    Abstract: Purpose. Triple-negative breast cancer (TNBC) is a heterogeneous and aggressive disease with poorer prognosis than other subtypes. We aimed to investigate the prognostic efficacy of multiple tumor markers and constructed a prognostic model for stage I-III TNBC patients. Patients and Methods. We included stage I-III TNBC patients whose serum tumor markers levels were measured prior to the treatment. The optimal cut-off value of each tumor marker was determined by X-tile. Then, we adopted two survival models (lasso Cox model and random survival forest model) to build the prognostic model and AUC values of the time-dependent receiver operating characteristic (ROC) were calculated. The Kaplan-Meier method was used to plot the survival curves and the log-rank test was used to test whether there was a significant difference between the predicted high-risk and low-risk groups. We used univariable and multivariable Cox analysis to identify independent prognostic factors and did subgroup analysis further for the lasso Cox model. Results. We included 258 stage I-III TNBC patients. CEA, CA125, and CA211 showed independent prognostic value for DFS when using the optimal cut-off values; their HRs and 95% CI were as follows: 1.787 (1.056–3.226), 2.684 (1.200–3.931), and 2.513 (1.567–4.877). AUC values of lasso Cox model and random survival forest model were 0.740 and 0.663 for DFS at 60 months, respectively. Both the lasso Cox model and random survival forest model demonstrated excellent prognostic value. According to tumor marker risk scores (TMRS) computed by the lasso Cox model, the high TMRS group had worse DFS (HR = 3.138, 95% CI: 1.711–5.033, p 〈 0.0001 ) and OS (3.983, 1.637–7.214, p = 0.0011 ) than low TMRS group. Furthermore, subgroup analysis of N0-N1 patients in the lasso Cox model indicated that TMRS still had a significant prognostic effect on DFS (2.278, 1.189–4.346) and OS (2.982, 1.110–7.519). Conclusions. Our study indicated that pretreatment levels of serum CEA, CA125, and CA211 had independent prognostic significance for TNBC patients. Both lasso Cox model and random survival forest model that we constructed based on tumor markers could strongly predict the survival risk. Higher TMRS was associated with worse DFS and OS both in stage I-III and N0-N1 TNBC patients.

    Abstract: The objective is to study the application of spiral CT in the diagnosis of the trachea in children. In this study, the effect of 64-slice multislice spiral CT in diagnosing infant bronchial bridge was studied based on an artificial neural network. From June 2020 to December 2020, 100 cases of children with the trachea in the outpatient department of our hospital were selected as the research object. They were divided into the study group and the control group, with 50 cases in each group. The results showed that among the 50 cases in the control group, 42 cases were found to have a bronchial foreign body and 8 cases were missed; the detection rate was 84%. There were 7 cases of trachea foreign body, 15 cases of left bronchial foreign body, 14 cases of right bronchial foreign body, and 6 cases of medium bronchial foreign body. The detection rate of the study group was significantly higher than that of the control group, with a statistical significance ( P 〈 0.05 ). Conclusion. The detection rate of neural networks in MSCT is higher than that of X-ray, and the MSCT based on the artificial neural network can clearly show the morphology, position, and the relationship between the foreign body and trachea, which is worthy of clinical promotion and application.

    Abstract: Methyl 7,7′-dimethoxy-5′-(morpholinomethyl)-[4,4′-bibenzo[d][1,3] dioxole]-5-carboxylate methanesulfonate (IMM) is an innovative drug for the treatment of nonalcoholic fatty liver disease (NAFLD) owing to its high efficacy and low toxicity. In this study, five minor impurities (I, II, III, IV, and V) were identified and analyzed using spectroscopic evidence, chemical synthetic methods, and liquid chromatography-tandem mass spectrometry (LC-MS/MS). The impurities included hydrolysates and oxidation by-products extracted from both the drug in its final formulation and during synthesis. Toxicity prediction revealed potential carcinogenicity of impurity V containing an N-oxygen fragment. A reliable and selective HPLC method for the quantitative analysis of impurities I–IV and a sensitive HPLC-MS/MS method for potential genotoxic impurity V were developed and optimized. The methods were validated based on the International Council for Harmonization guidelines. Satisfactory linearity was obtained for the analytes over the range of 0.1–2.0 μg/mL for impurities I–IV and 0.3–30.0 ng/mL for impurity V, and in all cases, the fitting correlation coefficients exceeded 0.999. The obtained limits of detection values were 0.05 ng/mL and 0.005 μg/mL for impurity V and impurities I–IV, respectively. The precision and repeatability of the methods were less than 1.08% and 8.72% for each impurity. The recovery percentages of all impurities were in the range of 91.18%–111.27%, with the relative standard deviation of less than 3.69%. The greenness assessment of the HPLC method and the HPLC-MS/MS method were evaluated by using AGREE software with a score value of 0.72 and 0.68, respectively. The recommended procedures that were accurate, specific, and ecofriendly were applied to the existing active pharmaceutical ingredients of IMM, and they generated satisfactory results.

    Abstract: Objective . To date, a series of studies were conducted to investigate the association between TLR2 (Toll-like receptor 2) Arg753Gln gene polymorphism and tuberculosis (TB). However, the results were inconsistent. This meta-analysis was performed to elucidate the roles of TLR2 Arg753Gln gene polymorphism in TB. Methods . All available articles were searched from online databases such as PubMed, Medline, CNKI, and Wanfang. Statistical analyses were performed using the STATA12.0 (Stata Corp LP, College Station, TX, United States) software. Results . 32 case-control studies comprising 5943 cases and 5991 controls were identified in this meta-analysis. Overall, the TLR2 Arg753Gln gene polymorphism was associated with high TB risk in allele model (A vs. G: OR=2.20, 95%CI=1.60-3.04, P≤0.01), dominant model (AA+AG vs. GG: OR=2.70, 95%CI=2.00-3.65, P≤0.01), and heterozygote model (AG vs. GG contrast: OR=2.97, 95%CI=2.39-3.69, P≤0.01). Subgroup analysis by ethnicity indicated that the A allele increased susceptibility to TB in Asian (OR=3.35, 95%CI=2.36-4.74) and Caucasian populations (OR=2.62, 95%CI=1.77-3.87), but not in African (2.08, 95%CI=0.62-2.72) or mixed populations (OR=0.76, 95%CI=0.36-1.14). Stratified analysis by sample type suggested that the A allele associated with high pulmonary tuberculosis (PTB) risks (OR=2.43, 95%CI=1.66-3.54), but not with extra pulmonary tuberculosis (EPTB) (OR=1.84, 95%CI=0.83-4.06). Conclusion . this meta-analysis suggested the following: (1) TLR2 Arg753Gln polymorphism is significantly associated with high TB risk. (2) In subgroup analysis based on ethnicity, TLR2 Arg753Gln polymorphism elevates the risk of TB in Asian and Caucasian populations, but not in African or mixed populations. (3) Stratified by sample type, TLR2 Arg753Gln polymorphism is associated with increased PTB risk, but not with EPTB.

    Abstract: In vitro culture of mesenchymal stem cells (MSCs) from mouse bone marrow (BM) has been hampered because of the low yield of MSCs during isolation and the contamination of hematopoietic cells during expansion. The lack of specific mouse BM-MSC markers increases the difficulty. Several techniques have been reported to improve the purity and in vitro growth of mouse BM-MSCs. However, systematic report on comparison of characteristics in primary BM-MSCs between different culture conditions is rare. Here, we studied the effects of oxygen concentrations and initial medium replacement intervals, along with cell passages, on mouse BM-MSCs isolated with differential adhesion method. BM-MSCs exhibited elevated proliferative and clonogenic abilities in 5% oxygen compared with 10% and 21% oxygen, as well as a better expression of the MSC marker Sca-1. Adipogenic and osteogenetic differentiation of BM-MSCs can be observed in both 21% and 5% oxygen. Adipogenic differentiation appeared stronger under normoxia conditions. BM-MSCs showed increased proliferative capacity and adipogenic/osteogenetic differentiation potential when initial medium replacement interval was 4 days compared with 1 day. As passage number increased, cells were more MSC-like in morphology and in expression of surface markers (positive for CD29, CD44, and Sca-1 and negative for CD11b, CD19, and CD45). These data provide new insight into optimizing the culture method and understanding the biological characteristics of mouse BM-MSCs during in vitro expansion.

    Abstract: Objective . To investigate the antibacterial effect of a novel antimicrobial peptide containing oral spray GERM CLEAN on Streptococcus mutans (S. mutans) in vitro and further explore the related mechanisms at phenotypic and transcriptional levels. Methods . The disk diffusion method was used to preliminarily appraise the antimicrobial effect of GERM CLEAN. The minimal inhibitory concentration (MIC) of GREM CLEAN towards S. mutans was determined by the broth dilution method. S. mutans virulence-related phenotypic assays including initial adhesive assay, pH drop, exopolysaccharides (EPS), and biofilm formation measurements and quantitative real-time PCR (qRT-PCR) were further applied to detect the inhibitory mechanisms of GREM CLEAN at 1/2MIC. Results . The diameter (10.18 ± 1.744 mm) of inhibition zones formed by GERM CLEAN preliminarily indicated its inhibitory effect on the major cariogenic bacteria S. mutans . The minimal inhibitory concentration of GERM CLEAN on S. mutans was 100% mass fraction (the stock solution). The study of the antibacterial mechanism showed that GERM CLEAN had a certain inhibitory effect on the initial adhesion, acid production, extracellular polysaccharides (EPS) production, and biofilm formation of S. mutans . GERM CLEAN disturbed S. mutans biofilm physiology mainly through destruction of biofilm architecture and suppression of bacterial growth. The results of qRT-PCR further confirmed that the expression levels of EPS and lactic acid generation genes including gtfB , gtfC , gtfD , and ldh were significantly repressed by treating with GERM CLEAN, and this was consistent with our phenotypic results. Conclusion . The novel antimicrobial peptide containing oral spray GERM CLEAN has an anti- Streptococcus mutans effect and the inhibitory property may be due to suppression of the virulence factors of S. mutans including adhesive, acidogenicity, EPS, and biofilm formation.

    Abstract: Objectives. Conventional approaches for patients with nonerosive gastroesophageal reflux disease (NERD) were not satisfactory. This study aimed to evaluate the effectiveness and mechanisms of Chinese herbal medicine Hewei Jiangni Decoction (HWJND) as a novel and promising regimen for NERD. Methods. A total of 128 patients with NERD were randomly assigned to the Treatment group and Control group. The patients from the Treatment group were administered HWJND (81 g) plus dummy omeprazole (20 mg) daily for 8 weeks, and the others were given dummy HWJND granules (81 g) plus omeprazole (20 mg). The clinical efficacy was assessed using the gastroesophageal reflux disease questionnaire (GERD-Q) scale, patient reported outcomes (PRO) scale, and short form health survey 36 (SF-36) scale at week 4. Moreover, its pharmacological and molecular mechanisms were elucidated based on network pharmacology and molecular docking. Results. Due to case shedding and other reasons, 109 patients, including 56 in the Treatment group and 53 in the Control group completed this study. Our results showed that HWJND significantly improved heartburn, regurgitation, epigastric pain, nausea, and sleep disturbance, which led to a significant reduction of GERD-Q scores in NERD patients. In addition, PRO scores of NERD patients with HWJND administration were improved, and sufficient relief of physical role, body pain, general health, social function, and mental health on the SF-36 scale was also observed in patients after HWJND treatment. We further showed that the curative effect of HWJND was close to that of omeprazole, except for the better improvement of general health and social function. What’s more, the main active ingredients of HWJND included quercetin, beta-sitosterol, naringenin, baicalein, and kaempferol were retrieved, and the protective effects of HWJND against NERD may be closely related to targets such as TNF, IL6, IL1B, MMP9, CXCL8, and EGFR, which were mainly enriched in IL-17 signaling pathway and TNF signaling pathway. Conclusion. Our findings demonstrate that HWJND is noninferior to oral omeprazole for the treatment of patients with NERD, plays a therapeutic role through multiple targets and diverse pathways, and holds promise for complementary and alternative therapy for the treatment of NERD.     Abstract: The AMP-activated protein kinase (AMPK)/peroxisome proliferator-activated receptor γ coactivator 1α (PGC-1α) pathway performs a crucial role in energy metabolism and mitochondrial network. Our previous study found that DT-010, a novel danshensu (DSS) and tetramethylpyrazine (TMP) conjugate, had significant cardioprotective properties in vitro and in vivo. We also reported that ERp57 served as a major target of DSS using the chemical proteomics approach. In this article, we focus on exploring the interrelationship between the regulation of the AMPK/PGC-1α pathway and promoting ERp57 expression induced by DT-010 in tert-butylhydroperoxide- (t-BHP-) induced H9c2 cell injury. The results showed that DT-010 activated the AMPK/PGC-1α pathway and increased ERp57 protein expression. Importantly, the above phenomenon as well as the mitochondrial function can be partially reversed by siRNA-mediated ERp57 suppression. Meanwhile, silencing AMPK significantly inhibited the ERp57 expression induced by DT-010. In addition, molecular docking and kinase assay in vitro revealed that DT-010 had no direct regulation effects on AMPK activity. Taken together, DT-010 exerted cardioprotective effects by regulating the crosstalk of AMPK/PGC-1α pathway and ERp57, representing a potential therapeutic agent for ischemic heart disease.

    Abstract: In order to explore the suitability evaluation of regional construction land in mountainous villages and towns, a method based on GIS technology is proposed. Supported by GIS and RS technology, the Delphi method is used to determine the natural, socioeconomic, and ecological security factors affecting the ecological suitability of regional construction land in mountainous villages and towns, and analytic hierarchy process is used to calculate the weight of relevant influencing factors. Following the principle of giving priority to ecological protection, a set of ecological suitability evaluation model system and method of regional construction land in mountainous villages and towns are established, based on which the basic ecological control areas of mountainous villages and towns in the study area are divided, so as to provide suggestions for rational and effective planning of land resources in mountainous villages and towns. The experimental results show that the K-means clustering method is divided into five categories: the most suitable land, the more suitable land, the basically suitable land, the unsuitable land, and the unavailable land. Finally, according to the suitability grade system of construction land, it is reclassified, the unsuitable land and unavailable land are divided into basic ecological control areas, the most suitable land and more suitable land are divided into construction control areas, and the basically suitable land is listed as ecological buffer areas. It is proved that the basic ecological control area determined by the model method is basically consistent with the current basic ecological control line of a city, which shows that the model is practical and scientific.
