    Abstract: Objectives: To evaluate excess deaths of gastrointestinal, liver, and pancreatic diseases in the United States during the COVID-19 pandemic. Methods: We retrieved weekly death counts from National Vital Statistics System and fitted them with a quasi-Poisson regression model. Cause-specific excess deaths were calculated by the difference between observed and expected deaths with adjustment for temporal trend and seasonality. Demographic disparities and temporal-spatial patterns were evaluated for different diseases. Results: From March 2020 to September 2022, the increased mortality (measured by excess risks) for Clostridium difficile colitis, gastrointestinal hemorrhage, and acute pancreatitis were 35.9%; 24.8%; and 20.6% higher than the expected. For alcoholic liver disease, fibrosis/cirrhosis, and hepatic failure, the excess risks were 1.4–2.8 times higher among younger inhabitants than older inhabitants. The excess deaths of selected diseases were persistently observed across multiple epidemic waves with fluctuating trends for gastrointestinal hemorrhage and fibrosis/cirrhosis and an increasing trend for C. difficile colitis. Conclusion: The persistently observed excess deaths of digestive diseases highlights the importance for healthcare authorities to develop sustainable strategies in response to the long-term circulating of SARS-CoV-2 in the community.

    Abstract: IHNV is a virus that infects salmonids and causes serious economic damage to the salmonid farming industry. There is no specific treatment for the disease caused by this pathogen and the main preventive measure is vaccination, but this is only possible for small groups of individuals. Therefore, it is important to investigate new oral vaccines to prevent IHNV. In this study, the CK6 chemokine protein of rainbow trout and the truncated G protein of IHNV were used to construct a secretory expression recombinant L.casei vaccine for rainbow trout. The results showed that the levels of IgM and IgT antibodies in rainbow trout reached the highest level on the 15th day after the secondary immunization, and the antibodies exhibited high inhibitory activity against viral infection. Furthermore, the expression of relevant cytokines in different tissues was detected and found to be significantly higher in the oral vaccine group than in the control group. It was also found that pPG-612-CK6-G/ L.casei 393 could stimulate splenic lymphocyte proliferation and improve mucosal immunity with significant differences between the immunized and control groups. When infected with IHNV, the protection rate of pPG-612-CK6-G/ L.casei 393 was 66.67% higher than that of the control group. We found that pPG-612-CK6-G/ L.casei 393 expressed and secreted the rainbow trout chemokine CK6 protein and IHNV truncated G protein, retaining the original immunogenicity of rainbow trout while enhancing their survival rate. This indicates that recombinant L.casei provides a theoretical basis and rationale for the development of an oral vaccine against IHNV and has important practical implications for the protection of rainbow trout from IHNV infection.

    Abstract: Polycyclic aromatic hydrocarbons (PAHs) are widely distributed in soil and are difficult to degrade, posing a great threat to the ecological environment and human health. Therefore, research on the distribution characteristics and risks of PAHs is of great significance to protect human and ecosystem health. Taking a typical chemical industry park in Chongqing as an example, the spatial distribution characteristics of PAHs content in 54 topsoil samples in the typical area were analyzed, and the soil PAHs pollution was evaluated by incremental models such as single-factor index and Nemerow comprehensive index. A factor decomposition model Positive Definite Matrix Factorization (PMF) was used to analyze the sources of PAHs. The results showed that 16 kinds of optimally controlled PAHs were detected, and the content of ΣPAHs in the topsoil ranged from ND to 16.07 mg/kg, with an average value of 1.78 mg/kg; spatially, pollutant levels are higher in the south and southwest of the park as well as in the center; source analysis showed that Chongqing The PAHs pollution in this typical chemical industry park in the city is from coke combustion sources, traffic emission sources, biomass combustion sources, oil sources, coal combustion sources and oil leakage sources, and the contribution rates to PAHs are 10.7%, 35.2%, 20.7%, and 5.0%, 24.6%, and 3.7%; respectively. The health risk assessment of soil PAHs shows that there is no potential carcinogenic risk of PAHs in different age groups in this area, and the main exposure route of adults is dermal & gt; ingestion & gt; inhalation, and the main exposure route of children is ingestion & gt; dermal & gt; inhalation.

    Abstract: Under the comprehensive reform of its household registration system, China has experienced the largest and fastest urbanization process in the world since its establishment 70 years ago, but with the deepening of urbanization, cities are facing greater environmental challenges. As the total amount of urban wastewater discharge increases, the condition of water pollution nationwide has become more severe. Thus, through the use of dynamic exogenous variable data envelopment analysis (DEA) this research selects the employment population and input cost of wastewater treatment in 31 provinces of China from 2013 to 2017 as input indicators, gross domestic product (GDP), wastewater production, and chemical oxygen demand (COD) in wastewater as output indicators, fixed assets as a carry-over variable, and urbanization level as an important influencing factor to evaluate and compare wastewater treatment efficiency in the northeast, central, western, and eastern regions with and without the influence of the level of urbanization. Findings show that the total efficiency score under the influence of the level of urbanization is generally higher than that without such an influence. The level of urbanization has a significantly positive effect on wastewater output efficiency in various regions, whereas wastewater output efficiency presents a downward trend from 2013 to 2017. The overall level of chemical oxygen demand efficiency is weak. When affected by the level of urbanization, the scores of COD efficiency in the eastern, northeast, and central regions have increased significantly. Finally, there is a large inter-provincial gap in the level of urbanization in the western region, while the level of urbanization has no significant impact on COD efficiency.

    Abstract: Calcified aortic valve disease (CAVD), the most common valvular heart disease, lacks pharmaceutical treatment options because its pathogenesis remains unclear. This disease with a complex macroenvironment characterizes notable cellular heterogeneity. Therefore, a comprehensive understanding of cellular diversity and cell-to-cell communication are essential for elucidating the mechanisms driving CAVD progression and developing therapeutic targets. In this study, we used single-cell RNA sequencing (scRNA-seq) analysis to describe the comprehensive transcriptomic landscape and cell-to-cell interactions. The transitional valvular endothelial cells (tVECs), an intermediate state during the endothelial-to-mesenchymal transition (EndMT), could be a target to interfere with EndMT progression. Moreover, matrix valvular interstitial cells (mVICs) with high expression of midkine (MDK) interact with activated valvular interstitial cells (aVICs) and compliment-activated valvular interstitial cells (cVICs) through the MK pathway. Then, MDK inhibited calcification of VICs that calcification was validated by Alizarin Red S staining, real-time quantitative polymerase chain reaction (RT-qPCR), and Western blotting assays in vitro . Therefore, we speculated that mVICs secreted MDK to prevent VICs’ calcification. Together, these findings delineate the aortic valve cells’ heterogeneity, underlining the importance of intercellular cross talk and MDK, which may offer a potential therapeutic strategy as a novel inhibitor of CAVD.

    Abstract: This study aimed to assess the circumferential and longitudinal strain of the fetal ascending aortic (AA) wall and establish a gestational age-associated longitudinal reference for aortic wall strain during the second half of pregnancy. Methods Singleton fetuses with gestational age (GA) at 20 + 0 to 24 + 6 weeks were prospectively collected from a low-risk population. Global circumferential strain (GCS) and mean longitudinal strain (MLS) of the ascending aorta were measured serially at 4-week intervals using the velocity vector imaging (VVI) technique. Fractional polynomials were conducted to obtain the best-fitting curves between GA and AA strains. GA-specific reference percentiles of GCS and MLS were established by multilevel modeling. Results A total of 223 fetuses with a total of 1,127 serial observations were enrolled. GCS presented a second-degree fractional polynomial smoothing regression along GA ( R 2 = 0.635, P & lt; 0.05). Fetal aortic GCS remained unchanged at ~27.29% (20.36–35.6%) before 31 weeks and increased significantly from 31.36% (26.38–37.12%) at 31 weeks to 43.29% (30.5–56.78%) at term. MLS presented a third-degree fractional polynomial smoothing regression along GA ( R 2 = 0.465, P & lt; 0.05). MLS remained steady at ~10.03% (3.28–17.62%) between 20 and 31 weeks and then increased significantly from 12.68% (7.42–20.1%) at 32 weeks to 17.5% (9.67–25.34%) at term. The GCS was significantly higher than the MLS in the ascending aorta wall ( p & lt; 0.001). Conclusion The fetal ascending aorta wall demonstrates obviously greater circumferential strain than longitudinal strain. Both strains remained steady before the late trimester and then gradually increased until delivery, suggesting progressive maturation of aortic elasticity mechanics.

    Abstract: The relationship between acute myeloid leukemia (AML) or acute lymphoblastic leukemia (ALL) and cardiac function is not well established. This study aimed to evaluate whether AML patients exist early myocardial damages prior to chemotherapy and to investigate its association with cardiovascular biomarkers. Methods Conventional echocardiography and three-dimensional speckle-tracking strain analysis were performed prospectively in 72 acute leukemia (AL) patients before any chemotherapy therapy (of whom 44 were AML patients, 28 ALL patients). The results were compared with those from 58 control group matched for age and gender. Results There were no significant differences in conventional biventricular systolic function parameters between AL patients and controls. The left ventricular global longitudinal strain (LVGLS) and right ventricular free wall longitudinal strain (RVFWLS) were significantly lower in AL patients (−23.0 ± 1.4% vs. −24.1 ± 1.3% and −27.9 ± 7.1% vs. −33.0 ± 4.6%, respectively, P   & lt; 0.001 for all). Compared with ALL patients, AML patients had lower LVGLS and RVFWLS (−22.7 ± 1.3% vs. −23.5 ± 1.6% and −26.2 ± 7.6% vs. −30.4 ± 5.5%, respectively, P   & lt; 0.05 for all). LVGLS was lower in ALL patients compared with controls (−23.5 ± 1.6% vs. −24.7 ± 1.4%, P   & lt; 0.05), however, there was no difference in right ventricular systolic function parameters between the two groups. LVGLS in AL patients was independently correlated with left ventricular ejection fraction (LVEF) and the absolute number of circulating lymphocytes. Conclusions Our findings suggest that baseline myocardial systolic function is lower in AL patients than controls. AML patients had lower baseline LVGLS and RVFWLS than controls and ALL patients. The decreased LVGLS is correlated with LVEF and the absolute number of circulating lymphocytes.

    Abstract: The purpose of this study was to investigate the prognostic significance of serum albumin to creatinine ratio (ACR) in patients receiving heart transplantation of end-stage heart failure. Methods From January 2015 to December 2020, a total of 460 patients who underwent heart transplantation were included in this retrospective analysis. According to the maximum Youden index, the optimal cut-off value was identified. Kaplan-Meier methods were used to describe survival rates, and multivariable analyses were conducted with Cox proportional hazard models. Meanwhile, logistic regression analysis was applied to evaluate predictors for postoperative complications. The accuracy of risk prediction was evaluated by using the concordance index (C-index) and calibration plots. Results The optimal cut-off value was 37.54 for ACR. Univariable analysis indicated that recipient age, IABP, RAAS, BB, Hb, urea nitrogen, D-dimer, troponin, TG, and ACR were significant prognostic factors of overall survival (OS). Multivariate analysis showed that preoperative ACR (HR: 0.504, 95% = 0.352–0.722, P   & lt; 0.001) was still an independent prognostic factor of OS. The nomogram for predicting 1-year and 5-year OS in patients who underwent heart transplantation without ACR (C-index = 0.631) and with ACR (C-index = 0.671). Besides, preoperative ACR level was a significant independent predictor of postoperative respiratory complications, renal complications, liver injury, infection and in-hospital death. Moreover, the calibration plot showed good consistency between the predictions by the nomogram for OS and the actual outcomes. Conclusion Our research showed that ACR is a favorable prognostic indicator in patients of heart transplantation.

    Abstract: Background: Tetralogy of Fallot (TOF) is one of the most common cyanotic congenital heart defects (CHDs). The patterns of fetal myocardial deformations in TOF have not been well-studied. This study aimed to assess biventricular myocardial deformations in fetuses with TOF compared with normal fetuses. Methods: A retrospective cohort study of fetuses with TOF and gestational age (GA)-matched controls was conducted at a single tertiary referral center from 2014 to 2020. All enrolled fetuses underwent detailed echocardiography, and four-chamber video-clips were recorded and analyzed offline for deformation assessment by using two-dimensional speckle tracking echocardiography (2D-STE). Comparisons for baseline characteristics, cardiac morphological measurements (ventricular, atrial, and great arterial diameters or ratios, global sphericity index), systolic function parameters [ejection fraction (EF), fractional area change (FAC)], and strain parameters [global longitudinal strain (GLS), global longitudinal strain rate in systole and diastole (GLSRs, GLSRd)] were performed between fetuses with TOF and GA-matched controls. Results: Fifty-two fetuses with TOF and 52 GA-matched controls were enrolled in this study. Fetuses with TOF exhibited similar left ventricular (LV) EF (58.51 ± 5.11% vs. 57.59 ± 5.38%, P = 0.16) and right ventricular (RV) FAC (43.64 ± 2.89% vs. 44.27 ± 3.04%, P = 0.25), compared to normal fetuses. While, in deformational analysis, TOF fetuses demonstrated significantly lower LV and RV GLS values (−22.57 ± 2.91% vs. −27.39 ± 4.38%, P & lt; 0.001 for LV GLS; −24.27 ± 3.18% vs. −28.71 ± 4.48%, P & lt; 0.001 for RV GLS). Both LV GLS ( r = −0.518, P & lt; 0.001) and RV GLS ( r = −0.534, P & lt; 0.001) were found negatively correlated with the aortic valve-to-pulmonary valve diameter ratio (AV:PV ratio). Z -scores of PV annulus and main pulmonary artery (MPA) also had positive correlation with LV and RV GLS, respectively. Conclusions: Decreased biventricular myocardial deformations can appear even in fetuses with TOF with normal systolic ventricular function. Both LV and RV GLS values are correlated with the severity of right ventricular outflow tract obstruction. It indicates 2D-STE may be a more sensitive tool to assess fetal cardiac function than the conventional echocardiographic methods.

    Abstract: Bacterial cells in their natural environments encounter rapid and large changes in external osmolality. For instance, enteric bacteria such as Escherichia coli experience a rapid decrease when they exit from host intestines. Changes in osmolality alter the mechanical load on the cell envelope, and previous studies have shown that large osmotic shocks can slow down bacterial growth and impact cytoplasmic diffusion. However, it remains unclear how cells maintain envelope integrity and regulate envelope synthesis in response to osmotic shocks. In this study, we developed an agarose pad-based protocol to assay envelope stiffness by measuring population-averaged cell length before and after a hyperosmotic shock. Pad-based measurements exhibited an apparently larger length change compared with single-cell dynamics in a microfluidic device, which we found was quantitatively explained by a transient increase in division rate after the shock. Inhibiting cell division led to consistent measurements between agarose pad-based and microfluidic measurements. Directly after hyperosmotic shock, FtsZ concentration and Z-ring intensity increased, and the rate of septum constriction increased. These findings establish an agarose pad-based protocol for quantifying cell envelope stiffness, and demonstrate that mechanical perturbations can have profound effects on bacterial physiology.
