with target control less than 3 times ( P 〈 0.05 ). Conclusions. The intensive management can effectively reduce the occurrence of microvascular complications. The incidence of all-cause death and the other endpoint events decreased in T2DM patients who achieved the joint target control more than 3 times during the nine-year management, which improved survival time and life quality. This trial is registered with ChiCTR-TRC-13003978 and ChiCTR-OOC-15006090 .

    Abstract: Saffron, which has many kinds of biological activities, has been widely used in medicine, cosmetics, food, and other fields of health promotion industries. Crocins are the main component of saffron ( Crocus sativus L.). At present, most of the extraction methods for crocins require long time or special instruments to complete the process and some of them are not suitable for industrial production at present. In this article, homogenate extraction technology which is a convenient and efficient method was developed for crocins extraction from saffron. Firstly, the influences of extraction voltage, extraction time, ethanol concentration, and temperature on crocins yield were studied by single factor experiments; and then response surface methodology (RSM) was used to optimize levels of four variables based on the result of single factor experiments. Results showed that the optimum extraction process conditions for crocins were as follows: extraction voltage, 110 V; ethanol concentration, 70%; extraction temperature, 57°C; and extraction time, 40 s. Based on these conditions, the extraction yield of crocins can reach 22.76% which is higher than ultrasonic extraction method. Therefore, homogenate extraction is an effective way to extract crocins from saffron with higher extraction yield and shorter extraction time.

    Abstract: Cervical cancer is one of the top lethal malignancies among women worldwide. The current treatment methods have so many drawbacks that new treatment methods need to be developed. Zhidai Decoction (ZDD) is an effective traditional Chinese herbal formulation for gynecological diseases. Its main effect is controlling abnormal leucorrhea which is a typical early clinical manifestation of cervical cancer. However, how ZDD directly affects cervical cancer has not been addressed. In this study, we established a mouse cervical cancer U14 cell subcutaneous transplantation tumor model and took an early intervention with ZDD to evaluate the antitumor effect of ZDD. In addition, we also investigated the regulatory effects of ZDD on the vaginal microbiota using 16S rRNA analysis in this study. Our results showed that ZDD can significantly improve systemic symptoms and reduce vaginal secretions of tumor-bearing mice. Compared with the CCM group (the cervical cancer model group), in the ZDD-treated group, the tumor inhibitory rate was 37.90%, the average daily food intake of mice was increased to 5.27 ± 0.74 g ( P 〈 0.05 ), and the survival time was obviously prolonged to 21 days ( P 〈 0.05 ). Analysis of the sequencing results of 16S rRNA showed that the main microbial genera of the CCM group were Pasteurella (27.20%) and Helicobacter (18.50%), while those in the ZDD group were Staphylococcus (13.22%) and Lactobacillus (4.68%). It revealed that ZDD has the effect of regulating the vaginal microbiota of cervical cancer, especially in increasing the relative abundance of Lactobacillus and Staphylococcus and decreasing the relative abundance of Pasteurella and Helicobacter . The analysis also showed that ZDD could adjust microbiota structure, species abundance, and community compositions of vaginal microbiota. In conclusion, ZDD displayed inhibitory effect on cervical cancer, and it might be based on restoring the balance of vaginal microbiota. Furthermore, our conclusion supports the promotion of ZDD in the early treatment of cervical cancer.

    Abstract: The Footscan platform is a useful tool for plantar pressure measurement. However, there is still controversy over whether or not the platform should be covered by top-layer during the test. This study was designed to compare the reliability of the Footscan platform and identify the differences of the foot loading parameters between without top-layer (WOT) and with top-layer (WT) protocols. Measurements were taken from thirty-two healthy subjects. Participants were tested with a Footscan platform using the WOT and WT protocols. Three trials were performed during two separate testing sessions with a 7-day interval. Peak pressure, contact time, contact area, and pressure-time integral at ten foot zones were recorded and calculated for intra- and intersession reliability using intraclass correlation coefficients (ICCs) and coefficients of variation (CVs). The reliability and values of the analyzed parameters for the two protocols were compared. Both protocols produced a moderate to good level of intra- and intersession reliability. Compared with the WT protocol, the WOT protocol showed higher ICCs, lower CVs, and higher values in most of the parameters analyzed. The results suggest that the WOT protocol showed better reliability than the WT protocol. We recommend not using the top-layer when performing the plantar pressure test.

    Abstract: Adrenocortical carcinoma (ACC) is a rare malignant tumor with poor prognosis. Ferroptosis, a new form of cell death, differs from other forms of cell death and plays a vital role in tumor progress. Our study aimed to establish a ferroptosis-related signature with prognostic value in ACC. RNA-seq data and corresponding clinical characteristics for ACC were downloaded from TCGA and GEO databases. Genes included in ferroptosis risk signature were assessed by univariable and multivariable Cox regression analysis as well as lasso regression analysis. The prognostic value of the ferroptosis risk signature was assessed using K-M and ROC curves. Furthermore, we performed GSEA to discover the enriched gene sets in high-risk group. Additionally, TIMER website was applied to detect a possible connection between the signature and immune cells infiltration. ssGSEA was performed to evaluate scores of immune cells and immune-related pathways in two groups. A ferroptosis signature comprised of six genes (SLC7A11, TP53, HELLS, ACSL4, PCBP2, and HMGB1) was constructed to predict prognosis and reflect the immune infiltration in ACC. Patients in high-risk group were inclined to have worse prognosis. The ferroptosis model performed well in predicting prognosis and could be served as an independent indicator in ACC. GSEA revealed that gene sets correlated with biological processes including cell cycle, DNA replication, base excision repair, and P53 signaling pathway were highly enriched in high-risk group. In addition, we discovered that the expressional levels of hub genes were linked to six immune cells’ infiltration in ACC tumor. ssGSEA revealed that contents of most immune cells significantly decreased in the high-risk group. In conclusion, the novel ferroptosis risk signature could be useful in predicting prognosis and reflecting immune infiltration in ACC. It also brings us new insights into the possible value of targeting ferroptosis during the therapy of ACC.

    Abstract: Adrenocortical carcinoma (ACC) is a rare malignancy with dismal prognosis. Hypoxia is one of characteristics of cancer leading to tumor progression. For ACC, however, no reliable prognostic signature on the basis of hypoxia genes has been built. Our study aimed to develop a hypoxia-associated gene signature in ACC. Data of ACC patients were obtained from TCGA and GEO databases. The genes included in hypoxia risk signature were identified using the Cox regression analysis as well as LASSO regression analysis. GSEA was applied to discover the enriched gene sets. To detect a possible connection between the gene signature and immune cells, the CIBERSORT technique was applied. In ACC, the hypoxia signature including three genes (CCNA2, COL5A1, and EFNA3) was built to predict prognosis and reflect the immune microenvironment. Patients with high-risk scores tended to have a poor prognosis. According to the multivariate regression analysis, the hypoxia signature could be served as an independent indicator in ACC patients. GSEA demonstrated that gene sets linked to cancer proliferation and cell cycle were differentially enriched in high-risk classes. Additionally, we found that PDL1 and CTLA4 expression were significantly lower in the high-risk group than in the low-risk group, and resting NK cells displayed a significant increase in the high-risk group. In summary, the hypoxia risk signature created in our study might predict prognosis and evaluate the tumor immune microenvironment for ACC.

    Abstract: To determine the force mechanism for the steel plate shear wall with slits, the pushover analysis method was used in this study. An estimated equation for the lateral bearing capacity which considered the effect of edge stiffener was proposed. A simplified elastic-plastic analytical model for the stiffened steel slit wall composed of beam elements was presented, where the effects of edge stiffeners were taken into account. The wall-frame analysis model was established, and the geometric parameters were defined. Pushover analysis of two specimens was carried out, and the analysis was validated by comparing the results from the experiment, the shell element model, and a simplified model. The simplified model provided a good prediction of the lateral stiffness and the strength of the steel slit wall, with less than 10% error compared with the experimental results. The mutual effects of the bearing wall and the frame were also predicted correctly. In the end, the seismic performance evaluation of a steel slit wall-frame structure was presented. The results showed that the steel slit wall could prevent the beams and columns from being damaged by an earthquake and that the steel slit wall was an efficient energy dissipation component.

    Abstract: ZHENG is the key theory in traditional Chinese medicine (TCM) and it is very important to find the molecular pharmacology of traditional Chinese herbal formulae. One ZHENG is related to many diseases and the herbal formulae are aiming to ZHENG. Therefore, many herbal formulae whose effects on a certain disease have been confirmed might also treat other diseases with the same ZHENG. In this study, the microarrays collected from patients with QiXuXueYu ZHENG (Qi-deficiency and Blood-stasis syndrome) before treatment and after being treated with Fuzheng Huayu Capsule were analyzed by a high-throughput gene microarrays-based drug similarity comparison method, which could find the small molecules which had similar effects with Fuzheng Huayu Capsule. Besides getting the results of anti-inflammatory and anti-fibrosis drugs which embody the known effect of Fuzheng Huayu Capsule, many other small molecules were screened out and could reflect other types of effects of this formula in treating QiXuXueYu ZHENG, including anti-hyperglycemic, anti-hyperlipidemic, hyposenstive effect. Then we integrated this information to display the effect of Fuzheng Huayu Capsule and its potential multiple-target molecular pharmacology. Moreover, through using clinical blood-tested data to verify our prediction, Fuzheng Huayu Capsule was proved to have effects on diabetes and dyslipidemia.

    Abstract: Traditional Chinese medicine (TCM) treatment is based on the traditional diagnose method to distinguish the TCM syndrome, not the disease. So there is a phenomenon in the relationship between TCM syndrome and disease, called Same TCM Syndrome for Different Diseases and Different TCM Syndrome for Same Disease. In this study, we demonstrated the molecular mechanisms of this phenomenon using the microarray samples of liver-gallbladder dampness-heat syndrome (LGDHS) and liver depression and spleen deficiency syndrome (LDSDS) in the chronic hepatitis B (CHB) and liver cirrhosis (LC). The results showed that the difference between CHB and LC was gene expression level and the difference between LGDHS and LDSDS was gene coexpression in the G-protein-coupled receptor protein-signaling pathway. Therein genes GPER, PTHR1, GPR173, and SSTR1 were coexpressed in LDSDS, but not in LGDHS. Either CHB or LC was divided into the alternative LGDHS and LDSDS by the gene correlation, which reveals the molecular feature of Different TCM Syndrome for Same Disease. The alternatives LGDHS and LDSDS were divided into either CHB or LC by the gene expression level, which reveals the molecular feature of Same TCM Syndrome for Different Diseases.

    Abstract: Activating transcription factor-(ATF-) 3, a stress-inducible transcription factor, is rapidly upregulated under various stress conditions and plays an important role in inducing cancer cell apoptosis. NBM-TP-007-GS-002 (GS-002) is a Taiwanese propolin G (PPG) derivative. In this study, we examined the antitumor effects of GS-002 in human hepatoma Hep3B and HepG2 cells in vitro . First, we found that GS-002 significantly inhibited cell proliferation and induced cell apoptosis in dose-dependent manners. Several main apoptotic indicators were found in GS-002-treated cells, such as the cleaved forms of caspase-3, caspase-9, and poly(ADP-ribose) polymerase (PARP). GS-002 also induced endoplasmic reticular (ER) stress as evidenced by increases in ER stress-responsive proteins including glucose-regulated protein 78 (GRP78), growth arrest- and DNA damage-inducible gene 153 (GADD153), phosphorylated eukaryotic initiation factor 2 α (eIF2 α ), phosphorylated protein endoplasmic-reticular-resident kinase (PERK), and ATF-3. The induction of ATF-3 expression was mediated by mitogen-activated protein kinase (MAPK) signaling pathways in GS-002-treated cells. Furthermore, we found that GS-002 induced more cell apoptosis in ATF-3-overexpressing cells. These results suggest that the induction of apoptosis by the propolis derivative, GS-002, is partially mediated through ER stress and ATF-3-dependent pathways, and GS-002 has the potential for development as an antitumor drug.
