    Abstract: Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is relatively rare in children. This article aimed to analyze clinical and renal histology findings and different responses to induction treatment associated with the long-term renal outcomes in children with AAV in a single center. Methods All pediatric patients with AAV admitted to Tongji Hospital from January 2002 to January 2021 were included in the study. The demographic, clinical, pathological, laboratory, and treatment data and outcomes were collected and analyzed to identify predictors associated with response to induction treatment and progression to end-stage renal disease (ESRD). Results In total, 48 children with AAV were included in this cohort; 81.25% of them were women, and 91.7% were microscopic polyangiitis (MPA). Kidney involvement was found in 45 patients (93.75%). The most common histopathological subtype was crescentic form in this cohort according to Berden’s classification. In total, 34 patients (70.8%) showed eGFR & lt;60 ml/min/1.73 m 2 at the time of diagnosis. Complete and partial remission was achieved in 8 patients (16.7%) and 19 patients (39.6%), respectively, following 6-month induction treatment. Half of the patients eventually progressed to ESRD at a mean time of (13.04 ± 15.83) months after diagnosis. The independent predictors of nonremission following induction treatment and progression to ESRD were baseline eGFR & lt;60 ml/min/1.73 m 2 and hypertension at diagnosis. Renal survival significantly decreased over time in patients with renal sclerotic subtypes or those with nonremission following induction treatment by Kaplan–Meier curve estimation. Conclusions Our study demonstrates that women, MPA, and crescentic subtypes are predominant in pediatric AAV in China. Initial renal failure (eGFR & lt;60 ml/min/1.73 m 2 ), hypertension, sclerotic pathological subtype, and nonremission following induction treatment are predictive of long-term renal outcomes.

    Abstract: Scholars have proposed the practice of split nitrogen fertilizer application (SNFA), which has proven to be an effective approach for enhancing nitrogen use efficiency. However, the combined effects of SNFA on wheat plant nitrogen use efficiency, ammonia (NH 3 ) emission flux, as well as the rates of nitrification and denitrification in different ecosystems remain unclear. Meanwhile, few studies have sought to understand the effects of the split nitrogen fertilizer method under water-saving irrigation technology conditions on nitrogen loss. The current study assessed soil NH 3 volatilization, nitrification, and denitrification intensities, as well as the abundance of nitrogen cycle-related functional genes following application of different treatments. Specifically, we applied a nitrogen rate of 240 kg⋅ha –1 , and the following fertilizer ratios of the percent base to that of topdressing under water-saving irrigation: N1 (basal/dressing, 100/0%), N2 (basal/dressing, 70/30%), N3 (basal/dressing, 50/50%), N4 (basal/dressing, 30/70%), and N5 (basal/dressing, 0/100%). N3 treatment significantly reduced NH 3 volatilization, nitrification, and denitrification intensities, primarily owing to the reduced reaction substrate concentration (NO 3 – and NH 4 + ) and abundance of functional genes involved in the nitrogen cycle ( amoA- AOB, nirK , and nirS ) within the wheat-land soil. 15 N tracer studies further demonstrated that N3 treatments significantly increased the grain nitrogen accumulation by 9.50–28.27% compared with that under other treatments. This increase was primarily due to an increase in the amount of nitrogen absorbed by wheat from soil and fertilizers, which was caused by an enhancement in total nitrogen uptake (7.2–21.81%). Overall, N3 treatment (basal/dressing, 50/50%) was found to effectively reduce nitrogen loss through NH 3 volatilization, nitrification and denitrification while improving nitrogen uptake by wheat. Thus, its application will serve to further maximize the yield and provide a fertilization practice that will facilitate cleaner wheat production in the North China Plain.

    Abstract: Clinical cardiovascular disease (CVD) and cognition impairment are common and often coexist in aging populations, and CVD is associated with greater cognition impairment risk; however, the association between cognition impairment and CVD risk is inconsistent. It is unknown if pathways that contribute to CVD are caused by impaired cognition. We hypothesized that cognition impairment would be associated with greater subclinical CVD including subclinical myocardial damage [assessed by high-sensitivity cardiac troponin T (hs-cTnT)] and cardiac strain or dysfunction [assessed by N-terminal pro-B-type natriuretic peptide (NT-proBNP)] . Methods This analysis included Atherosclerosis Risk in Communities Study (ARIC) participants who underwent global cognition z -score tests between 1991 and 1993. Cardiac biomarkers were measured from stored plasma samples collected between 1996 and 1999. Logistic regression models were used to determine the association of cognitive function with subclinical CVD risk. Results There were 558/9216 (6.1%) and 447/9097 (5.0%) participants with incident elevated hs-CTnT (≥14 ng/L) and NT-proBNP (≥300 pg/mL) levels, respectively. Comparing the lowest and highest quartiles of global cognition z -scores, a higher incidence of elevated hs-CTnT [odds ratio (OR) = 1.511, 95% confidence interval (CI): 1.093–2.088, P = 0.013] and NT-proBNP (OR = 1.929, 95% CI: 1.350–2.755, P & lt; 0.001) were observed, respectively. In structural equation modeling, the indirect effect of global cognition z -score on major adverse cardiac events was 42.1% ( P & lt; 0.05). Conclusion Impairments in baseline cognitive function were associated with subclinical myocardial damage or wall strain. Although future studies are warranted to investigate the pathophysiological mechanisms behind these associations, our study suggests common pathways between cognitive and cardiac dysfunction.

    Abstract: As a validated assessment tool for functional disability (activities of daily living), the Barthel index (BI) assessed initially at admission has the potential to stratify patients with high-risk acute coronary syndrome (ACS). Dynamic trajectory evaluation of functional capacity in hospitals may provide more prognostic information. We aimed to establish a novel dynamic BI-based risk stratification program (DBRP) during hospitalization to predict outcomes among ACS patients. Methods A total of 2,837 ACS patients were included from the Retrospective Multicenter Study for Early Evaluation of Acute Chest Pain. The DBRP rating (low, medium, and high-risk categories) was calculated from dynamic BI at admission and discharge. The primary outcome was all-cause mortality, and the secondary outcome was cardiac mortality. Results Of all the included patients, 312 (11%) died during a median follow-up period of 18.0 months. Kaplan–Meier analysis revealed that the cumulative mortality was significantly higher in patients in the higher risk category according to the DBRP. Multivariable Cox regression analysis indicated that, compared to the low-risk category, the higher risk category in the DBRP was an independent strong predictor of all-cause mortality after adjusting for confounding factors (medium-risk category: hazard ratio [HR]: 1.756, 95% confidence interval [95% CI] : 1.214–2.540; P = 0.003; high-risk category: HR: 5.052, 95% CI: 3.744–6.817; P & lt; 0.001), and the same result was found for cardiac mortality. Conclusion The DBRP was a useful risk stratification tool for the early dynamic assessment of patients with ACS.     Abstract: Emerging evidence is examining the precise role of intestinal microbiota in the pathogenesis of type 2 diabetes. The aim of this study was to investigate the association of intestinal microbiota and microbiota-generated metabolites with glucose metabolism systematically in a large cross-sectional study in China. 1160 subjects were divided into three groups based on their glucose level: normal glucose group (n=504), prediabetes group (n=394), and diabetes group (n=262). Plasma concentrations of TMAO, choline, betaine, and carnitine were measured. Intestinal microbiota was measured in a subgroup of 161 controls, 144 prediabetes and 56 diabetes by using metagenomics sequencing. We identified that plasma choline [Per SD of log-transformed change: odds ratio 1.36 (95 confidence interval 1.16, 1.58)] was positively, while betaine [0.77 (0.66, 0.89)] was negatively associated with diabetes, independently of TMAO. Individuals with diabetes could be accurately distinguished from controls by integrating data on choline, and certain microbiota species, as well as traditional risk factors (AUC=0.971). KOs associated with the carbohydrate metabolism pathway were enhanced in individuals with high choline level. The functional shift in the carbohydrate metabolism pathway in high choline group was driven by species Ruminococcus lactaris , Coprococcus catus and Prevotella copri . We demonstrated the potential ability for classifying diabetic population by choline and specific species, and provided a novel insight of choline metabolism linking the microbiota to impaired glucose metabolism and diabetes.

    Abstract: The fluid mobility characteristics in the pores with various apertures for tight-sandstone would finally determine the fluid mobility and production of tight oil and gas reservoirs. In this study, the tight-sandstone core samples collected from Middle Jurassic Xishanyao Formation in Santanghu Basin were launched the fluid mobility measurements under various centrifugal speeds. With the NMR fractal theory pore classification method, the various types of pores in the tight-sandstone core samples were classified, and the fluid mobility in different types of pores were also investigated. The results show that the tights-sandstone core samples were significantly influenced by compaction, and the core samples are relatively dense, the mineral intergranular solution pores and colloidal intergranular pores are the main storage spaces. With a constant increase of the centrifugal speed, the fluid mobility increases continuously, and the fluid mobility for CTOS-19 features stronger than that of CTOS-7, which is related to the complexity of pore structure in tight-sandstone sample. Compared with the pore aperture in CTOS-19, the pore aperture in CTOS-7 is smaller, and the connectivity between the smaller and larger pores is poorer, leading to the poorer fluid mobility. Besides, the NMR fractal theory pore classification method also shows that the COTS-7 features more pore types than COTS-19, five and four types respectively. The type P2 and P3 pores are dominant in COTS-7 and CTOS-19 core samples, and the connectivity between type P2 and P3 pores contributes dominantly to the fluid mobility. With the NMR fractal theory pore classification method, the complexity of the distribution of fluid and fluid mobility in tight reservoirs could be studied quantitatively, and the results can efficiently guide the development of residual oil and gas in tight oil reservoirs.

    Abstract: Allogeneic amnion tissues have been widely used in tissue repair and regeneration, especially a remarkable trend of clinical uses in chronic wound repair. The virus inactivation procedures are necessary and required to be verified for the clinical use and approval of biological products. Cobalt-60 (Co-60) or electron-beam (e-beam) is the common procedure for virus and bacterial reduction, but the excessive dose of irradiation was reported to be harmful to biological products. Herein, we present a riboflavin (RB)-ultraviolet light (UV) method for virus inactivation of amnion and chorion tissues. We used the standard in vitro limiting dilution assay to test the viral reduction capacity of the RB-UV method on amnion or chorion tissues loaded with four types of model viruses. We found RB-UV was a very effective procedure for inactivating viruses of amnion and chorion tissues, which could be used as a complementary method to Co-60 irradiation. In addition, we also screened the washing solutions and drying methods for the retention of growth factors.
