    Abstract: There is limited research on the changes of biomechanical characteristics of the lumbar extensor myofascia in elderly patients with chronic low back pain. This study aimed to compare the biomechanical properties of the lumbar extensor myofascia in elderly patients with chronic low back pain and healthy people when resting and to analyze the relationship between the Japanese Orthopedic Association (JOA) score, visual analog scale (VAS) score, Cobb angle, and disease course and the biomechanical characteristics of the lumbar extensor myofascia. This case-control study included 40 elderly patients with chronic low back pain and 40 healthy volunteers. MyotonPRO was used to measure the biomechanical properties of the bilateral lumbar extensor myofascia (at L3/L4 level) in all participants, and the reliability of the MyotonPRO test was measured. Cobb angle was measured from lumbar computed tomography or magnetic resonance imaging data. JOA and VAS scores were used to evaluate lumbar function and pain. We found that muscle tone, stiffness, and elasticity of the left and right lumbar extensor myofascia in patients with chronic low back pain were very reliable among different operators. The average lumbar extensor muscle tone and stiffness were significantly higher in patients with chronic low back pain than those in healthy controls. The average elasticity of the lumbar extensor myofascia of patients with chronic low back pain was significantly lower than that of the healthy controls. The JOA score was negatively correlated, while the VAS score was positively correlated with the mean values of tone, stiffness, and elasticity of the bilateral lumbar extensor myofascia (logarithmic decrement). Disease course had no significant correlation with muscle tone, stiffness, and elasticity of the lumbar extensor myofascia. No significant correlation was found between Cobb angle and muscle tone, stiffness, and elasticity of the lumbar extensor myofascia in either group.

    Abstract: Background. Increasing attention has been paid to electroacupuncture (EA) for promoting postoperative rehabilitation, but the effectiveness of EA for rehabilitation after total knee arthroplasty (TKA) remains obscure. Objective. To examine the effect of EA on rehabilitation after TKA. Methods. Database searches on PubMed, CINAHL, Embase, and China National Knowledge Infrastructure (CNKI) were carried out to obtain articles, from inception to 15 October 2020. All identified articles were screened, and data from each included study were extracted independently by two investigators. Meta-analysis was conducted to assess the effects of acupuncture on pain, range of knee motion, and postoperative vomiting after TKA. Results. In the current study, a total of ten randomized clinical trials were included according to the inclusion and exclusion criteria. Compared to basic treatment, EA combined with basic treatment showed a significantly greater pain reduction on 3, 7, and 14 days postoperatively after TKA. However, we found that EA had no significant improvement in enhancing the range of knee motion and decreasing the percentage of vomiting. Subgroup analysis suggested that a combination of EA and rehabilitation training was superior to rehabilitation training in pain relief, while EA combined with celecoxib capsules showed no significant difference in improving pain compared to celecoxib capsules alone. Conclusions. In the postacute phase after TKA, EA, as a supplementary treatment, could reduce postoperative pain, but no evidence supported the benefits of EA for improving ROM of knee and decreasing the ratio of vomiting. Additional high-quality and large-scale RCTs are warranted.

    Abstract: Background . Interlimb asymmetries are considered to be closely related to knee osteoarthritis (KOA), but foot posture asymmetries in patients with KOA are scarcely reported. Objectives . We aimed to explore the asymmetrical difference in foot posture between the healthy adults and KOA patients and the relationship between foot posture asymmetry and static stability. Methods . 21 subjects suffering from KOA in the patient group (PG) and 21 healthy adults in the control group (CG) were included in this study. Foot postures on both feet were evaluated by using the foot posture index (FPI); subsequently, asymmetrical FPI scores between two feet were calculated for the two groups. Meanwhile, all the participants were tested with a standing on Dynamic and Static Balancing Instrument (Pro-kin 254P, TecnoBody Company, Italy) for bilateral stability assessment, and center of pressure (COP) parameters including sway length (SL, mm) and sway area (SA, mm 2 ) were recorded. Results . Compared to CG, a larger percentage of supinated feet was examined in relatively severe legs (5, 23.81%), relatively moderate legs (4, 19.05%), and merged results (9, 21.43%) of PG ( P 〈 0.05 ), and the percentage of severe asymmetry (38.1%) was higher in the PG ( P 〈 0.05 ). Moreover, these relationships between the absolute value of asymmetry score and SL or SA were significant in CG and PG, the P values below 0.01. Conclusions . Foot posture asymmetry is significantly associated with static stability both in KOA patients and healthy adults, and more severe asymmetry in foot posture was observed in KOA patients, so it is critical to evaluate foot posture asymmetry for treatment and rehabilitation for patients with KOA.

    Abstract: The special engineering characteristics of loess will inevitably bring hidden trouble to the construction of shallow buried subway tunnel. The purpose of this paper is to investigate the variation law of surrounding rock stress, lining stress, and surface settlement in loess tunnel construction and further explore the optimum construction method in the soft loess area. A model test considering the different excavation methods and prereinforcement measures was conducted to measure the surrounding rock pressure, lining stress, and surface settlement under different working conditions. The results show that the sequence of tunnel excavation had a great influence on the stability of the tunnel. As the experimental steps increased, the surrounding rock stress and lining stress at each monitoring section gradually increase. Especially, stress concentration could easily occur at the arch foot of the tunnel. The pre-reinforcement measure, especially the WSS grouting, was more crucial than the excavation method. Considering the number of elements, the combination of DSDM excavation method and WSS grouting reinforcement method was proposed as the final construction method of the tunnel in soft loess area in this paper. The research results can effectively guide the design and construction of a shallow tunnel in the soft loess area.

    Abstract: In order to obtain the accurate contact parameters in the simulation process of powder screw conveying, this paper took wheat flour as an example, based on the discrete element JKR (Johnson-Kendall-Roberts) contact model, and directly calibrated the simulation contact parameters in the process of screw conveying in response to the mass flow rate of wheat flour. Firstly, the simulation density of wheat flour particles was calibrated, and the simulation density of wheat flour particles was 1320 kg/m. Then, Plackett-Burman experiment was used to screen out the parameters that had significant influence on the mass flow rate: surface energy JKR, coefficient of static friction between wheat flour and wheat flour, and the coefficient of static friction between wheat flour and stainless steel. The second-order regression model of mass flow rate and significance parameters was established and optimized based on Box-Behnken experiment, and the optimal combination of significance parameters: JKR was obtained to be 0.364; the static friction coefficient of wheat flour to wheat flour was 0.437; and the static friction coefficient of wheat flour to stainless steel was 0.609. Finally, the calibration parameters were used for simulation. By comparing the mass flow rate of simulation and experiment, the relative error of the two was 1.37%. The simulation and experiment flow rate values at different rotating speeds (60 r/min, 80 r/min, 100 r/min, 120 r/min, and 140 r/min) were further compared, and the errors were all within 3%. The method of directly calibrating the simulation contact parameters through the screw conveying process can improve the accuracy of screw conveying simulation, and providing a method and basis for powder contact parameters calibration and screw conveying simulation of wheat flour.

    Abstract: Background. Knee osteoarthritis (KOA) is a high incidence chronic joint disease that seriously affects patients’ quality of life, and current treatment methods have limited efficacy. Self-management may be an effective strategy for KOA, and clinicians have been showing increased interest recently. However, the effectiveness of self-management for KOA remains controversial. Purpose. This study aims to systematically evaluate the effectiveness of self-management for KOA. Methods. We screened articles published in MEDLINE, Cochrane Library, EMBASE, and Web of Science until September 17, 2021. The main outcomes included pain, knee function, stiffness, WOMAC (total), physical function, arthritis self-efficacy (ASE-pain), arthritis self-efficacy (ASE-other symptoms), mental health, and quality of life. Results. Thirteen randomized controlled trials (RCTs) were finally included (n = 1610). Meta-analysis showed differences in pain, knee function, stiffness, ASE-pain, ASE-other symptoms, mental health, and quality of life between the self-management and control groups. Of the nine outcomes evaluated, four were highly heterogeneous, and the quality of evidence ranged from very low to moderate. Conclusion. The meta-analysis results showed that self-management might help improve the pain, knee function, stiffness, ASE, mental health, and quality of life in patients with KOA. However, it has no significant effect on WOMAC (total) and physical function. Considering that this study has some limitations, we cannot draw clear conclusions based on the results of this study. Nevertheless, we offer much needed insight and encourage more rigorously designed and implemented RCTs in the future to substantiate our conclusions.

    Abstract: Objective. Impaired static stability and proprioception have been observed in individuals with knee osteoarthritis (KOA), which serves as a major factor increasing risk of fall. This study aimed to investigate the effects of backward walking (BW) on static stability, proprioception, pain, and physical function in KOA patients. Methods. Thirty-two subjects with knee osteoarthritis were randomly assigned to either an BW group (BG, n = 16) or a control group (CG, n = 16). The participants in the BG received combination treatment of a 4-week BW training and conventional treatments, while those in the CG was treated with conventional treatments alone. All the participants were tested for the assessment of static stability [center of pressure (COP) sway, including sway length (SL, mm) and sway area (SA, mm2)] and proprioception [average trajectory error (ATE, %) and completion time (CT, second)] . Additionally, pain and knee function scores were measured by the numerical rating scale (NRS) and the Western Ontario and McMaster Universities Osteoarthritis (WOMAC) Index, respectively. The assessments were conducted before and after intervention. Results. The COP sway (SA and SL), ATE, NRS, and WOMAC showed a significant decline at week 4 in the two groups in contrast to their baseline ( P 〈 0.05 ). Moreover, after 4-week intervention, the SA [(610.50 ± 464.26) mm2 vs. (538.69 ± 420.52) mm2], NRS [(1.56 ± 0.63) vs. (2.25 ± 0.86)] , and WOMAC [(11.69 ± 2.50) vs. (16.19 ± 3.94)] showed a significantly greater decrease in the BG compared to the CG ( P 〈 0.05 , respectively). However, the proprioception (ATE and CT) was closely similar between both groups at week 4 ( P 〉 0.05 ). Conclusion. BW is an effective adjunct to conventional treatment in reducing pain, improving physical function and static stability for KOA patients. It should be taken into consideration when developing rehabilitation programs for people with KOA.

    Abstract: The calculation of the core source term is affected by various factors, such as fuel consumption, enrichment, specific power, and operation mode. The activity of lanthanides, fission products, and the photon source strength were calculated using the ORIGEN program. The weights of each factor were calculated by multivariate analysis of variance. The results show that the radioactivity of actinides and fission products increased with the increase in fuel consumption. As enrichment increased, the radioactivity of fission products and actinides decreased. The radioactivity of fission products and actinides increased linearly with the change in specific power, with a correlation coefficient of 1. The changes in fuel consumption and enrichment have little effect on low-energy photons, but significantly affected high-energy photons. The change in specific power has little effect on the photon generation of different energy groups. The operation mode has little effect on the radioactivity of the nucleus and fission products. Multivariate analysis of variance shows that specific power is the most influential factor, followed by enrichment; the least influential factor is fuel consumption.
