    Notes: Abstract: Although multiple tests are often performed prior to surgery for the primary treatment of breast cancer, the role of perioperative testing has yet to be clearly defined. The purpose of this study was to examine the use of perioperative testing in the Department of Defense (DOD) health care system.Records from all patients who received an operation for breast cancer between April 1, 1990 and March 1, 1992 in the DOD health care system were reviewed. The results of bone scans (BS), abdominal CT scans, brain scans (CT, MRI), chest radiographs (CXR), blood tests, and specified parameters from the history and physical examination were analyzed. Chi-square statistics were performed.2,364 women underwent surgery for breast cancer during this period. The percentage of patients that received each test (A) and the percentage of the tests that were suggestive of metastatic disease (B) are as follows ([A%], [B%]): CXR (94%, 1.3%); BS (23%, 8%); abdominal CT (4%, 9%); and brain scans (1%, 11%). There were more abnormal BS in patients with an elevated alkaline phosphates than in those with a test that was normal or never performed (20% vs. 4.8%, p 〈 0.001). Elevated liver function tests or central nervous system signs or symptoms were not associated with abnormal studies.In the past, patients with breast cancer underwent an extensive array of blood tests, CXR, and scans of bones, liver, and brain in the attempt to detect occult metastatic disease prior to surgery. In a large and diverse system such as the DOD, however, a perioperative metastatic work-up, which includes a BS, brain scan, and abdominal CT scan is not routinely performed, suggesting that clinicians selectively perform specific tests during this period. Despite this tendency, when tests are performed on breast cancer patients in the perioperative period, they are infrequently suggestive of metastatic disease.

    Notes: Purpose The in vivo wear resistance of 12 composite resins were compared with an amalgam control using the Latin Square experimental design. Sixteen edentulous patients wearing specially designed complete dentures formed the experimental population.Materials and Methods The Michigan Computer Graphics Measurement System was used to digitize the surface of the control and composite resin samples before and after 3-month test periods to obtain wear data. The 12 composite resins selected for this investigation based on their published composite classification types were seven fine particle composites, three blends, and two microfilled composite resins. The Latin Square experimental design was found to be valid with the factor of material being statistically different at the 5% level of significance. Wear was computed as volume loss (mm3/mm2), and all of the composites studied had more wear than the amalgam control (P= .001).Results After 3 months, the mean (error) of wear of the amalgam was 0.028 (0.006). Means (error) of wear for the 12 composites were ranked from most to least wear by mean wear volume loss.Conclusions The absence of any relationship between mean wear volume loss and the volume percentage filler was confirmed by the correlation coefficient r= -0.158.

These guidelines may help practitioners determine appropriate treatments for their patients. Four categories are defined, ranging from Class I to Class IV, with Class I representing an uncomplicated clinical situation and a Class IV patient representing the most complex and higher-risk situation. Each class is differentiated by specific diagnostic criteria. This system is designed for use by dental professionals who are involved in the diagnosis of patients requiring treatment for complete edentulism. Potential benefits of the system include: 1) better patient care, 2) improved professional communication, 3) more appropriate insurance reimbursement, 4) a better screening tool to assist dental school admission clinics, and 5) standardized criteria for outcomes assessment.

    Notes: A technique is described for fabricating autopolymerizing acrylic resin copings for CeraOne provisional restorations. This method has the advantage of providing a chemical bond rather than only a bond between the mechanical coping and the relined provisional crown, as is the case when using the provisional components supplied by the manufacturer. The technique describes provisionalization at the time of stage II surgery. This type of coping can also be used after a period of soft tissue healing when a healing abutment is employed. The benefits of the procedure include increased patient satisfaction and reduced chair time for repairs.

This classification system is similar to the classification system for complete edentulism previously developed by the ACP. These guidelines are intended to help practitioners determine appropriate treatments for their patients. Four categories of partial edentulism are defined, Class I to Class IV, with Class I representing an uncomplicated clinical situation and class IV representing a complex clinical situation. Each class is differentiated by specific diagnostic criteria. This system is designed for use by dental professionals involved in the diagnosis and treatment of partially edentulous patients. Potential benefits of the system include (1) improved intraoperator consistency, (2) improved professional communication, (3) insurance reimbursement commensurate with complexity of care, (4) improved screening tool for dental school admission clinics, (5) standardized criteria for outcomes assessment and research, (6) enhanced diagnostic consistency, and (7) simplified aid in the decision to refer a patient.

    Notes: Abstract The purpose of this study was to examine differences in both perceived social support and loneliness according to the three stages of adolescent development (early, middle, and late), as determined by chronological age and gender. The sample consisted of 113 early adolescents, 106 middle adolescents, and 106 late adolescents. Data were collected in classroom settings. All participants responded to the PRQ85—Part II, A Social Support Measure; the Revised UCLA Loneliness Scale; and a demographic data sheet. Results of the study, which used the two-way analysis of variance, indicated that there were no statistically significant differences in perceived social support or loneliness across the three stages of adolescence. Findings also indicated that girls reported statistically significantly higher levels of perceived social support than boys; however, there were no statistically significant gender differences in loneliness.

    Notes: We evaluated 50-year-old bottomland forests in southwestern Kentucky restored from agriculture by planting and natural regeneration in terms of their development toward mature forests. We described and compared the structure and composition of the plant communities of three stands of each type (planted, naturally regenerated, and mature). Increment cores were analyzed to reconstruct developmental trends. Future trends were predicted from analyses of the midstory and understory composition. Both planting and natural regeneration adequately replaced the structural attributes of the historical bottomland forest. The existing structural differences are expected to diminish over time. Neither regeneration method replaced the wildlife value of the mature bottomland forests due to insufficient establishment and subsequent ingrowth of heavy mast species (particularly oaks and hickories). There was evidence that the understory species compositions of the restored forest types were similar to that of the mature stand type. All forests, including the mature stands, appeared to be succeeding from hydric to mesic species compositions as a result of human-altered hydrology and natural floodplain processes. We speculate that the historical bottomland species composition will probably not persist on any of the study sites in the long term.

    Notes: Purpose In the American National Standards Institute/American Dental Association specification no. 19, compatibility of impression materials with dental stones is assessed by the presence of a 20μm-wide line reproduced on an unmodified calcium sulfate dihydrate cast. In actual dental practice, modified type IV dental stones are used, although little is known of their compatibility with polysulfide impression materials. This study evaluated the compatibility of 6 polysulfide impression materials and 11 modified type IV dental stones.Materials and Methods A line 20 μm wide was etched on four glass dies. Four samples of each combination of impression material and dental stone were prepared according to the manufacturer's directions with an additional 3 minutes for the final setting time. Compatibility was determined by the presence of the reproduced line on the dental stones, as observed under low angle 10 × magnification by four rater groups.Results The line was reproduced on all of the impression specimens, and the examiners recorded 66 positive identifications of the line on the stone casts out of a possible 1,056 ratings for a total of 6.25% of the specimens. Out of a possible 66 impression-stone combinations, only 18 reproduced the 20-μm line. The combinations reproducing the lines most frequently (75%) were Neoplex with Blue Die Stone (Columbus Dental, St Louis, MO) and Coeflex with Indic Die Stone (Coe Lab Inc, Chicago, IL).Conclusions The study showed that many combinations of polysulfide impression materials and modified type IV dental stones did not reproduce the 20-μm line; therefore, not every polysulfide is compatible with every type IV dental stone.

    Notes: Dental periodicals are the fundamental source of prosthodontic research. The ability to understand and contribute to dental literature is basic to the prosthodontic profession. The purpose of this study is to tally relative frequency with which various descriptive (n = 18), graphical (n = 7), and inferential statistical procedures (n = 68) are used in the prosthodontic literature. Our method consists of four procedures: journal selection, choice of 1987 through 1988 articles with inferential statistical content, tally of the statistical procedures in those articles, and quality control procedures used in obtaining these data. At least 50% of 10 prosthodontists selected 17 of 100 journals most likely to be read by prosthodontists. In the 17 journals, 1,320 articles were screened of which 406 were selected and evaluated for their statistical procedures. The bar and line plots were the most common graphical procedures occurring in over one fourth of the 406 articles. Percentages, means, and standard deviations occurred in more than 40%. Although 58% used the .05 significance level, only 0.3% mentioned power. Analysis of variance was used more often than the t tests (42%v 29%), whereas correlation/regression (21%) and chi-square tests (14%) were used less often. The t tests, analysis of variance (Duncan, Tukey, and Student-Newman-Keuls multiple comparison procedures), chi-square tests, correlation and regression, and the Wilcoxon tests occurred in at least 5% of the 406 articles.
