    Abstract: This article presents the preparation and implementation of a systematic intervention with sixteen families, each of which had a member hospitalized in a long-term care setting. A guide based on the systemic approach was developed. As well as the steps to be followed, the guide offers the operational reference points which are required for intervention at the process level. The content analysis of the various nurse-family-hospitalized member encounters revealed that confusion, revolt and aggressivity of the hospitalized member were behaviors with which the family had difficulty coping. Particularly in a situation where an elderly woman had to face the disorientation and the aphasia of her hospitalized spouse, infantilisation and overprotection were observed. Moreover, such women experienced a high level of loneliness, had great difficulty in reorganizing their activities of daily living and reported important health problems. The main features of the intervention are discussed; among them are the necessity to help individuals to express their perceptions and their feelings in relation to the situation experienced and the importance of acting as a role model in order to help the family and the ill member toward a more functional form of communication.

    Abstract: Many people suffering from dementia live at home, in spite of the demands that such a situation puts on those who care for them. The role of helper is usually assumed by a particular family member who is therefore more likely to feel the impact of the caregiving responsibility. Researchers are concerned with the relationship between the well-being of these helpers and their informal social support. This article presents a critical review of 21 studies of that relationship. The various dimensions of social support considered in the studies are analyzed, as are the methodological limits. A discussion of findings about the relationships between social support and wellbeing completes the analysis. It appears that researchers have a limited conception of social support and assess only a few of its many dimensions in a variety of ways. Very few authors explicitly specify underlying assumptions or a theory relevant to their conceptualization of social support. The weakness and the diversity of this conceptualization are reflected in the measurement tools which consist of a few ad hoc questions on social support in nearly 50% of the studies. Few studies consider the support availability, duration, conflict, and reciprocity. The buffering effect model of support is examined in one study. Inadequate sample size, unrepresentative samples, and lack of control in regard to confounding variables are among the methodological problems identified. When researchers use an evaluative approach to measure social support, the relationships between support and well-being are more often significant than when a descriptive approach is used. In future studies, researchers should be careful to better articulate the theoretical notions proposed in studies on caring for a family member with dementia at home with those coming from studies on social support in general. It is important for researchers to strengthen their study design and to carry out longitudinal studies; they must also try to understand how the influence of society works. From this perspective, one avenue for the future might involve studying the sequence of relationships between stress, support, coping strategies, and well-being. This is all the more important because at the present time certain studies on the caring of persons with dementia are based on a conceptual framework of stress. Such a framework considers support as a mediating factor.

    Abstract: This study examines various aspects of the mental health of aged caregiving spouses. Two factors are considered: the type of health problem of the care-receiving spouse (cognitive or physical impairments), and sex. Two samples of spouses providing care are examined: (a) those providing care to spouses suffering from cognitive impairments and (b) those providing care to spouses suffering from chronic obstructive pulmonary disease (COPD). These participants are compared to a group of non-caregiver aged spouses as well as other aged American normative samples. All participants completed the Brief Symptom Inventory developed by Derogatis and Spencer (1982). Results show that those providing care to their cognitively impaired spouses tend to be more mentally fragile than the caregivers of spouses suffering from COPD and the non-caregivers. The only difference found between men and women caregivers or men and women non-caregivers is related to interpersonal sensitivity. When compared to aged American normative samples, non-caregiver spouses showed less symptoms of obsessive-compulsiveness, anxiety, and hostility. Our study points to the importance of considering the type of health problem of the care receiver. It is also important to assess various aspects of mental health and to be able to use a measure of mental health which is specific to the aged population of Québec.

    Abstract: A support program was offered to elderly individuals and their family during the admission period in a nursing home. Based on crises intervention and theoretical notions of family functioning, the program consists of two sessions before admission, one, the day of admission and two others, following the admission. The first and third sessions were held with the aged person and the family, the second and fourth with the family alone and the fifth was a session with a group of admitted elderly individuals. Nineteen aged persons and a family member of each of the nineteen families were part of the experimental and control groups.
