    Notes: Plasma prostaglandin F (PGF) was measured by a radioimmunoassay method in 24 subjects with tumours of the female genital tract. Levels in the tumour group ranged from 25 pg/ml to 134 pg/ml and were almost all greater than the values observed in a control group of 41 healthy women who had a mean (±SD) level of 21 (±3.4) pg/ml. There was a relationship between plasma PGF levels and the stage of the disease. Patients with resectable tumours showed a fall in PGF levels postoperatively and those with disseminated malignancy demonstrated increasing levels as the disease progressed. Plasma PGF may provide a clinical prognostic marker in the management of patients with malignant tumours.

    Notes: The concentration of free fatty acids was measured in blood taken from the umbilical artery and vein of 29 infants during elective Caesarean section and compared with levels in maternal venous blood. The mean venous-arterial (v–a) difference was 0·07 mEq/1. There was a direct and significant correlation between the umbilical v–a difference and maternal venous concentrations.

    Notes: Termination of pregnancy was performed under local anaesthesia as an outpatient procedure on 251 patients. The gestational ages ranged from 5 to 12 weeks. Either the portable Karman curette equipment or the syringe kits were used. The incidence of complications was low. Only 6 per cent of patients would have preferred general anaesthesia. The optimal gestational age at which to terminate pregnancies appeared to be between 6 and 10 weeks. Ninety-one per cent of patients used effective contraception after the termination.

    Notes: One hundred patients with an incomplete abortion, 14 patients with retained products of conception after a second trimester termination, 5 patients with missed abortion and 2 patients with secondary postpartum haemorrhages (making a total of 121 patients) had a uterine evacuation in a treatment room using the portable Karman curette equipment. No general anaesthesia was used but patients were given intravenous pethidine or papavaretum (Omnopon) and diazepam. Four patients found the procedure painful but only two of them would have preferred general anaesthesia. There were no immediate complications but three patients required re-evacuation of the uterus.

    Notes: Ninety patients, premedicated with temazepam 20 mg, undergoing suction termination of pregnancy, were studied. Forty patients received alfentanil (500 μ g), and thirty halothane (1.5%) as supplements to either etomidate or methohexitone–nitrous oxide–oxygen anaesthesia. A further 20 patients received alfentanil 250 μg at induction of anaesthesia with methohexitone, followed by a further 250 μg immediately prior to the onset of surgery. Recovery was assessed by the time patients took to open eyes on command, to giving their correct date of birth, and to performing the deletion test. Satisfactory operating conditions were not obtained with the combination of etomidate and halothane. Faster recovery was seen in patients with either etomidate or methohexitone and receiving alfentanil (p 〈 0.01) Testing by the ‘p’ deletion test showed impaired psychomotor performance at 30 minutes after cessation of anaesthesia in all groups compared with the pre-operative scores. Side effects leading to unsatisfactory anaesthesia were more frequent in patients who had received etomidate (p 〈0.01).

    Notes: Four out of 100 patients undergoing regional hip block (obturator nerve and nerve to obturator femoris) developed convulsions. These followed the injection of bupivacaine to block the nerve to the quadratus femoris muscle. The reasons for this are discussed. Not only has hip nerve regional blockade been shown to be ineffective in the relief of chronic pain of osteoarthritis but it must be concluded that the method described carries on unacceptably high morbidity.

    Notes: Lankesteria clarki sp. n. from the treehole mosquito, Aedes sierrensis (Ludlow) in California, is described. This species can be separated from the closely related ones, L. culicis (Ross) and L. barretti Vávra, by the totally intracellular nature of the trophozoite, the shape of the gamont, the position of the gamont nucleus and the structure and position of the residual body in the spore. Morphologic and ultrastructural investigations of gametogenesis and sporogony were conducted and the host relationship discussed. Lankesteria clarki was found in mosquitoes collected from all treeholes examined near Novato in Marin County, California and an examination of over 6,000 larvae of A. sierrensis showed an infection rate of 27.5%. The parasite is capable of destroying the midgut epithelial cells during its trophic stage and Malpighian tubule cells during gametogenesis and sporogony.

    Description: Human immunodeficiency virus type 1 (HIV-1) infection is a significant global public health problem for which development of an effective prophylactic vaccine remains a high scientific priority. Many concepts for a vaccine are focused on induction of appropriate titers of broadly neutralizing antibodies (bNAbs) against the viral envelope (Env) glycoproteins gp120 and gp41, but no immunogen has yet accomplished this goal in animals or humans. One approach to induction of bNAbs is to design soluble, trimeric mimics of the native viral Env trimer. Here, we describe structural studies by negative-stain electron microscopy of several variants of soluble Env trimers based on the KNH1144 subtype A sequence. These Env trimers are fully cleaved between the gp120 and gp41 components and stabilized by specific amino acid substitutions. We also illustrate the structural consequences of deletion of the V1/V2 and V3 variable loops from gp120 and the membrane-proximal external region (MPER) from gp41. All of these variants adopt a trimeric configuration that appropriately mimics native Env spikes, including the CD4 receptor-binding site and the epitope for the VRC PG04 bNAb. These cleaved, soluble trimer designs can be adapted for use with multiple different env genes for both vaccine and structural studies.
