    Abstract: BACKGROUND: There is a formal contradiction in the Republic of Mari El between the favorable ecological situation in localities of the Republic and the above-limit morbidity rates caused by the negative impact of habitat factors. As such, there is a need to conduct a specified quantitative examination using modern econometric approaches. AIM: By using panel data, to identify and substantiate statistical relations between the morbidity rate of the population in municipal entities of the Mari El Republic and ecological parameters of the environment (physical and chemical and bioindication indicators). MATERIAL AND METHODS: This study used the panel data on morbidity rates of the population, including children, adolescents, and adults, and environmental pollution across 15 municipal entities within the Mari El Republic from 20092019, as well as spatial data on fluctuating asymmetry for 2019 that were based on a field study and integrally reflect the environmental quality. Statistical analysis for dynamic panel data involved cointegration and modeling with a mechanism of balance adjustment, whereas that for spatial data involved nonparametric correlation analysis. All statistical analyses were conducted using the statistics packages EViews 11 and Stata 16 IC. RESULTS: It was established that over the long term the increasing pollution level of drinking water from the distribution network and the atmosphere has a statistically significant effect (2=79.79; p 0.001) on the increase in figures of congenital abnormalities (malformation), deformations and chromosome breakages in children aged 0 to 14 years. Adjustment of the indicator of congenital abnormalities (malformation), deformations and chromosome breakages in children aged 0 to 14 years with a deviation from the equilibrium trajectory occurs in about 2 years. It is required to develop measures to systematically provide the population with good-quality drinking water and reduce emissions of pollutants into the atmosphere. Stable positive correlations of long-term average annual morbidity rate of the whole population and the adult population aged 18 and older being ill with malignant tumors are associated with the change in environmental quality (indicators of the fluctuating asymmetry in the impact zone). CONCLUSION: A panel regression model was designed that explains the change in time of the indicator of congenital abnormalities (malformation), deformations and chromosome breakages in children aged 0 to 14 years within one municipal entity with the increasing pollution level of the atmosphere and drinking water. Being ill with malignant tumors is related to a complex of various stress factors (varying between the human impact and climate change).

    Abstract: Hypothesis/aims of study. To date, one of the most important avenues of research in the field of reproductive medicine is the searching for new biochemical markers of oocyte quality and the prediction of the effectiveness of in vitro fertilization (IVF) protocols. The aim of this study was to assess the effect of relaxin levels in blood serum and follicular fluid on the efficiency of ovulation stimulation, fertilization, and characteristics of the embryos. Study design, materials and methods. This prospective randomized cohort study included 11 patients undergoing infertility treatment in a superovulation stimulation protocol using gonadotropin-releasing hormone antagonists. Age, body mass index, hormonal status, ovarian response, endometrial thickness and structure, the number and quality of oocytes and embryos, as well as fertilization efficiency were assessed. The level of relaxin in blood serum and follicular fluid samples was determined on the day of transvaginal follicle puncture using enzyme immunoassay. Results. A correlation between follicular fluid relaxin levels and body mass index, age, the number of oocytes, and their fertilization efficiency (p 0.05) was established. Changes in follicular fluid relaxin level were revealed depending on the gonadotropin preparations (p 0.05) and triggers of final maturation of oocytes (p 0.05). The tendency of the effect of gonadotropin doses on circulating relaxin levels, and of the hormone itself on endometrial thickness and the quality of oocytes was determined. Conclusion. Determination of the relaxin concentration can be considered as a promising method for predicting the result of ovarian stimulation and the efficiency of fertilization in IVF protocols.

    Abstract: BACKGROUND: At present, the knowledge on initial human embryogenesis stages is limited to the period of development from zygote to blastocyst. The creation of models based on the interaction between the embryo and endometrium in vitro, which accurately imitate the in vivo processes, represents the major way for implantation and post-implantation evaluation. Currently, no reports have been made on models reflecting the both processes simultaneously: the interaction of the embryo with the substrate, which represents many aspects of normal implantation, and the early post-implantation embryo development. The creation of a relevant model would allow investigation of implantation and early post-implantation processes as a whole. AIM: The aim of this study was to evaluate the vitality and developmental potential of human embryos from the day 6 blastocyst stage during their extended co-incubation with the endometrium in a culture medium specifically designed for cultivation to the blastocyst stage. MATERIALS AND METHODS: Embryos obtained in assisted reproductive technology programs were cultivated from the day 6 blastocyst stage up to 14 days of development in vitro in a culture medium designed for cultivation to the blastocyst stage, in the presence of endometrium cells. On day 14 of development, embryos and endometrial samples were first evaluated under an inverted microscope using Hoffman modulation contrast, then transferred to a special mold and impregnated with paraffin for cytoblock preparation. Obtained blocks were sliced, stained with hematoxylin and eosin and morphologically assessed. RESULTS: The first sample visual assessment on day 14 of cultivation in a culture medium with endometrium cells revealed a viable developing embryo with no signs of degradation. During the histological examination, the endometrial sample corresponded to the secretory phase of the cycle. The morphological assessment of the conceptus detected trophoblast cells. The second sample visual assessment on the day 14 of cultivation in a culture medium with endometrium cells revealed a viable embryo with no signs of degradation, which was in direct contact with the endometrial component. A histological examination detected a secretory endometrial fragment of the surface (luminal) epithelium. During the morphological assessment of the embryo, trophoblast cells were detected. CONCLUSIONS: The data obtained indicate the ability of the embryo to further develop from the day 6 blastocyst stage up to 14 days in a culture medium specifically designed for cultivation to the blastocyst stage, in the presence of endometrium cells. The latter can serve as an experimental model for both in vitro endometrial receptivity evaluation and intercellular interactions during implantation investigation.
