    Abstract: Gastrointestinal tract disorders are a public health problem in our country. The objective of this study was to find out hospital based information and risk factors of prevalence of the patients suffering from gastrointestinal disorders. We included all suffering from upper abdominal pain and visiting KYAMCH, A total number of 1196 patients were endoscopically examined. Among them 25.54% were found to be suffering from upper gastrointestinal tract disorders, 9.86% had erosion, 8.01% had ulcer, 3.62% had inflammation and 4.04% was found to be malignant. Duodenum Ulcer Disease among male 4% and in females 3%. The prevalence of gastrointestinal disorders in a tertiary level hospital in rural Bangladesh is increasing.KYAMC Journal Vol. 4, No.-1, July 2013, Page 316-320

    Abstract: The present study was designed to have a comprehensive epidemiological data of the circulating diseases in Pubna cattle in Pubna and sirajgonj districts of Bangladesh. Two sites were selected for the study as the Nucleus Breeding Hard (NBH); which is located in the BLRI-RS, Baghabari, Sahjadpur, Siragjang and the community; consists of three villages of Bera upazila of Pabna district namely Khar-bagan, Hatail-aralia and Char-sabolla. The total cattle population was 576 (506 in the community and 70 in the NBH). The overall weather was hot and humid. A total of 965 clinical cases of cattle of different age and sex were diagnosed during the period from July, 2017 to June, 2018.The descriptive statistics suggested that endo-parasites (15.67%), FMD (27.51%), non-specific diarrhea (7.73%) and post-partum anoestrus (7.78%) are the most prevailing diseases in the community and endo-parasites (21.82%), foot-rot (15.15%), non-specific fever (7.27%), dermatochalasis (5.45%) and conjunctivitis (4.85%) are the most prevailing diseases in the NBH. Population data shows that there was an endemic outbreak of FMD in the community between March to May 2018 where almost 50% cattle were affected by the disease though calf morbidity was low (2.69%) but all the affected calf died. Non-specific diarrhea was more common in older individuals (14% in over 3years old cattle) mostly in cows (14.2%) and occurred in summer (9.84%) and winter (8.81%) may be because of the ignorance of the farmers to supply clean water to their animals. Although, we routinely de-worm our cows in the community and in the NBH round the year we found many cases of parasitic gastro-enteritis in both the locations which suggested that our de-worming practice is not working. Repeated study of the same kind can provide us a large set of data on cattle disease prevailing in the area which may provides valuable insight to design and implement priority based research on specific disease and to take efficient control strategies against that disease.     Abstract: Generation of secondary current in a meandering river flow due to the centrifugal force acting on the bend is three dimensional (3D) in nature. But in case of practical problems like alluvial geomorphic processes, 3D models are not proved to be efficient. Hence, two dimensional (2D) models are generally adopted for such problems. This study offers a presentation of numerical simulation results for turbulent flows around bends of a meandering channel for different meander angles. 2D models were built by the use of iRIC Nays2DH solver for flow simulation of 45˚, 90˚, 135˚, 180˚ meandering bends with varying widths of 0.15m, 0.25m, 0.75m, 1.00m, 1.25m and 1.50m considering constant Froude number with constant meandering length (ML/W) and constant radius of curvature (R/W). Zero equation model was used as turbulence closure with finite differential advections as upwind scheme. The flow behavior has been studied at the apex and cross over portions of bends. From the simulation, the velocity at outer bank was found to be lower than that of the inner bank while the water surface elevation was found to be higher than that of the inner bank. Increase in transverse slope of water surface at bend was observed for increasing meander angles, Froude numbers and decreasing channel widths. Journal of Engineering Science 12(2), 2021, 93-101

    Abstract: Background: Peripheral arterial disease (PAD) is a marker of increased risk for cardiovascular events and of poorer prognosis in patients with coronary artery disease (CAD). The prevalence of unknown PAD amongst patients with ACS varies between studies according to the mode of diagnosis.Aims: To evaluate the prevalence of peripheral arterial disease (PAD) in diabetic ACS patients with or without tobacco user by using ankle brachial index (ABI).We also assess the probable predictors of PAD among these patients.Methodology: This prospective observational study was conducted in the Department of Cardiology, Ibrahim Cardiac Hospital and Research Institute, Dhaka, Bangladesh starting from 1st January 2016 to 30th April 2016 over a period of four months. A total of 60 patients were studied. They were grouped on the basis of their smoking habit. Diabetic patients with ACS and tobacco user (smoke and smoke less) in group I and without tobacco user in group II.Results: The mean age of the studied patients was 56.63 ± 8.95 years, range from 25-90 years. 73.30% was male and 26.70% was female. Twenty three patients of tobacco user in group-I (n=30) had peripheral artery disease and ten patients of group-II (n=30) had peripheral artery disease. It was statistically significant (p=0.003).Conclusion: There is correlation between peripheral arterial disease and coronary artery disease. Diagnosis and supervision of patients with PAD is important for preventing the local progression of the disease and effective secondary prevention of future coronary and cerebrovascular events.Bangladesh Heart Journal 2017; 32(1) : 3-9

    Abstract: The moth, Eublemma amabilis Moore is very destructive to lac insect and lac encrustation. The moth is generally white-pinkish in colour and it lays gray-white and round eggs, depressed in the centre. The newly hatched larva enters the lac insect either through one of the opening in the cell or by tunneling a hole through the encrustation. A single larva damages 42-50 matured cells prior to pupation. It completes six generation in a year and causes comparatively more injury to the "Kartki" crop than to the Baishaki crop. The morphological details of this predator were studied. Several natural enemies of E. amabilis have been recorded.     Abstract: The phytosociological analysis, species diversity and types, fidelity, presence, constancy, Sørensen similarity index and changes in forest cover, land and water areas since 1972 were analyzed. Forty six species were recorded both in quadrats and growing outside which belong to 26 families and 41 genera, 19 were distinguished as true mangrove species and the rest were mangrove associates. Leguminosae and Rhizophoraceae were the dominant families represented by 5 spp. each. Three species were found to be invasive, namely Blumea lacera (Burm. F.) DC., Catharanthus roseus (L.) G. Don., Wedelia chinensis (Osbeck) Merr. The maximum importance value index was found in Heritiera fomes Buch.-Ham (48.08). The overall species diversity (H) of SMF was found 3.81, species richness (d) was 9.10 and evenness (e) was 0.47. The forest area was 385,237 ha in 2015. The satellite images showed that the forest and water area of SMF decreased gradually till 2015 in comparison to 1972. The bare land of Sundarbans showed a significantly increasing trend till 2015 since 1972.

    Abstract: The response of mukhi kachu (Colocasia esculenta L.) cultivars (Bilashi and local variety) to inorganic (Urea 140 kg ha-1, TSP = 95 kg ha-1, MP= 145 kg ha-1 and Urea 150 kg ha-1, TSP= 100 kg ha-1, MP= 150 kg ha-1) and organic (Cowdung 5,000 kg ha-1, Mustard oil cake 100 kg ha-1 and Poultry dropping 8000 kg ha-1) fertilizers was determined at the farm of Patuakhali Science and Technology University, Bangladesh. Different levels of organic and inorganic fertilizers showed significant effect on number of cormels, dry weight of cormels and yield of the crop. Organic fertilizers produced significantly higher yields of mukhi kachu over the inorganic fertilizers. The variety Bilashi produced the highest yield (25.66 t ha-1) with the application of mustard oil cake.

    Abstract: Objective: Goiter is a common form of thyroid swelling among population living in areas of iodine deficiency. Of the goiters, non-toxic multinodular goiter (MNG) is the most common and benign form of thyroid disease. However, studies have shown that long-standing MNGs may harbour malignancy. The present study was, therefore, undertaken to ascertain the prevalence of malignancy in clinically diagnosed non-toxic MNGs. Methods: This cross-sectional study was conducted in the Department of Surgery (all four-units) and ENT Department, Rajshahi Medical College Hospital, Rajshahi from July 2007 to June 2008 on 100 patients of clinically non-toxic MNG who underwent thyroid surgery. Biopsy materials were taken from the excised nodules and were sent for histopathological examination to see what proportion of them harboured malignancy. Result: The findings of the study showed that 40% of patients were 30-40 years old with mean age of the patients being 35.5 ± 10.1 (range: 14-75) years. Majority (87%) of the patients was female. Half of the patients with non-toxic MNGs had a history of thyroid swelling for 1-5 years and the rest half for 〉 5 years with median duration of illness being 5.5 years (range: 1-30 years). Of the 100 patients 4% had stridor and 6% dyspnoea; dysphagia and cervical lymphadenopathy each was 6%. Nearly 60% of the patients had goiter of size 15 sq-cm or below. Sub-total thyroidectomy was the most common operation performed (33%) followed by right hemi-thyroidectomy (24%), near total thyroidectomy (20%), left hemi-thyroidectomy (18%) and total thyroidectomy (5%). Histopathological examination of resected specimens revealed that 15% of the MNGs had malignancy with papillary to follicular carcinoma ratio being 4:1. Histopathological typing showed that 79% was simple MNGs, 4% follicular adenoma, 12% papillary carcinoma, 3% follicular carcinoma and 2% chronic thyroiditis. Neither age nor sex was found to be associated with presence of malignancies in MNGs (p = 0.865 and p = 0.647 respectively). The goiter-size was also not associated with presence of malignancies (p = 0.691). However, the mean duration of thyroid swelling in patients who had malignancy was much higher (8.5 years) than that in patients who did not haveany malignancy (5.7 years) (p = 0.024). Conclusion: The study concluded that a small proportion of long-standing non-toxic MNGs may turn into malignancy. Therefore, routine operative treatment without confirming that the cases are malignant does not seem to be justified. Ibrahim Card Med J 2020; 10 (1 & 2): 84-88

    Abstract: Background & Objective: Fine-needle aspiration cytology (FNAC) is frequently used to evaluate a breast lump, for it is safe, reliable, and time saving outdoor procedure for evaluation of breast lumps. However, in recent times, the diagnosis of breast lesions has mostly become dependent on Tru-cut biopsies with a gradual reduction in the rate of performing fine-needle aspiration cytology (FNAC). The present study was undertaken to compare the diagnostic accuracy of Tru-cut Biopsy and Fine Needle Aspiration Cytology (FNAC) in the evaluation of a clinically palpable breast lump. Methods: This cross-sectional study was conducted in the in Department of Surgery, Rajshahi Medical College & Hospital, Rajshahi, Bangladesh over a period of one year from July 2019 to June 2020. A total of 60 women with clinically palpable breast lumps were consecutively included in the study. All patients underwent FNAC and Tru-cut Biopsy to make a provisional (clinical) diagnosis of breast lump followed by histopathology of the excision biopsy taken during surgery to confirm the diagnosis. The diagnostic accuracies (sensitivity, specificity, positive and negative predictive values) of the two diagnostic modalities (FNAC and Tr-cut Biopsy) were then judged by comparing them with the histopathological diagnosis of the excision biopsy. Result: In the present study majority (86.7%) of the lesions was malignant in terms of histopathology with almost all being infiltrating carcinoma (98%). Only 8(13.3%) cases were benign breast lesions and all of them were fibroadenoma. While Tru-cut or Core-needle biopsy (CNB) diagnosed 85% of the lesions as malignant, FNAC diagnosed 65% of lesions as malignant. The sensitivity of CNB was observed to be 98.1% and its specificity was 100%. In contrast, the sensitivity of FNAC was much lower (65%), although its specificity was comparable to that of CNB (100%). While the overall diagnostic accuracy of CNB was 98.3%, that of FNAC was 78.3%. The test of agreement between Tru-cut biopsy and FNAC using kappa-statistics showed that the two diagnostic modalities had moderate agreement in differentiating malignant breast tumours from the benign ones (k-value = 0.494, p 〈 0.001). Conclusion: The study concluded that Tru-cut biopsy and FNAC both are hundred percent specific in excluding a benign breast lesion. However, the sensitivity of FNAC is much lower compared to that of CNB with overall diagnostic accuracy of Tru-cut biopsy being much higher than FNAC. Ibrahim Card Med J 2021; 11 (1): 21-27
