    Abstract: Objective: To describe the diffusion of cardiac catheterization technologies and time trends of their use according to setting and geographic region in Germany during a 13-year period. It is hypothesized that the cardiac catheterization technology has matured from an experimental state to a broadly accepted technology. Methods: Data come from the annual survey of the German Society for Cardiovascular Research. All German cardiac catheterization units are requested to provide data on volume and type of catheterization procedures. Data are available from 1984 to 1996. Number and type of procedure, type of unit, diagnoses, and complications are all recorded. The overall response rate is 90%, on average. Results: The total number of catheterization units was 324 in 1996, or an average of 3.69 units per 1 million population. In 1996, all of the East German Länder and districts were below average. Utilization of cardiac catheterization procedures increased exponentially during the study period. The number of angiographies rose from about 45,000 in 1984 to more than 450,000 in 1996; the number of angioplasties increased almost by a factor of 50 to 125,000 procedures in 1996. Inverse correlations between the rates per million population of either coronary angiographies or PTCAs and mortality rates from ischemic heart disease were observed at the level of the German Länder. Conclusion: Further studies taking patient characteristics, long-term outcomes, and other factors in account are necessary to clarify the large geographic variations and the negative relationship between utilization rates and coronary heart disease mortality found in this study.

    Abstract: The onset of convection in a rotating cylindrical annulus with parallel ends filled with a compressible fluid is studied in the anelastic approximation. Thermal Rossby waves propagating in the azimuthal direction are found as solutions. The analogy to the case of Boussinesq convection in the presence of conical end surfaces of the annular region is emphasised. As in the latter case, the results can be applied as an approximation for the description of the onset of anelastic convection in rotating spherical fluid shells. Reasonable agreement with three-dimensional numerical results published by Jones, Kuzanyan & Mitchell ( J. Fluid Mech. , vol. 634, 2009, pp. 291–319) for the latter problem is found. As in those results, the location of the onset of convection shifts outwards from the tangent cylinder with increasing number $\def \xmlpi #1{}\def \mathsfbi #1{\boldsymbol {\mathsf {#1}}}\let \le =\leqslant \let \leq =\leqslant \let \ge =\geqslant \let \geq =\geqslant \def \Pr {\mathit {Pr}}\def \Fr {\mathit {Fr}}\def \Rey {\mathit {Re}}N_{\rho }$ of density scale heights until it reaches the equatorial boundary. A new result is that at a much higher number $N_{\rho }$ the onset location returns to the interior of the fluid shell.

    Abstract: Objective: Even though gastrointestinal complaints are among the most frequent reasons to contact general medical practioners, little is known about the actual care of these patients, especially the use of diagnostic imaging technologies. Methods: In a network of 57 family practitioners and 29 general internists in Lower Saxony, Germany, 1,217 contacts with patients with gastrointestinal diseases (16%, gastritis; 12%, gastroenteritis; 6%, cholelithiasis; 5% each, ulcus ventriculi and duodeni, and 14%, no final diagnosis) were documented. The effects of patient and physician/practice side factors on the use of ultrasonography and radiography were modeled using multivariate logistic regression. Results: For ultrasonography, diagnosis and ownership of an ultrasound unit (odds ratio [OR] = 3.33) were highly significant predictors ( p 〈 .0001), followed by unknown diagnosis at beginning of contact (OR = 1.92; p = .0019), physician specialty (OR for internists = 1.89; p = .0025), and severity ( p = .0085). For radiography, ownership of an ultrasound unit was the most significant factor (OR = 0.34; p 〈 .0001), followed by severity ( p 〈 .0009), ownership of x-ray apparatus (OR = 2.56; p = .0025), physician specialty (OR for internists = 1.98; p = .0358), and unknown diagnosis at beginning of contact (OR = 1.79; p = .0451). Not significant were age and sex of patient and diagnosis for radiography. Conclusions: Physicians use diagnostic imaging technologies for patients with gastrointestinal complaints according to severity and knowledge about the diagnosis, but ownership of technology is the most predictive factor.

    Abstract: Stellar radiative zones are typically assumed to be motionless in standard models of stellar structure but there is sound theoretical and observational evidence that this cannot be the case. We investigate by direct numerical simulations a three-dimensional and time-dependent model of stellar radiation zones consisting of an electrically conductive and stably stratified anelastic fluid confined to a rotating spherical shell and driven by a baroclinic torque. As the baroclinic driving is gradually increased a sequence of transitions from an axisymmetric and equatorially symmetric time-independent flow to flows with a strong poloidal component and lesser symmetry are found. It is shown that all flow regimes characterised by significant non-axisymmetric components are capable of generating a self-sustained magnetic field. As the value of the Prandtl number is decreased and the value of the Ekman number is decreased, flows become strongly time dependent with progressively complex spatial structure and dynamos can be generated at lower values of the magnetic Prandtl number.
