    Abstract: Several research studies have revealed that migraine has a solid link with gastrointestinal diseases especially irritable bowel syndrome (IBS). This study was carried out to investigate therapeutic potential of diet based on IgG elimination combined with probiotics on migraine plus irritable bowel syndrome. A total of 60 patients diagnosed with migraine plus IBS were recruited for the study. IgG antibodies against 266 food varieties were detected by ELISA. Then, the subjects were randomized into three groups for treatment of IgG elimination diet or probiotics or diet combined with probiotics. Migraine symptom, gut function score, medication use, and serum serotonin level were measured at baseline, 7 weeks, and 14 weeks. Improvement of migraine and gut symptom was achieved at a certain time point. Reduced use of over-the-counter- (OTC−) analgesics was seen in all groups. However, use of triptans did not show significant difference. An increased serum serotonin level was seen in subjects treated with elimination diet and elimination diet combined with probiotics. IgG elimination diet combined with probiotics may be beneficial to migraine plus IBS. It may provide new insight by understanding the intricate relationship between migraine and gastrointestinal diseases.

    Abstract: Objective . To analyse the potential risk factors of nosocomial infections in patients with active rheumatoid arthritis (RA). Methods . A total of 2452 active RA patients at Hospitals in Shanghai between January 2009 and February 2011 were analyzed. Their demographic and clinical characteristics were compared with those without infection, and the potential risk factors were determined by logistic regression analysis. Results . Multivariate analysis indicated the gender ( OR = 0.70 , 95% CI 0.53–0.92), duration in hospital ( OR = 1.03 , 95%CI 1.01–1.05), number of organs involved ( OR = 0.82, 95%CI 0.72–0.92), number of disease-modifying antirheumatic drugs ((DMARDs) ( OR = 1.22, 95%CI 1.061–1.40)), corticosteroid therapy ( OR = 1.02, 95%CI 1.01–1.03), peripheral white blood cell counts ((WBC) ( OR = 1.04, 95%CI 1.00–1.08)), levels of serum albumin ( OR = 0.98 , 95%CI 0.97–0.99), and C-reactive protein ((CRP) ( OR = 1.03 , 95%CI 1.01–1.04)) that were significantly associated with the risk of infections. Conclusion . The female patients, longer hospital stay, more organs involved, more DMARDs, corticosteroid usage, high counts of WBC, lower serum albumin, and higher serum CRP were independent risk factors of infections in active RA patients.

    Abstract: The Triassic Zhangjiatan shale in the Ordos basin, recognized as one of most promising shale oil and gas resources in China, is one example of medium-low-maturity lacustrine shale reservoirs wherein the pore network has been proven to be different from those of both conventional and high-maturity marine shale reservoirs. Here, we examine the quantitative characterization of pore size and movable fluid distribution of the Zhangjiatan shale reservoir. Eight shale core plugs were measured in a series of nuclear magnetic resonance (NMR) experiments to determine the pore structure and pore fluid transport during the processes of centrifuging and heating. The complete pore size distribution was obtained from the T 2 spectrum by integrating NMR, high-pressure mercury intrusion, and nitrogen gas adsorption. The results show that the movable fluid saturation of Zhangjiatan shale is 19.47–33.02% and the capillary-bound fluid saturation is 14.53–27.62%, whereas the unrecoverable fluid saturation was 43.51–65.26%. The movable fluids are mainly detected in macropores and mesopores with a minimum pore size of 50.5–121.2 nm, while capillary-bound fluids are mainly found in small pores and some mesopores with a minimum size of 25.8–67.5 nm. Almost all unrecoverable fluids are in micropores and small pores. In contrast to high-maturity shale reservoirs, the Zhangjiatan shale shows a poor correlation between total organic carbon content and unrecoverable fluid saturation but a good correlation between clay minerals and unrecoverable fluid saturation, showing that most micropores are associated with clay minerals and the organic matter pores are less developed. This study provides an accurate determination of the pore size distribution and pore fluid typing of Zhangjiatan shale, which is of great significance for the development of high-quality but medium-low-maturity shale reservoirs in China.

    Abstract: Objective. To better understand the status of medical treatment for human epidermal growth factor receptor 2 (HER2)-positive breast cancer and the differences between the Chinese and the international clinical practice. Methods. This was a retrospective, nationwide, multicenter, epidemiological study of advanced breast cancer patients from China. Between January 01, 2012, and December 31, 2014, a total of 3649 patients, covering 7 geographic regions and 21 institutions, participated in this series of studies. HER2-positive breast cancer was selected among the group and adopted into this study. In comparison, we summarized the demographics and clinical characteristics of HER2-positive breast cancer from the Surveillance, Epidemiology, and End Results (SEER) database. Results. A total of 918 patients diagnosed as HER2-positive breast cancer patients were included. The median age at diagnosis was 46 years (ranging, 23 to 78) with a single-peak incidence. The proportions of stages II–IV at diagnosis and distance metastasis in viscera were more than half of the participants. In comparison, the prevalence of estrogen or progesterone receptor-positive expression and luminalB subtype was relatively lower than that of the United States. The receipt of chemotherapy was fairly higher, while the usage of targeted therapy was seriously insufficient. Tumor size was in significantly positive associations with the duration of targeted therapy (Kendall’s correlation coefficient = 0.3, P 〈 0.0001 ), while no prohibitive variables among clinical characteristics were detected. Conclusion. Our study suggested that HER2-positive breast cancer patients were characterized as a younger trend, a lower prevalence of hormonal receptor (HR)-positive expression, and less accessible to anti-HER2 targeted therapy with insufficient duration over the past few years in China. Concerted efforts should be exerted for promising survival benefits in the future. The trial registration number is https://clinicaltrials.gov/ct2/show/NCT03047889.

    Abstract: Background. It is critical to accurately identify patients with severe acute pancreatitis (SAP) and moderately SAP (MSAP) in a timely manner. The study was done to establish two early multi-indicator prediction models of MSAP and SAP. Methods. Clinical data of 469 patients with acute pancreatitis (AP) between 2015 and 2020, at the First Affiliated Hospital of Fujian Medical University, and between 2012 and 2020, at the Affiliated Union Hospital of Fujian Medical University, were retrospectively analyzed. The unweighted predictive score (unwScore) and weighted predictive score (wScore) for MSAP and SAP were derived using logistic regression analysis and were compared with four existing systems using receiver operating characteristic curves. Results. Seven prognostic indicators were selected for incorporation into models, including white blood cell count, lactate dehydrogenase, C-reactive protein, triglyceride, D-dimer, serum potassium, and serum calcium. The cut-offs of the unwScore and wScore for predicting severity were set as 3 points and 0.513 points, respectively. The unwScore ( AUC = 0.854 ) and wScore ( AUC = 0.837 ) were superior to the acute physiology and chronic health evaluation II score ( AUC = 0.526 ), the bedside index for severity in AP score ( AUC = 0.766 ), and the Ranson score ( AUC = 0.693 ) in predicting MSAP and SAP, which were equivalent to the modified computed tomography severity index score ( AUC = 0.823 ). Conclusions. The unwScore and wScore have good predictive value for MSAP and SAP, which could provide a valuable clinical reference for management and treatment.

    Abstract: 18F-FDG PET/MRI has been applied to the diagnosis and preoperative staging in various tumor types; however, reports using PET/MRI in gastric cancer are rare because of motion artifacts. We investigated the value of PET/MRI for preoperative staging compared with PET/CT in gastric cancer (GC). Thirty patients with confirmed GC underwent PET/CT and PET/MRI. TNM staging for each patient was determined from the PET/MRI and PET/CT images. The diagnostic performance of PET/MRI and PET/CT was calculated compared with the pathologic TNM stage. The two methods were compared using statistical analyses. The accuracy for T staging between PET/MRI and PET/CT was 76.9% vs. 57.7%, respectively. In T 1 and T 4a staging, the sensitivity and specificity for PET/MRI vs. PET/CT was 1.0 vs. 0.6 and 1.0 vs. 0.8, respectively. The area under the curve (AUC) for PET/MRI vs. PET/CT was 1.00 vs. 0.78 in the T 1 stage, 0.73 vs. 0.66 in the T 2 stage, 0.72 vs. 0.57 in the T 3 stage, and 0.86 vs. 0.83 in the T 4 stage. The accuracy for N staging of PET/MRI vs. PET/CT was 53.9% vs. 34.0%, and that for N 0 vs. N + was 85.0% vs. 77.0%. The sensitivity for PET/MRI in N3 staging was 0.67 and 0 for PET/CT. There was a statistically significant difference in the AUC for N 1 staging (PET/MRI vs. PET/CT, 0.63 vs. 0.53, p = 0.03 ). SUVmax/ADC positively correlated with tumor volume and Ki-67. PET/MRI performs more accurately in TNM staging compared with PET/CT and is optimal for accurate N staging. SUVmax/ADC has positive correlations with tumor volume and Ki-67.

    Abstract: CU test and CD test were performed to capture the stress-strain characteristics of the clay in Dongting Lake area, China. Since the classical constitutive model cannot ideally describe the strain hardening behavior of the clay, an improved constitutive model that involved three damage factors was proposed. The damage first factor D 1 , which represents elemental volume strength F , was fitted by the Weibull function. The second and third damage factors D 2 and D 3 , which represent the porosity ratio e and the drainage rate P ε respectively, were fitted by Harris function. The CD test damage model is constructed by D 1 , D 2 , and D 3 . In the CU test, the porosity ratio e is a constant value, and the drainage volume is 0, so that D 2 is constant value and D 3 is 0. Therefore, the CU test damage model of the clay was constructed using the damage first factor D 1 . In the end, the accuracy of the proposed CU/CD damage constitutive models of the clay was validated with the experimental results. It is found that the results fitted by the proposed methodology are in good agreement with the experimental results.

    Abstract: Background and Aims. Elevated serum uric acid (SUA) is associated with an increased risk of nonalcoholic fatty liver disease (NAFLD); however, whether this association is causal is undetermined. Methods. Each participant from the Dongfeng-Tongji cohort study based on 27,009 retirees was interviewed face-to-face following a clinical examination. Covariance, logistic regression analysis, and instrumental variables were used to assess associations between SUA and (severity of) NAFLD and the causal link. Results. Among 8,429 subjects free of NAFLD at baseline, 2,007 participants developed NAFLD after 5 years of follow-up. The multivariable-adjusted odds ratio (OR) for NAFLD for individuals in the fourth quartile of SUA level versus those in the first was 1.71 (95% CI: 1.45-2.01, P for trend 〈 0.001) and was more dramatic in women or normal-weight persons. Furthermore, SUA was materially associated with greater mean markers of hepatic necroinflammation and greater probabilities of fibrosis. In genetic analyses, both single nucleotide polymorphisms (rs11722228 to SLC2A9 and rs2231142 to ABCG2) were pronouncedly associated with increased SUA concentrations, ranging from 0.19 to 0.22 mg/dl. No significant associations were observed between SNPs and potential confounders. No association was observed between the SUA-increasing allele and NAFLD, with an OR of 0.98 (95% CI: 0.90-1.08) per genetic score. This was not significantly different ( P = 0.25 ) from what was expected (1.03, 95% CI: 1.03-1.03). Conclusions. SUA was positively associated with NAFLD incidence especially in female and normal-weight individuals and the suspected progression risk of newly developed NAFLD. However, the Mendelian randomization analyses lend no causal evidence, suggesting high SUA as a marker and not a cause of NAFLD.
