    Abstract: Objective. To better understand the status of medical treatment for human epidermal growth factor receptor 2 (HER2)-positive breast cancer and the differences between the Chinese and the international clinical practice. Methods. This was a retrospective, nationwide, multicenter, epidemiological study of advanced breast cancer patients from China. Between January 01, 2012, and December 31, 2014, a total of 3649 patients, covering 7 geographic regions and 21 institutions, participated in this series of studies. HER2-positive breast cancer was selected among the group and adopted into this study. In comparison, we summarized the demographics and clinical characteristics of HER2-positive breast cancer from the Surveillance, Epidemiology, and End Results (SEER) database. Results. A total of 918 patients diagnosed as HER2-positive breast cancer patients were included. The median age at diagnosis was 46 years (ranging, 23 to 78) with a single-peak incidence. The proportions of stages II–IV at diagnosis and distance metastasis in viscera were more than half of the participants. In comparison, the prevalence of estrogen or progesterone receptor-positive expression and luminalB subtype was relatively lower than that of the United States. The receipt of chemotherapy was fairly higher, while the usage of targeted therapy was seriously insufficient. Tumor size was in significantly positive associations with the duration of targeted therapy (Kendall’s correlation coefficient = 0.3, P 〈 0.0001 ), while no prohibitive variables among clinical characteristics were detected. Conclusion. Our study suggested that HER2-positive breast cancer patients were characterized as a younger trend, a lower prevalence of hormonal receptor (HR)-positive expression, and less accessible to anti-HER2 targeted therapy with insufficient duration over the past few years in China. Concerted efforts should be exerted for promising survival benefits in the future. The trial registration number is https://clinicaltrials.gov/ct2/show/NCT03047889.

    Abstract: Visual or haptic feedback based on error modulation has been used to improve the effect of robot-assisted rehabilitation training. However, there are several investigations on the effects of error modulation-based visual and haptic feedback fusion strategies on motor learning and motivation. To observe the influence of different feedback fusion strategies on motor learning and motivation, a parallel controlled study was conducted, dividing 30 healthy subjects into three groups with similar skill levels. The no error modulation group received visual and haptic feedback without error modulation; the visual amplification haptic reduction group received visual error amplification combined with haptic error reduction, and the visual reduction haptic amplification (VRHA) group received visual error reduction combined with haptic error amplification. Each subject implemented a trajectory-tracking task with an upper limb rehabilitation robot. They went through baseline, training, assessment, and generalization tests and completed 340 consecutive tracking movements. To evaluate motor learning and motivation, the average tracking error, the root mean square (RMS) of surface electromyography (sEMG) signals, and the intrinsic motivation inventory scale were all examined. In the assessment tests, the average tracking error was significantly decreased in all three groups. In particular, the VRHA group had a larger reduction in average tracking error in the generalization test, lower RMS of sEMG signals both in the assessment and generalization tests and higher perceived competence in the assessment tests. The VRHA fusion strategy significantly improved the subjects’ motor learning and transfer ability, decreased muscle activation, and increased motor learning motivation. These findings may provide some new insights for multisensory feedback fusion technology in the application of rehabilitation robots.

    Abstract: The ablation and acceleration of diamond-like high-density carbon foils irradiated by thermal X-ray radiations are investigated with radiation hydrodynamics simulations. The time-dependent front of the ablation wave is given numerically for radiation temperatures in the range of 100–300 eV. The mass ablation rates and ablation pressures can be derived or implied from the coordinates of ablation fronts, which agree well with reported experiment results of high-density carbon with radiation temperatures T rad in the range of 160–260 eV. It is also found that the $T_{{\rm rad}}^3$ scaling law for ablation rates does not apply to T rad above 260 eV. The trajectories of targets and hydrodynamic efficiencies for different target thicknesses can be derived from the coordinates of ablation fronts using a rocket model and the results agree well with simulations. The peak hydrodynamic efficiencies of the acceleration process are investigated for different foil thicknesses and radiation temperatures. Higher radiation temperatures and target thicknesses result in higher hydrodynamic efficiencies. The simulation results are useful for the design of fusion capsules.

    Abstract: Vascular stent interventional therapy is the main method for clinical treatment of coronary artery diseases. However, due to the insufficient biocompatibility of cardiovascular materials, the implantation of stents often leads to serious adverse cardiac events. Surface biofunctional modification to improve the biocompatibility of vascular stents has been the focus of current research. In this study, based on the structure and function of extracellular matrix on vascular injury healing, a novel fibronectin-loaded poly-l-lysine/heparin nanoparticles was constructed for stent surface modification. In vitro blood compatibility evaluation results showed that the nanoparticles-modified surface could effectively reduce platelet adhesion and activation. In vitro cellular compatibility evaluation results indicated that the nanocoating may provide adequate efficacy in promoting the adhesion and proliferation of endothelial cells and thereby accelerate endothelialization. This study provides a new approach for the surface biological function modification of vascular stents.

    Abstract: The vertical additional force induced by the drainage consolidation settlement of the topsoil is one of the main factors for shaft wall fracture. To date, the number of aquifers of topsoil rises with the depth increasing of shaft, which would lead to a more serious additional force effect. Thus, studying the vertical additional force law of the shaft when drainage settlement occurs in its surrounding topsoil containing multiaquifers is of great significance to predicting the shaft stress and guarantying shaft safety. In this study, mechanical analysis of the topsoil with the shaft crossing multiaquifer was carried out, and the settlement of each aquifer and aquiclude was calculated by separating the single-slope drainage consolidation and the double-slope drainage consolidation. Then, the calculation model of vertical additional force was established due to the settlement caused by the reaction of the additional force on the topsoil containing multiaquifers, and the calculation model of shaft wall stress was also developed. Verification of this model was conducted by comparing the filed measurement data of the shaft wall strain and the theoretical data calculated by the stress model. Finally, the effect laws of drainage velocity, central aquifer thickness, location, and number on the additional force were obtained and analysed. This paper is expected to provide theoretical support for predicting the additional force and shaft wall stress during its service time.

    Abstract: Traditional Chinese medicine suggests that renal deficiency is a causative factor of asthma, and tonifying kidney drugs are believed to be an appropriate and beneficial treatment. The adrenal medullary chromaffin cells (AMCC) transition to the neuronal phenotype is known to occur in asthma, as evidenced by degranulation of chromaffin granules, decline of epinephrine (EPI) and phenylethanolamine-n-methyl transferase (PNMT), and obvious alterations in cellular architecture. In this study, rats were sensitized and challenged with ovalbumin, then treated with Kidney-Tonifying Recipe (KTR) to evaluate the therapeutic effect. Tissues were evaluated for changes in pathology and EPI, PNMT, and peripherin expression. Degranulation of chromaffin granules and appearance of neurite-like process were found in AMCC from asthmatic rats, and these changes were corrected by KTR treatment. EPI and PNMT expressions were decreased in asthmatic rats and increased by KTR treatment. Peripherin expression was increased in asthmatic rats and decreased in the KTR-treated group. Morphological changes and decreases in EPI were observed when cultured AMCC were exposed to sera from asthmatic rats in vitro , and these changes were attenuated with the addition of sera from KRT-treated rats. These results suggest that the Kidney-Tonifying Recipe is capable of repairing asthma-associated alterations in endocrine function and the ultrastructure of AMCC.

    Abstract: A high Mandard score may indicate the tumor is insensitive to chemotherapy. We analyzed tumor regression and lymph node response under different Mandard scores to assess the impact of Mandard score on prognosis. Methods. Mandard scores and ypN stage of postoperative pathological reports were recorded. The results were reviewed by a professional pathologist. The radiologist compared the tumor regression before and after chemotherapy by computed tomography (CT). The survival of all patients was obtained by telephone follow-up. Multivariate Cox regression was used to assess the relationship between overall risk of death and Mandard score, imaging evaluation, and ypN stage. Results. In the Mandard score (4-5) group, the median survival time for PR and ypN0 patients was 68.5 and 76.7 months. While in the Mandard score (1-2) group, the median survival time for PD and ypN3a patients was 15.6 and 14.5 months. Imaging evaluation of tumor regression (PR 68.5 months, SD 27.8 months, and PD 10.2 months) and lymph node remission (ypN0 76.7 months, ypN1 61.6 months, ypN2 18.0 months, ypN3a 18.7 months, and ypN3b 18.3 months) showed improved survival. Mandard score, imaging evaluation, and ypN stage are important prognostic factors affecting prognosis. Conclusion. A high Mandard score does not mean neoadjuvant chemotherapy is ineffective in gastric cancer. Patients with imaging evaluation of tumor regression and ypN stage reduction may benefit from neoadjuvant chemotherapy.

    Abstract: Objectives. To compare the clinical outcomes of using different hemostatic agents after transurethral plasmakinetic resection of the prostate (TUPKP) in benign prostatic hyperplasia (BPH) patients. Methods. The patients were divided into 5 groups according to the hemostatic agents used after TUPKP, including the haemocoagulase agkistrodon for injection (HCA), hemocoagulase for injection (HC), hemocoagulase bothrops atrox for injection (HCB), ethylenediamine diaceturate injection (EDD), and tranexamic acid (TXA). Propensity score matching was performed based on age, body mass index, prostate volume, hypertension status, fasting blood glucose, smoking, and drinking history. The hospitalization time, bladder irrigation time, indwelling catheterization time, the patency of urine flow, and blood transfusion records were used as outcome indicators to compare the clinical effects of these five agents. Results. We finally matched 65 pairs receiving HCA or HC, 71 pairs receiving HCA or HCB, 38 pairs receiving HCA or TXA, and 29 pairs receiving HCA or EDD. Compared with HC, HCA given during the perioperative period significantly reduced the median hospitalization time [7.00 days (5.00, 8.00) vs. 9.00 days (8.00, 10.00); p 〈 0.001 ] and median catheterization time (109.00 hours [88.00, 129.00] vs. 164.00 hours [114.00, 189.00], p 〈 0.001 ). Compared with EDD, the median hospitalization time (7.00 days [6.00, 8.00] vs. 10.00 days [8.00, 11.00] ; p 〈 0.001 ) and median catheterization time (113.00 hours [95.00, 143.00] vs. 160.00 hours [139.00, 168.00] ; p 〈 0.001 ) were also significant shorter in HCA group. Compared with HCB, median bladder irrigation time (45.00 hours [27.00, 71.00] vs. 49.00 hours [45.00, 72.00] ; p = 0.04 ) was shorter in the HCA group. However, there were no statistical differences in outcomes between HCA and TXA. Conclusions. HCA probably has an advantage over HC, HCB, and EDD in reducing the hospitalization time, catheterization time, and bladder irrigation time among BPH patients undergoing TUPKP.
