    Abstract: The study is concerned with the outcomes of the treatment of 364 patients with combined trauma (craniocerebral injuries of different severity and fractures of skeleton bones [453]). Patients management and fracture fixation methods are presented. Primary osteosynthesis (during the first day after trauma) was done in 45.9% of victims and delayed osteosynthesis - in 54.1%. The following fixators were applied: plates (46.6% of victims), external fixation devices (36.6%), rods (9.9%), other types (6.9%). Early osteosynthesis (during the first three days after trauma) did not increase the lethality and complication rate. At follow up period of 12-18 months the long term results were studied in 98 of 364 patients. Good outcome was achieved in 79.6% of cases, satisfactory - in 17.3% and unsatisfactory in 3.1% of cases. The authors consider that primary and early osteosynthesis in patients with the combination of craniocerebral and skeleton injuries enable to improve the outcomes, reduce the lethality rate, shorten the duration of hospitalisation and promote early rehabilitation.

    Abstract: This work presents the results of in a noninvasive topographic and anatomical study of the gallbladder and the anterior abdominal wall parameters in the right hypochondrium region (based on the results of spiral computed tomography and 3D modeling) in both sexes with different body types. It is established that there are some features of the gallbladder’s bottom and neck holotopia, depending on sex and body type of the patient. Extreme variants of the gallbladder’s position are revealed. The bottom of the gallbladder is located: a) the most lateral position in hyperstenic men; b) maximum medial in astenic women; c) maximum cranial in astenic men; d) the most caudal in astenic women. The gallbladder’s neck is located: a) the maximum lateral position in normostenic men; b) maximum medial in astenic women; c) maximum cranial in astenic men; c) the maximum caudal in astenic women. A mathematical algorithm is proposed and a software package is developed to determine the gollbladder’s bottom and neck probabilistic location in the three-dimensional coordinate system in patients depending on gender and body type. The developed technique allows to choose the type of operative access, to plan its individual localisation, and also to calculate criteria of operative access depending on patient’s anthropometric data during preparation for intervention. This allows to decide on the rationality of the use of a surgical access, choose its optimal location, the size of the skin incision, thereby reducing surgical trauma during the intervention and leads to improved quality of treatment of patients with gallstone disease.

    Abstract: Aim. To investigate the relation of different genotypes and alleles of PAI-1 gene with the course of coronary artery disease. Methods. We observed 80 patients with coronary artery disease (51 males and 29 females) aged 39 to 65 years living in the city of Baku. Genotyping of a polymorphic marker 4G(-675)5G was performed by means of polymerase chain reaction followed by restriction fragment length analysis. Genomic deoxyribonucleic acid (DNA) was extracted from venous blood leukocytes. To determine the frequency distribution of genotypes and alleles of PAI-1 in ischemic heart disease all patients were divided into 6 groups depending on the course of the disease and its outcomes. Results. In the population of Baku genotype 44 of PAI-1 gene is significantly more common in people with coronary artery disease compared to individuals without coronary artery disease and is a risk factor for this disease. Genotype 55 of PAI-1 gene is more common in people without coronary artery disease and thus is a protective genotype against this disease. Allele 4 of PAI-1 was statistically more frequently recorded in patients with ischemic heart disease, and allele 5 was significantly more frequently observed in patients without this pathology. Therefore, allele 4 is a risk factor for the occurrence of coronary heart disease and allele 5 is protective against this disease. Alleles 5 and 4 are not associated with the course and outcomes of ischemic heart disease. Conclusion. According to the results of the study on the population of Baku, genotype 44 of PAI-1 gene is a risk factor of ischemic heart disease, genotype 55 of PAI-1 gene is protective against this pathology, allele 4 is characteristic for people with coronary artery disease (a risk factor), allele 5 is characteristic for people without it (protective role) whereas the named genotypes of PAI-1 are not connected to the course and outcomes of this disease.

    Abstract: Aim. Improvement of complex treatment results in patients with diabetic foot syndrome by introducing methods of gravitational surgery and α-lipoic acid. Methods. The results of treatment were analyzed for 558 patients with diabetic foot syndrome treated in Scientific Centre of Surgery named after M.A. Topchubashov (Baku, Azerbaijan) from 1988 to 2015. The age varied from 28 to 83 years. The patients included 416 men and 142 women. The control group included 90 patients who at the perioperative period underwent basic therapy including antibiotics, anticoagulants, antiaggregants, dextrans, angioprotectors, spasmolytics, corticosteroids, narcotic and non-narcotic analgesics. The study group included 468 patients, along with traditional therapy receiving efferent methods (plasmapheresis, ultraviolet blood irradiation, ozone therapy) and α-lipoic acid. 282 patients of the study group received outpatient treatment and 186 - complex inpatient surgical treatment. A comparative evaluation of the results was performed separately in three groups: angiopathy, neuropathy, angioneuropathy. The results were evaluated by clinical and instrumental examinations before and after treatment (6, 12, 60 months and more). Results. In the study group a satisfactory result of treatment was registered in 85.5% of patients, in the control group - in 62.2%, unsatisfactory in 14.5 and 37.8% of patients, respectively (p=0.046). Conclusion. The use of efferent methods and α-lipoic acid provided prompt elimination of numerous pathogenetic disorders observed in diabetes mellitus, decrease of amputation frequency and improvement of complex surgical treatment results in patients with diabetic foot syndrome.
