    Abstract: Upfront autologous stem cell transplantation (ASCT) has been recommended for patients who are newly diagnosed with peripheral T-cell lymphoma (PTCL), and CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisone), an anthracycline-based chemotherapy has been the frontline chemotherapy for PTCL. However, it is not clear whether anthracycline-based chemotherapies such as CHOP could be standard induction therapy for PTCL. Methods We conducted a randomized phase II study to compare CHOP with fractionated ifosfamide, carboplatin, etoposide, and dexamethasone (ICED) for patients eligible for ASCT. The primary endpoint was progression-free survival (PFS) and secondary endpoints included objective response rate, overall survival (OS), and safety profiles. Results Patients were randomized into either CHOP (n = 69) or ICED (n = 66), and the characteristics of both arms were not different. PTCL-not otherwise specified (NOS, n = 60) and angioimmunoblastic T-cell lymphoma (AITL, n = 53) were dominant. The objective response rate was not different between CHOP (59.4%) and ICED (56.1%), and the 3-year PFS was not different between CHOP (36.7%) and ICED (33.1%). In AITL patients, CHOP was favored over ICED whereas ICED was associated with more cytopenia and reduced dose intensity. Patients who received upfront ASCT after achieving complete response to CHOP or ICED showed 80% of 3-year OS. Discussion In summary, our study showed no therapeutic difference between CHOP and ICED in terms of response and PFS. Thus, CHOP might remain the reference regimen especially for AITL based on its better outcome in AITL, and upfront ASCT could be recommended as a consolidation of complete response in patients with PTCL.

    Abstract: Preclinical studies suggest that repeated exposure to anesthetics during a critical period of neurodevelopment induces long-term changes in synaptic transmission, plasticity, and behavior. Such changes are of great concern, as similar changes have also been identified in animal models of neurodevelopmental disorders (NDDs) such as autism. Because of overlapping synaptic changes, it is also possible that anesthetic exposures have a more significant effect in individuals diagnosed with NDDs. Thus, we evaluated the effects of early, multiple anesthetic exposures in BTBR mice, an inbred strain that displays autistic behavior. We discovered that three cycles of sevoflurane anesthesia (2.5%, 1 h) with 2-h intervals between each exposure in late postnatal BTBR mice did not aggravate, but instead improved pathophysiological mechanisms involved with autistic behavior. Sevoflurane exposures restored E/I balance (by increasing inhibitory synaptic transmission), and increased mitochondrial respiration and BDNF signaling in BTBR mice. Most importantly, such changes were associated with reduced autistic behavior in BTBR mice, as sociability was increased in the three-chamber test and repetitive behavior was reduced in the self-grooming test. Our results suggest that anesthetic exposures during neurodevelopment may affect individuals diagnosed with NDDs differently.

    Abstract: This study focuses on revisiting the tectostratigraphic framework of the Ulleung Basin and conceptualizing neotectonics around the western East Sea margin. Based on the analysis of 2D and 3D multi-channel seismic reflection data and offshore drill wells, we divided the entire sedimentary successions of the Ulleung Basin into four tectostratigraphic sequences, named TS1 (c. 23–16 Ma), TS2 (c. 16–9 Ma), TS3 (c. 9–4 Ma), and TS4 (c. 4 Ma–present), in ascending order. The results show that each sequence has been deformed once or multiple times in different periods by juxtaposing two major compressional structures named the Dolgorae Thrust-Fold Belt and the Gorae Anticline. Interpretation of the stratal deformation and termination patterns of the syn- and post-deformational sequences of each structures suggests that the thrusting and folding of the Dolgorae Thrust-Fold Belt was active from c. 16 Ma to c. 9 Ma under the NNW–SSE compressional stress regime (Stage-2), whereas the Gorae Anticline was active from 4 Ma to the present under the ENE–WSW compressional stress regime (Stage-4). Between these two compressional events, there was an intervening period of regional slow subsidence driven by thermal contraction of the back-arc lithosphere and isostatic sedimentary loading (Stage-3). Based on the stratigraphic and structural reconstruction, we propose a 4-stage tectonic model: Stage-1) back-arc opening stage associated with the southward drift of the Japanese islands (c. 23–16 Ma), Stage-2) tectonic-inversion stage in association with the reorganization of the Pacific and Philippine Sea plates and clockwise rotation of SW Japan (c. 16–9 Ma), Stage-3) post-inversion stage with regional thermal and isostatic subsidence (c. 9–4 Ma), and Stage-4) neotectonic stage in which embryonic subduction is nucleating on the East Sea margins under the E–W compressional stress regime (c. 4 Ma–present).

    Abstract: Korean historical literature records a major offshore earthquake with an associated tsunami in the East Sea of Korea in 1681. The event also generated strong ground motion and landslides over the Korean Peninsula. This study examined the occurrence and characteristics of the reported tsunami along the eastern coast of the peninsula using numerical modeling of tsunami propagation from submarine faults identified in a recently compiled marine fault map. Results from some scenarios indicated runup heights in good agreement with descriptions in historical records. We also examined the time required for tsunamis to travel from the causative faults to vulnerable areas along the eastern coast under various scenarios. Our results successfully hindcast the occurrence and effects of local tsunamis in the historical literature and have important implications for assessing tsunami hazard and risk for coastal areas of the Korean Peninsula.

    Abstract: Alpha-1,6-mannosyl-glycoprotein 2-β- N -acetylglucosaminyltransferase [EC 2.4.1.143, N -acetylglucosaminyltransferase II (GnTII)] catalyzes the transfer of N-acetylglucosamine (GlcNAc) residue from the nucleotide sugar donor UDP-GlcNAc to the α1,6-mannose residue of the di-antennary N-glycan acceptor GlcNAc(Xyl)Man 3 (Fuc)GlcNAc 2 in the Golgi apparatus. Although the formation of the GlcNAc 2 (Xyl)Man 3 (Fuc)GlcNAc 2 N-glycan is known to be associated with GnTII activity in Arabidopsis thaliana , its physiological significance is still not fully understood in plants. To address the physiological importance of the GlcNAc 2 (Xyl)Man 3 (Fuc)GlcNAc 2 N-glycan, we examined the phenotypic effects of loss-of-function mutations in GnTII in the presence and absence of stress, and responsiveness to phytohormones. Prolonged stress induced by tunicamycin (TM) or sodium chloride (NaCl) treatment increased GnTII expression in wild-type Arabidopsis (ecotype Col-0) but caused severe developmental damage in GnTII loss-of-function mutants ( gnt2-1 and gnt2-2 ). The absence of the 6-arm GlcNAc residue in the N-glycans in gnt2-1 facilitated the TM-induced unfolded protein response, accelerated dark-induced leaf senescence, and reduced cytokinin signaling, as well as susceptibility to cytokinin-induced root growth inhibition. Furthermore, gnt2-1 and gnt2-2 seedlings exhibited enhanced N-1-naphthylphthalamic acid-induced inhibition of tropic growth and development. Thus, GnTII’s promotion of the 6-arm GlcNAc addition to N-glycans is important for plant growth and development under stress conditions, possibly via affecting glycoprotein folding and/or distribution.

    Abstract: Tourette’s syndrome (TS) is a childhood neurodevelopmental disorder characterized by sudden, repetitive, involuntary, and irregular muscle movement and vocalization. Recently, non-pharmaceutical methods, such as behavioral therapy, psychotherapy, and deep brain stimulation, have been introduced as alternatives to pharmacological treatment for TS. This study aimed to systematically review and synthesize qualitative evidence on the experiences of children with TS and their caregivers. A meta-synthesis of qualitative evidence could help provide a comprehensive understanding of the challenges experienced by children with TS and their caregivers with the aim of providing more effective treatment and services for them. Materials and methods A systematic search was conducted using MEDLINE/PubMed, EMBASE, Cumulative Index to Nursing and Allied Health Literature, PsycARTICLES, and three Korean databases (Korean Medical Database, Research Information Sharing Service, and ScienceON) in July 2021. Studies were included if they collected and analyzed qualitative data from children with tic disorder or TS, or their caregivers. Qualitative research findings on the experiences and perspectives of children with TS and their caregivers were critically appraised and synthesized using the Joanna Briggs Institute methodology. Results Eight eligible studies were included. The findings from these studies (i.e., themes or subthemes of qualitative research) were aggregated into categories (a group of similar findings) and synthesized findings (a group of categorized findings). Ultimately, the 60 findings were aggregated into 15 categories. Finally, four synthesized findings were derived from the 15 categories: (i) continuation of challenging daily life, (ii) denying that TS causes emotional distress, (iii) accepting and understanding TS as part of oneself, and (iv) looking to the future. Conclusion Children with TS and their caregivers experience physical and psychological distress and social deprivation. Avoiding and suppressing TS causes secondary distress such as guilt. However, seeking social support and accepting the disorder reduce the distress caused by symptoms and lays the foundation for later growth. Even in the face of adversity, children with TS and their caregivers find personal value and acquire a more open and optimistic attitude toward life. This review shows that acceptance-based therapy and social support should be provided for the treatment and management of TS.

    Abstract: The effects of benthic dissolved organic carbon (DOC) flux on the dynamics of DOC in the deep continental margins (200 – 2000 m depth) is poorly understood. We investigated heterotrophic prokaryotes (hereafter bacteria) production (BP) and the bio-reactive properties of sediment-derived dissolved organic matter (SDOM) to elucidate microbially mediated cause-effect relationships regarding the rapid consumption of dissolved oxygen (DO) and accumulation of humic-like fluorescent DOM (FDOM H ) in the deep-water column (750 – 2000 m depth range) of the Ulleung Basin (UB) in the East Sea. BP in the deep water (2.2 μmol C m -3 d -1 ) of the UB was among the highest reported for various deep-sea sites. The high DOC concentration (55 μM) likely supported the high BP seen in the deep-water column of the UB. Concentrations of DOC and C1 component of the FDOM H , which is indicative of microbial metabolic by-products, were 13-fold and 20-fold greater, respectively, in pore water than in the overlying bottom water, indicating that the sediment in the continental margins is a significant source of DOM in the overlying water column. Fine-scale water sampling revealed that BP near the sediment (0 – 30 m above the seafloor; 2.78 μmol C m -3 d -1 ) was 1.67 times higher than that measured in the water column above (30 – 100 m above the seafloor; 1.67 μmol C m -3 d -1 ). In addition, BP increased in the bottom water incubation amended with SDOM-containing pore water (PW). The results demonstrated that SDOM contains bio-reactive forms of DOM that stimulate heterotrophic microbial metabolism at the expense of oxygen in the bottom water layer. The accumulation of C1 component in both PW-amended and unamended bottom water incubation (i.e., without an extra DOM supply from sediment) further indicated that refractory DOM is produced autochthonously in the water column via heterotrophic metabolic activity. This explains in part the microbially mediated accumulation of excess FDOM H in the deep-water column of the UB. Overall results suggest that the benthic release of bio-reactive DOM may be of widespread significance in controlling microbial processes in the deep-water layer of marginal seas.

    Abstract: We evaluated the effectiveness of extended dual antiplatelet therapy (DAPT) usage after 2nd-generation drug elution stent implantation in acute myocardial infarction (AMI) survivors with high ischemic risk characteristics who had no major bleeding for 24 months under at least 1 year of DAPT maintenance. Materials and methods The primary ischemic and bleeding endpoints were the risk of mortality and the risk of BARC 3 or 5 (major) bleeding. We investigated the event rates for 2–5 years after the index procedure. Results Of 3382 post-AMI survivors who met the PEGASUS-TIMI 54 (PEGASUS) criteria and without major bleeding until 2 years, 2281 (67.4%) maintained DAPT over 24 months, and 1101 (32.5%) switched DAPT to a single antiplatelet agent. The & gt;24 M DAPT group showed a lower risk of mortality than the 12–24 M DAPT group (7.2 vs. 9.2%; adjusted hazard ratio: 0.648; 95% confidence interval: 0.595–0.976; p & lt; 0.001). The mortality risk was significantly greater as the number of PEGASUS criteria increased ( p & lt; 0.001). DAPT & gt; 24 months was not significantly associated with a decreased risk for major bleeding in the population meeting the PEGASUS criteria (2.0 vs. 1.1%; p = 0.093). The results were consistent after propensity-score matching and inverse probability weighting to adjust for baseline differences. Conclusion Extended DAPT over 24 months was associated with a lower risk of mortality without increasing the risk of major bleeding among 2 years survivors after AMI who met the PEGASUS criteria and had no major bleeding events before 24 months.

    Abstract: Castleman disease (CD), classified as unicentric CD (UCD) or multicentric CD (MCD), is a rare non-neoplastic lymphoproliferative disorder of unknown origin. Owing to its rarity, the clinical characteristics, therapeutic modalities, treatment outcomes, and prognostic factors related to UCD or MCD are not well defined. Method We retrospectively analyzed 88 patients with CD, including those with hyaline-vascular, plasma-cell, mixed type, hypervascular, and plasmablastic subtypes, for presenting symptoms, physical, laboratory, and radiologic findings, and treatment response in the Korean population. Results The median patient age was 44 years (range: 18–84 years) with slight predominance of women (53.4%). UCD and MCD accounted for 38.6% (n=34) and 61.4% (n=54) of cases, respectively. Histopathologically, UCD patients were classified as 88.2% (n=30) hyaline-vascular and 11.8% (n=4) plasma cell types, whereas MCD patients were classified as 27.8% (n=15) hypervascular, 61.1% (n=33) plasma cell, 7.4% (n=4) mixed, and 3.7% (n=2) plasmablastic types. Twelve (13.6%) patients exhibited a poor performance status with an Eastern Cooperative Oncology Group score of 2. The most common presenting symptom was sustained fever, followed by fatigue, anorexia, peripheral edema, and weight loss. Furthermore, splenomegaly, pleural effusion, and ascites were observed to be associated with CD. Surgical resection and siltuximab were the preferred treatment modalities for UCD and MCD, respectively, with favorable symptomatic, laboratory, and radiologic outcomes and safety profiles. The overall survival was 90.2%, with no significant difference between the UCD and MCD groups (p=0.073), but progression-free survival was significantly poorer in the MCD group (p=0.001). Age ≥60 years and splenomegaly significantly affected the overall and progression-free survival rates. Conclusion Patients with UCD had favorable outcomes with surgical resection of a solitary mass, whereas in patients with MCD, old age and splenomegaly were identified as independent prognostic factors. Further well-designed prospective studies under advancing knowledge of the pathophysiology of MCD are warranted to establish suitable guidelines for the discontinuation or prolonging infusion intervals of siltuximab and treatment modalities for HHV-8 positive MCD patients or patients with siltuximab failure.
