    Abstract: In order to estimate the potential benefits of interventions against multiple geohelminth species in endemic areas, an improved understanding of the population biology of multiple infections is required. This paper presents a detailed analysis of the associations among Ascaris lumbricoides , Trichuris trichiura and hookworm infections in 1539 schoolchildren on Pemba Island, Tanzania, where 58% of the sampled children carried infections of all 3 parasites at the time of the study. Infection intensities of different species were positively correlated, and individuals with single-species infections had generally lower species-specific egg counts than individuals with multiple-species infections. There was no age- or sex-related clustering of infections. A weak clustering of intense infections among individuals with multiple-species infections wa s observed, which became more pronounced as the threshold defining an intense infection increased for each species. The results suggest that individuals with multiple species infections are likely to be at highest risk of geohelminth-related morbidity, not only because of the number of infections they harbour, but also because they generally carry heavier infections of each species.

    Abstract: Strongyloides stercoralis is rarely recognized as a major public health issue, probably because its burden is largely underestimated. We reviewed the literature (both PubMed and ‘grey’ literature) about the prevalence of strongyloidiasis in Latin America, an area of presumable high endemicity. There were finally 88 papers involved in the analysis, covering the period between 1981 and 2011. Studies were heterogeneous in several aspects, such as the populations screened and the diagnostic methods used. Most of the studies relied on direct coproparasitological examination, which has low sensitivity for the detection of S. stercoralis larvae. The following countries presented areas of high prevalence ( 〉 20%): Argentina, Ecuador, Venezuela, Peru and Brazil. Globally, for most of the included countries it was not possible to define reliable data because of paucity and/or inadequacy of studies. S. stercoralis requires specific diagnostic methods for its detection; therefore, surveys should be specifically designed in order to avoid underestimation of the infection.

    Abstract: We estimated the number of people unaware of their human immunodeficiency virus (HIV) infection in our province, Pavia (population 540 000) in Lombardy, Italy, by means of anonymous unlinked testing of 10 044 serum/plasma samples residual from clinical analyses at the outpatient clinic of Policlinico San Matteo in 2014 and 2015. Ethical and legal approval was obtained prior to study start. Samples were irreversibly anonymised, only retaining gender and 5-year age class. Five sample pools were tested for HIV using LIAISON ® XL MUREX HIV Ab/Ag (DiaSorin, Saluggia, Italy). If the pool tested positive, individual samples underwent confirmatory tests, Innotest HIV Antigen mAb (Fujirebio Europe, Gent, Belgium) and HIV BLOT 2·2 (MP Diagnostics, Singapore). Among the 10 044 samples processed, eight were confirmed positive (0·08%, 95% confidence interval 0·03–0·16%), all were males and age was 〉 50 in 3 (37·5%). If projected to the entire population of the Pavia province, this would result in approximately 1000 people unaware of their HIV infection, with age older than expected. In Italy, HIV testing is voluntary, universally free-of-charge and (upon request) anonymous. Nevertheless, this study demonstrates that it is suboptimally employed, and that new strategies and population-level actions will be needed to achieve better implementation of HIV testing and HIV control in our province.
