    Abstract: Iron is an essential element for all microorganisms. Bacteria and fungi produce versatile siderophores for binding and storing this essential transition metal when its availability is limited in the environment. The aim of the study was to optimize the fermentation medium of Aspergillus fumigatus for siderophore production. Triacetyl-fusarinine C and ferricrocin yields were dependent on glucose and glycine supplementations as well as the initial pH of the culture media. The optimal fermentation medium for triacetylfusarinine C production contained 8% glucose, 0.4% glycine and the initial pH was set to 5.9. Meanwhile, maximal ferricrocin yields were recorded in the presence of 10% glucose, 0.5% glycine and at an initial pH of 7.4. Under optimized fermentation conditions, the yields for triacetylfusarinine C and ferricrocin increased up to 2.9 g/l culture medium and 18.9 mg/g mycelium, respectively.

    Abstract: Introduction: The first renal transplantation was completed in 1991 at the University of Debrecen. In 2013 Hungary joined Eurotransplant. Aim: The authors retrospectively compared the trends. Method: Comparison between Period A (from January 1, 2008 to August 31, 2013) and Period B (from September 1, 2013 to October 22, 2015). Results: The proportion of living transplants rose from 3.5% to 9.1 %. During period B over 25% of utilized donors were over 60 years of age. Recipients with body mass index above 30 kg/m 2 increased from 12% to 31%. Prevalence of diabetes among recipients rose twofold. Uretero-neocystostomy was used during period A (99%) while in period B end to side uretero-ureteral anastomosis has also gained popularity (68%). In 2013 the authors introduced routine use of induction treatment. Acute rejection rate decreased from 34% to 8%. The rate of surgical complications did not change. Acute bacterial infections decreased from 41% to 33%. Cumulative renal allograft 1, 3 and 5 year survival rates were 86.6%, 85% and 82.7% in group A vs. projected rates 88%, 84% and 84% in group B, respectively. Conclusions: Despite the growing proportion of expanded criteria donors, the authors were able to maintain a low incidence of delayed graft function and a favorable graft survival.     Abstract: Growing NH 4 + content of groundwater results in increasing exchangeable and fixed ammonium ion content of the soil. NH 4 + bond in the soil may go again into solution parallel with the dilution of the soil solution but at a slower rate than fixing. This process influences significantly the NH 4 + content of the soil. In settlements with no sewerage system the high NH 4 + content of sewage flowing out of uninsulated septic tanks may increase the fixed NH 4 -N content of the soil that could have a significant effect on the quality of groundwater even after the potential disappearance of pollution sources. In this study the effects of the fixed NH 4 -N content of the soil around an uninsulated residential septic tank on the purification processes of the groundwater were investigated. The septic tank in the study area was dismantled in 2014 after 27 years of operation as a sewerage system was constructed. When the tank was still in operation in 2012 and 2013, very high, 55–75 mg l - 1 NH 4 + content was measured in the water of the monitoring well 1 metre from the tank in the course of seasonal sampling. When sewage outflow was terminated in 2014 concentrations decreased right away but even 5 years after pollutant supply was stopped, concentrations (35–57 mg l - 1) highly exceeding the pollution limit (0.5 mg l - 1) were measured. Considering this very high concentration, it can be assumed that great amount of NH 4 + is still released into the groundwater. In order to prove this, the exchangeable and fixed NH 4 -N and NO 3 -N contents of the soil were determined by 20 cm down to a depth of 4 metres (2019). The measurements indicated the significant accumulation of exchangeable and fixed NH 4 -N in the zone between 220 and 400 cm. Highest fixed NH 4 -N concentrations of 457 mg l - 1 were found between 220 and 240 cm suggesting that sewage outflow was most intense at this depth. Slow decrease in concentrations can be observed in deeper zones but concentrations higher than 350 mg l - 1 were measured between 220 and 380 cm. Based on correlation analyses, the quantity of fixed NH 4 -N shows no correlation with the soil texture thus it can be stated that the vertical pattern of NH 4 -N content is determined dominantly by sewage outflow and its depth. In the unsaturated zone of the borehole a significant accumulation of NO 3 -N was also identified. The maximum of NO 3 -N was found in the zone between 100 and 140 cm. The peak nitrate calculated for NO 3 - ion with a value 〉 1300 mg kg - 1 is 2.5 times the limit set for the nitrate content of the geological medium. Based on the results, exchangeable and fixed NH 4 -N contents in the soil are still very high, 5 years after sewage outflow was stopped. The continuous solution of this component still contributes to the high NH 4 + content of the groundwater. As a result, the contaminated soil in the immediate environment of the septic tank is still a pollution source.

    Abstract: Összefoglaló. A korábban SARS-CoV-2-vírusfertőzésen átesett betegeknél különösen fontos az elektív műtét időpontjának megválasztása. Egy több, mint százezer beteget felölelő tanulmány szerint átlagosan 7 hét szükséges az elektív műtétre bocsátáshoz. Sürgős műtétek esetében a beteg általános állapota és a műtéttől várt javulás alapján mérlegelendő a műtét időpontja. A vitális indikációjú műtétek képezik a legnagyobb rizikót, hiszen ezen esetekben igen csekély idő és lehetőség van a preoperatív betegelőkészítésre. A SARS-CoV-2-vírusfertőzés nem minden esetben múlik el nyomtalanul. Szövődményeként felléphetnek cardiopulmonalis, véralvadási, központi idegrendszeri, mozgásszervi problémák, akut veseelégtelenség, ion- és vércukorháztartás felborulása, valamint gastrointestinalis eltérések is, melyek lehetnek rövid vagy hosszú távúak, illetve egyes esetekben maradandó károsodások is. A preoperatív kivizsgálási stratégiát, a műtéti érzéstelenítés módszerét (általános vagy regionális), valamint az intraoperatív monitorozást ezen szövődmények súlyosságának, valamint a műtét típusához megfelelően kell megválasztani. A posztoperatív időszakban a SARS-CoV-2-vírusfertőzésen átesett betegek körében gyakrabban lehet szükség emelt szintű monitorozásra vagy intenzív osztályos megfigyelésre. Orv Hetil. 2022; 163(18): 695–701. Summary. The timing of elective surgery is very important in patients, who suffered SARS-CoV-2 virus infection. According to a study of more than 100,000 patients, it takes approximately seven weeks to be safely admitted to elective surgery. In the case of emergency surgeries, the date of the surgery should be considered based on the patient’s general condition and the expected improvement from the surgery. Surgery with a vital indication takes the greatest risk, as there is very little time and opportunity for preoperative examination in these cases. SARS-CoV-2 virus infection does not go away without a trace. Complications may include cardiopulmonary, coagulation, central nervous system, locomotor problems, acute renal failure, ionic and diabetic disorders, and gastrointestinal abnormalities, which may be short-term or long-term, and in some cases permanent. The preoperative examination strategy, the method of anesthesia (general or regional) and the intraoperative monitoring should be chosen according to the severity of these complications and the type of surgery. In the postoperative period, patients suffered with SARS-CoV-2 virus infection may require more frequent monitoring or intensive ward monitoring. Orv Hetil. 2022; 163(18): 695–701.

    Abstract: Abstract: Introduction: According to the literature, somatic and psychological factors are connected with asthma bronchiale. The health behavior of these patients can be influenced by the virtual world. Aim: We wish to analyze social media and internet usage, self-esteem and sociodemographic data connected to attachment. Method: For our research, the Facebook Intensity Scale, the Problematic Internet Usage Questionnaire, the Rosenberg Self-Esteem Scale and a socio-demographic questionnaire were used. We interviewed 175 asthmatic children from the age of 10 to 18 (92 boys, 83 girls). Results: Asthmatic girls, children who went to daycare (under the age of 3) and who were not breastfed use Facebook more than boys, children who stayed at home instead of daycare and who were breastfed (p 〈 0.01). There is no difference in their internet usage. Breastfeeding (even if it only lasted for 1–7 days) can strengthen attachment which is shown in our results regarding social media usage compared with children who were not breastfed (p 〈 0.01). A negative relation with medium strength was found between the number of siblings (ρ = –0.400; p 〈 0.01) and the self-esteem and Facebook usage (ρ = –0.475; p 〈 0.01). There is a positive correlation between Facebook usage and the size of the place the children live in (ρ = 0.492; p 〈 0.01). Conclusions: Asthmatic youth use social media more frequently which provides social feedback to compensate their self-esteem, especially by girls. As protection factors, we can emphasize the existence of human relationships and the importance of an early mother-child bond. Orv Hetil. 2018; 159(47): 1971–1980.

    Abstract: The incidence of infective endocarditis is underestimated in solid organ transplant recipients. The spectrum of pathogens is different from the general population. The authors report the successful treatment of a 58-year-old woman with infective endocarditis caused by atypical microorganism and presented with atypical manifestations. Past history of the patient included alcoholic liver cirrhosis and cadaver liver transplantation in February 2000. One year after liver transplantation hepatitis B virus infection was diagnosed and treated with antiviral agents. In July 2007 hemodialysis was started due to progressive chronic kidney disease caused by calcineurin toxicity. In November 2013 the patient presented with transient aphasia. Transesophageal echocardiography revealed vegetation in the aortic valve and brain embolization was identified on magnetic resonance images. Initial treatment consisted of a 4-week regimen with ceftriaxone (2 g daily) and gentamycin (60 mg after hemodialysis). Blood cultures were all negative while serology revealed high titre of antibodies against Chlamydia pneumoniae. Moxifloxacin was added as an anti-chlamydial agent, but neurologic symptoms returned. After coronarography, valvular surgery and coronary artery bypass surgery were performed which resulted in full clinical recovery of the patient. Orv. Hetil., 2015, 156(22), 896–900.
