    Abstract: Objective. (1) To propose a novel technique named thoracic extensive laminoplasty (TELP) in curing severe thoracic ligamentum flavum ossification (STOLF) and (2) to compare outcomes between TELP and laminectomy in curing STOLF. Methods. Cases with fused or tuberous STOLF (Sato classification) treated from Jan 2015 to Jan 2017 were reviewed and divided into the TELP group (G1) and laminectomy group (G2) according to their surgical management. Data on demographics, complications, pre- and postoperative symptoms, residual spinal canal area (RSCA-1), residual spinal cord area (RSCA-2), modified Japanese Orthopedic Association score (mJOA), and health-related quality of life (HRQOL, based on the SF-36) were collected. Results. Fifty-nine G1 and sixty-two G2 patients were enrolled. No significant differences in demographic data or preoperative data of RSCA-1, RSCA-2, mJOA, or HRQOL were observed between the two groups ( p 〉 0.05 ). Patients in G1 and G2 showed similar postoperative improvements in RSCA-1 and RSCA-2 at the final follow-up ( p 〉 0.05 ). However, patients in G1 showed higher postoperative improvements in mJOA ( OR = 2.706 , 95% CI: 1.279~5.727, p = 0.008 ) at the final follow-up. Patients in G1 also showed higher postoperative improvements in HRQOL than patients in G2 ( p 〈 0.05 ) at the final follow-up, and patients with more severe STOLF presented with better improvements in HRQOL in G1 ( p 〈 0.05 ). Dural laceration and cerebrospinal fluid leakage were observed in seven G2 patients, and no complications were found in G1 patients after surgery. Conclusion. TELP is a novel, effective, and safer surgical technique in treating STOLF and could be a substitute for traditional laminectomy.

    Abstract: Sand production is a problem that is often encountered in unconventional oil and gas exploitation and that is difficult to effectively solve. Accurate online monitoring of sand production is one of the keys to ensuring the safety and long-term production of oil wells as well as efficient production throughout the life cycle of production wells. This paper proposes a method for monitoring sand production in offshore oil wells that is based on the vibration response characteristics of sand-carrying fluid flow impinging on the pipe wall. This method uses acceleration sensors to obtain the weak vibration response characteristics of sand particles impinging on the pipe wall on a two-dimensional time-frequency plane. The time-frequency parameters are further optimized, and the ability to identify weakly excited vibration signals of sand particles in the fluid stream is enhanced. The difference between the impact response of the sand particles and the impact response of the fluid flow to the pipe wall is identified, and corresponding indoor verification experiments are carried out. Under different sand contents, particle sizes, and flow rates (sand content 0-2‰, sand particle size 96-212 μm, and flow velocity 1-3 m/s), the impact response frequency of sand particles to the pipe wall exhibits good consistency. The characteristic frequency band of sand impacting the pipe wall is 30-50 kHz. A statistical method is used to establish the response law of the noise signal of the fluid. Based on this knowledge, a real-time calculation model of sand production in offshore oil wells is constructed, and the effectiveness of this model is verified. Finally, a field test is carried out with a self-developed sand production signal dynamic time-frequency response software system on 4 wells of an oil production platform in the Bohai Sea. This system can effectively distinguish sand-producing wells from non-sand-producing wells. The dynamic time-frequency response, field test results, and actual laboratory results are consistent, verifying the effectiveness of the method proposed in this paper and further providing a theory for improving the effectiveness of the sand production monitoring method under complex multiphase flow conditions. This study also provides technical guidance for the industrial application of sand production monitoring devices in offshore oil wells.

    Abstract: Purpose . To observe ocular surface changes in Type II diabetic patients with different disease durations and to understand the correlations between clinical parameters and diabetic durations. Methods . In this cross-sectional, prospective study, 51 healthy controls and 91 patients with Type II diabetes were enrolled. The diabetics were divided into 3 subgroups according to the disease duration, including duration 〈 10 y group, 10 to 20 y group, and ≥21 y group. All subjects underwent clinical ocular examinations, including lipid layer thickness (LLT), blinking rate, tear meniscus height (TMH), noninvasive tear film break-up time (NI-BUT), meibography, superficial punctate keratopathy (SPK) scoring, corneal sensitivity, and Schirmer I test. They were also evaluated using the standard patient evaluation of eye dryness (SPEED) questionnaire. Results . SPEED score, meiboscore, SPK score, LLT, Schirmer I test, and corneal sensitivity differed significantly between the diabetic and healthy control groups. Further, SPEED score, Schirmer I test, corneal sensitivity, meiboscore, and blink rate significantly differed among the 3 diabetic subgroups and the control group. In diabetics, the SPEED score correlated with the SPK score, blink rate, TMH, and LLT; NI-BUT with TMH, LLT, and blink rate; TMH with the SPK score; Schirmer I test with the SPK score; and corneal sensitivity with the meiboscore. More importantly, the Schirmer I test, corneal sensitivity, and SPEED score negatively correlated with diabetic duration. Conclusion . Diabetic duration is an important factor that affects functions of the lacrimal functional unit in patients with Type II diabetes. The trends of changes in the ocular parameters vary along the course of diabetes.

    Abstract: The eukaryotic initiation factor 3 (eIF3) is the largest and most complex translation initiation factor in mammalian cells. It consists of 13 subunits and among which several were implicated to have significant prognostic effects on multiple human cancer entities. To examine the expression profiles of eIF3 subunits and determine their prognostic value in patients with lung adenocarcinoma (LUAD), the genomic data, survival data, and related clinical information were obtained from The Cancer Genome Atlas (TCGA) project for a secondary analysis. The results showed that among ten aberrantly expressed eIF3 subunits in tumours compared with adjacent normal counterparts ( p 〈 0.05 ), only upregulated eIF3D could predict poor overall survival (OS) outcome independent of multiple clinicopathological parameters ( HR = 2.043 , 95% CI: 1.132-3.689, p = 0.018 ). Chi-square analysis revealed that the highly expressed eIF3D group had larger ratios of patients with advanced pathological stage (68/40 vs. 184/206, p = 0.0046 ), residual tumour (13/4 vs. 163/176, p = 0.0257 ), and targeted molecular therapy (85/65 vs. 138/164, p = 0.0357 ). In silico analysis demonstrated that the altered expression of eIF3D was at least regulated by both copy number alterations (CNAs) and the hypomethylation of cg14297023 site. In conclusion, high eIF3D expression might serve as a valuable independent prognostic indicator of shorter OS in patients with LUAD.
