    Notes: Waveguides which are single mode at a wavelength of 1.32 μm, and which have linear electro-optic properties, have been fabricated by spin coating polymers onto etched indium phosphide. The side-chain polymer, which comprises the core of the waveguide, develops linear electro-optic properties following an electric field alignment process. A phase modulator has been fabricated by this method and was found to have a switching voltage of 30 V, for a π phase change. The total insertion loss of the device was measured between single-mode lensed fibers to be 9.4 dB. The electro-optic coefficient of the core polymer was calculated from the switching voltage using the geometry of the device and found to range from 6 to 11×10−12 m/V.

    Notes: Kinetics of structure formation in electrorheological (ER) suspensions are characterized by the time required to form percolating fibers following application of an external electric field. Experimental results are compared with predictions from simulations using an electrostatic polarization model for ER suspensions. Results from the two approaches agree, with large response times at small concentrations, decreasing to 10 ms at large concentrations and field strengths of 2 kV/mm.

    Notes: Neutron scattering measurements of the critical fluctuations associated with the spin-density-wave transition at the Néel temperature, TN≈312 K, in a single-Q chromium crystal are reported. Critical fluctuations are observed emanating from allowed magnetic satellite positions and from satellite positions, corresponding to absent magnetic domains, at which no elastic scattering occurs. The inelastic scattering from these "silent satellites'' grows rapidly with increasing temperature, becoming equal to the allowed single-Q satellite scattering at TN. The line shapes of the inelastic scattering at both the allowed and silent satellites are found to be intrinsically asymmetric, reflecting the underlying nesting geometry of the Fermi surface. Energy scans at the silent satellite positions reveal a weakly temperature dependent, characteristic energy below which the observed scattering intensity falls rapidly to zero. © 1996 American Institute of Physics.

    Notes: We describe an interdigital linear electro-optic modulator produced from a novel polymer. The polymer was formed into a thin phase grating by simultaneous application of heat and a periodic electric field. The first maximum generated by diffraction of 633-nm HeNe laser light passing through the grating was electrically modulated to a depth of over 17%. The performance of the modulator is compared with a quadratic electro-optic modulator having the same electrode structure but where the polymer was replaced by nitrobenzene. It is estimated that the application of a thermopoling field strength of 20 V/μm to the polymer would impart a linear electro-optic coefficient of about 30 pm/V, which is comparable to the largest coefficient of lithium niobate.

    Notes: Nonclassical symmetry solutions of physically relevant partial differential equations are considered via the reduction methods of Bluman and Cole and Clarkson and Kruskal. Consistency conditions will be provided to show that, if satisfied, these two methods are equivalent in the sense that they lead to the same symmetry solutions. The Boussinesq equation and Burgers' equation are used as illustrative examples. Exact solutions, one of which is new, will be presented for Burgers' equation obtained from the Bluman and Cole method, yet not obtainable by Clarkson and Kruskal's method.

    Notes: We have used multilayer mirror optimization methods to enhance the coupling of pump light into vertical-cavity surface-emitting lasers (VCSELs). With previously reported devices, pump light was coupled into VCSEL cavities through interference notches in the mirror reflectance spectrum. This approach is sensitive to temperature dependent reflectance spectrum shifts. We have created devices with a wide pump-band window of low reflectance. We report the simulation, growth, and optically pumped lasing of such optimized low-ripple VCSELs. Further, broadband pump windows open the possibility of spectrally-broad optical pumps and they eliminate need for costly tunable pump lasers. © 1996 American Institute of Physics.

    Description: Introduction Low back pain (LBP) is a prevalent and costly condition. The majority of patients experiencing LBP are managed in primary care, where first-line care recommendations consist of advice to self-manage and remain active. Internet interventions present a potential means of providing patients with tailored self-management advice and evidence-based support for increasing physical activity. Methods/analysis This protocol describes a single-blind, randomised controlled feasibility trial of an internet intervention developed to support the self-management of LBP in primary care. Patients are being randomised to 1 of 3 groups receiving either usual primary care, usual primary care with the addition of an internet intervention or an internet intervention with physiotherapist telephone support. Patients are followed up at 3 months. Primary outcomes are the feasibility of (1) the trial design/methods, (2) the delivery of the internet intervention and (3) the provision of telephone support by physiotherapists. Secondary outcomes will include exploratory analysis of estimates and variation in clinical outcomes of pain and disability, in order to inform a future main trial. Ethics/dissemination This feasibility trial has undergone ethical scrutiny and been approved by the National Health Service (NHS) Research Ethics Committee, REC Reference 13/SC/0202. The feasibility findings will be disseminated to the research community through presentations at conferences and publication in peer review journals. Broader dissemination will come following a definitive trial. Trial registration number ISRCTN 31034004.

    Description: Objectives Current musculoskeletal outcome tools are fragmented across different healthcare settings and conditions. Our objectives were to develop and validate a single musculoskeletal outcome measure for use throughout the pathway and patients with different musculoskeletal conditions: the Arthritis Research UK Musculoskeletal Health Questionnaire (MSK-HQ). Setting A consensus workshop with stakeholders from across the musculoskeletal community, workshops and individual interviews with a broad mix of musculoskeletal patients identified and prioritised outcomes for MSK-HQ inclusion. Initial psychometric validation was conducted in four cohorts from community physiotherapy, and secondary care orthopaedic hip, knee and shoulder clinics. Participants Stakeholders (n=29) included primary care, physiotherapy, orthopaedic and rheumatology patients (n=8); general practitioners, physiotherapists, orthopaedists, rheumatologists and pain specialists (n=7), patient and professional national body representatives (n=10), and researchers (n=4). The four validation cohorts included 570 participants (n=210 physiotherapy, n=150 hip, n=150 knee, n=60 shoulder patients). Outcome measures Outcomes included the MSK-HQ's acceptability, feasibility, comprehension, readability and responder burden. The validation cohort outcomes were the MSK-HQ's completion rate, test–retest reliability and convergent validity with reference standards (EQ-5D-5L, Oxford Hip, Knee, Shoulder Scores, and the Keele MSK-PROM). Results Musculoskeletal domains prioritised were pain severity, physical function, work interference, social interference, sleep, fatigue, emotional health, physical activity, independence, understanding, confidence to self-manage and overall impact. Patients reported MSK-HQ items to be ‘highly relevant’ and ‘easy to understand’. Completion rates were high (94.2%), with scores normally distributed, and no floor/ceiling effects. Test–retest reliability was excellent, and convergent validity was strong (correlations 0.81–0.88). Conclusions A new musculoskeletal outcome measure has been developed through a coproduction process with patients to capture prioritised outcomes for use throughout the pathway and with different musculoskeletal conditions. Four validation cohorts found that the MSK-HQ had high completion rates, excellent test–retest reliability and strong convergent validity with reference standards. Further validation studies are ongoing, including a cohort with rheumatoid/inflammatory arthritis.
