    Abstract: Background . Osteoporosis and fragility fracture are the major complications after liver transplantation (LT). The aim of the study was to determine the incidence and risk factors of hip fracture after LT. Methods . We conducted a retrospective population-based cohort study, enrolling the patients receiving LT between January 1999 and December 2010. Control subjects were randomly matched to every recipient by age and sex by 1 : 10 ratios. Results . During the follow-up period, 17 recipients (0.77%) and 70 (0.32%) control subjects suffered from hip fractures. The incident rates (per 10000 person-years) were 21.49 for recipients and 7.52 for controls (adjusted hazard ratio = 2.71; 95% confidence interval = 1.21–6.05). The cumulative incidence of hip fracture was significantly higher among the recipients ( p 〈 0.0001 ). Among the recipients, the subjects aged 〉 65 years at transplantation and with pretransplant steroid use are more susceptible to posttransplant hip fracture. Immunosuppressive agents did not significantly affect the risk of hip fracture among recipients. Conclusions . Liver transplantation is a risk factor for hip fractures. Aged 〉 65 years at transplantation and pretransplant steroid use are risk factors for posttransplant hip fractures among the recipients.

    Abstract: Background . Cholangitis is the infectious disease involving the biliary tract, which may induce systemic inflammation. Bone loss is a well-known sequelae after systemic inflammatory disease, and one grave complication after osteoporosis is hip fracture. We want to know whether cholangitis can contribute to increased risk of hip fracture. Methods . All the patients diagnosed with cholangitis since January 1, 2001, to December 31, 2009, were assessed. All the subjects with cancer history, traumatic accident, and previous fracture were excluded. We selected the controls without cholangitis and matched the controls to cholangitis patients by age, sex, osteoporosis, and the use of steroid for more than 30 days by approximately 1:4 ratio. Results . There were 2735 subjects in the cholangitis cohort and 10915 in the noncholangitis cohort. There were 101 hip fractures in the cholangitis cohort with the incidence density of 7.58 per 1000 person-years. As for the noncholangitis cohort, 366 individuals suffered from hip fracture with the incidence density of 5.86 per 1000 person-years. The risk of hip fracture was higher in the cholangitis cohort with a 1.29-fold increased risk than the noncholangitis cohort (hazard ratio = 1.29, 95% confidence interval = 1.03-1.61). The association between cholangitis and the hip fracture was more prominent among subjects less than 65 years (hazard ratio = 2.65, 95% confidence interval =1.30-5.39) and the subjects without comorbidities (hazard ratio = 3.01, 95% confidence interval = 1.42-6.41). Conclusions . Cholangitis is associated with higher risk for hip fracture, especially among young subjects free from medical comorbidities.

    Abstract: Hypoxia-inducible factor (HIF)-1 α and vascular endothelial growth factor (VEGF) are important angiogenic factors in human cancers. Relative to VEGF-C, prognostic significance of VEGF-D expression and its association with HIF-1 α expression remain elusive in esophageal squamous cell cancer (ESCC). We studied expression of HIF-1 α and VEGF-D using immunohistochemistry in 85 resected ESCC specimens and correlated results with patients' clinicopathologic parameters and survival. Association between expression of HIF-1 α and VEGF-D was investigated using a concordance analysis. High expression of HIF-1 α and VEGF-D was observed in 52 (61.2%) and 56 (65.9%) patients, respectively. HIF-1 α expression correlated well with tumor stage ( P = 0.041), whereas VEGF-D expression correlated with tumor stage ( P = 0.027) and N status ( P = 0.019). Groups of high HIF-1 α and VEGF-D showed worse survivals than those of low expression ( P = 0.002 and 0.001, respectively). Multivariate analysis supported expression of HIF-1 α and VEGF-D as significant survival predictors ( P = 0.044 and 0.035, respectively). A concordance rate of 69.5% was observed between expression of HIF-1 α and VEGF-D. In conclusion, protein expression of HIF-1 α and VEGF-D are independent prognostic predictors. An association between expression of HIF-1 α and VEGF-D suggests that these two angiogenic factors are essential in progression of ESCC.

    Abstract: Pyogenic liver abscess (PLA) is a potentially fatal disease that can stimulate prominent systemic inflammation. Osteoporotic hip fracture is a major complication of systemic inflammation. This study tried to determine the epidemiology of hip fractures among PLA patients. All subjects admitted due to PLA during 1999∼2010 were assessed, excluding the subjects with a history of high energy trauma, malignancy, and previous hip fracture. We matched the control subjects to PLA patients according to age, gender, and the coding of osteoporosis by 1 : 4 ratio. The PLA patients had a 1.17-fold risk of hip fracture than the controls (aHR = 1.17, 95% CI = 1.07–1.29) after adjusting for gender, age, and comorbidities. Considering death as the competing event of suicide, the PLA patients had 1.10-fold suicide risk (aHR = 1.10, 95% CI: 1.00–1.21) than the control subjects under the competing risks regression model. The cumulative incidence of hip fracture was higher in the PLA cohort (log-rank test, p 〈 0.001 ). When compared to the controls, the fracture risk was 18.4-fold (aHR = 18.4, 95% CI = 13.0–26.1) for the PLA patients admitted 2-3 times per year and 46.0-fold (aHR = 46.0, 95% CI = 31.2–67.8) for the PLA patients admitted ≧4 times per year. The impact of PLA is more prominent among the subjects aged 〈 45 years (aHR = 2.81, 95% CI = 1.42–5.56). Preventive measures for hip fracture might be warranted for PLA patients.

    Abstract: Introduction. Postoperative delayed hyponatremia is a complication associated with transsphenoidal pituitary surgery. Due to a wide spectrum of symptoms, the reported incidence and predictors of postoperative delayed hyponatremia vary among studies, and this deserves to be reviewed systematically. Methods. PubMed, EMBASE, and CENTRAL databases were searched until September 1, 2020. Studies were included when (1) the event number of delayed hyponatremia after transsphenoidal pituitary surgery was reported, or (2) the associated factors of such complication were evaluated. Results. A total of 27 studies were included for meta-analysis. The pooled incidence of overall and symptomatic delayed hyponatremia was 10.5% (95% confidence interval (CI) = 7.4–14.7%) and 5.0% (95% CI = 3.6–6.9%), respectively. No overt variations of the pooled estimates were observed upon subgroups stratified by endoscopic and microscopic procedure, publication year, and patients’ age. In addition, 44.3% (95% CI = 29.6–60.2%) of unplanned hospital readmissions within 30 days were caused by delayed hyponatremia. Among the predictors evaluated, older age was the only significant factor associated with increased delayed hyponatremia (odds ratio = 1.16, 95% CI = 1.04–1.29, P  = 0.006). Conclusion. This meta-analysis and systematic review evaluated the incidence of postoperative delayed hyponatremia and found it as a major cause of unplanned readmissions after transsphenoidal pituitary surgery. Older patients are more prone to such complications and should be carefully followed. The retrospective nature and heterogeneity among the included studies and the small number of studies used for risk factor evaluation might weaken the corresponding results. Future prospective clinical studies are required to compensate for these limitations.

    Abstract: Postcardiac arrest acidosis can decrease survival. Effective medications without adverse side effects are still not well characterized. We aimed to analyze whether early administration of glutamine could improve survival and protect cardiomyocytes from postcardiac arrest acidosis using animal and cell models. Forty Wistar rats with postcardiac arrest acidosis (blood pH 〈 7.2) were included. They were divided into study (500 mg/kg L-alanyl-L-glutamine, n = 20 ) and control (normal saline, n = 20 ) groups. Each of the rats received resuscitation. The outcomes were compared between the two groups. In addition, cardiomyocytes derived from human induced pluripotent stem cells were exposed to HBSS with different pH levels (7.3 or 6.5) or to culture medium (control). Apoptosis-related markers and beating function were analyzed. We found that the duration of survival was significantly longer in the study group ( p 〈 0.05 ). In addition, in pH 6.5 or pH 7.3 HBSS buffer, the expression levels of cell stress (p53) and apoptosis (caspase-3, Bcl-xL) markers were significantly lower in cardiomyocytes treated with 50 mM L-glutamine than those without L-glutamine (RT-PCR). L-glutamine also increased the beating function of cardiomyocytes, especially at the lower pH level (6.5). More importantly, glutamine decreased cardiomyocyte apoptosis and increased these cells’ beating function at a low pH level.

    Abstract: X-ray radiography is currently used in dentistry and can be divided into two categories: two-dimensional (2D) radiographic images (e.g., using periapical film, cephalometric film, and panoramic X-ray) and three-dimensional (3D) radiographic images (e.g., using dental cone-beam computed tomography (CBCT)). Among them, 2D periapical film images are most commonly used. However, 2D periapical film compresses 3D image information into a 2D image, which means that depth cannot be identified from the image. Such compressed images lose a considerable amount of information, reducing their clinical applicability. A 2.5D periapical radiography system prototype was developed by our research team. Our previous study indicated that this prototype could be used to capture images at different depths of an object. However, the prototype was limited by its commercially available intraoral periapical sensor, which had a low temporal resolution and could not capture multiple images in a short period of time. Therefore, the total time required for image capture was too long for practical clinical application. The present study developed a high-frame-rate intraoral periapical sensor with a sensor imaging speed of up to 15 Hz. The primary components of the developed intraoral periapical sensor include a scintillator, complementary metal oxide semiconductor chip, component circuit board, and video processing board. The external dimensions of the sensor are 41 × 26 × 6.6  mm 3 . The performance of the developed high-frame-rate intraoral periapical sensor was verified through qualified and quantified analyses using line pairs. The results showed that the resolution of the developed intraoral periapical sensor could reach 18 lp/mm. The sensor was further installed in our 2.5D periapical radiography system to conduct image capturing. The results indicated that the developed sensor could be used for high-frame-rate imaging to incorporate tomosynthesis to obtain reconstructed slice images of different depths. The developed sensor has the potential for clinical dentistry applications in the future.

    Abstract: Background . Type 2 diabetes mellitus patients are known to have higher risk of developing dementia while aspirin use has been shown to prevent incident dementia. This study was conducted to evaluate the potential benefits of aspirin use on dementia in patients with type 2 diabetes mellitus and identify the appropriate dosage of aspirin that provides the most benefit. Method . A Taiwan nationwide, population-based retrospective 8-year study was employed to analyze the association between the use of aspirin and incidence of dementia including Alzheimer’s disease and non-Alzheimer’s dementia using multivariate Cox-proportional hazards regression model and adjusting for several potential confounders. Results . Regular aspirin use in mean daily dosage of within 40 mg was associated with a decreased risk of developing incident Alzheimer’s dementia in patients with type 2 diabetes mellitus (adjusted HR of 0.51 with 95% CI of 0.27–0.97, p value 0.041). Conclusion . A mean daily dosage of aspirin use within 40 mg might decrease the risk of developing Alzheimer’s disease in patients with type 2 diabetes mellitus.
