    Notes: The physiological and subjective effects of being touched on the wrist by another person were investigated in 20 normal adults at rest and during immersion of the hand in ice water. Touching reliably reduced heart rate compared to an immediately preceding baseline and compared to an alpha biofeedback condition. Heart rate during painful ice water stimulation was also lower when the subject was touched as compared to when he/she practiced alpha biofeedback, but this effect was not mediated by a reduction in the perceived painfulness of the ice water. Instead, touching and pain had independent, additive effects on heart rate. Touching did not produce generalized reductions in respiratory rate, SRR frequency, or frontalis EMG activity, although subjects did rate being touched as more pleasant and more relaxing than practicing alpha.

    Notes: The stability of physiological activity in the distal colon was investigated by recording 5–6 hours in each of 6 healthy adults. Contractions and myoelectric slow waves were recorded from the sigmoid colon (25–30 cm from the skin surface) and rectum (10–15 cm), and pressure waves were recorded from the proximal small intestine. The activity index (sum of areas of all waves divided by recording time) varied by 200% to 800% across 4-min samples for all motility and myoelectric slow wave recordings. Spectral analysis indicated that contractile activity waxed and waned in a cycle with a period of 40–55 min in the colon and 64–80 min in the small intestine. Myoelectric slow wave activity in the colon cycled with a period of 30–40 min. Contractile activity in the sigmoid colon was correlated with similar activity in the rectum, but myoelectric slow wave activity in the colon was not correlated with myoelectric slow waves in the rectum. The frequency composition of contractions and slow waves was unstable over time.

    Notes: Abstract Urinary incontinence can occur poststroke owing to weakness or incoordination of sphincter muscles, impaired bladder sensation, or hyperreflexic, neurogenic bladder. Four male subjects who had urinary incontinence associated with a stroke that had occurred 8 months to 10 years earlier, and who averaged 1.6 to 7.5 accidental voidings per week, participated in an outpatient study with a 4-week scheduled-voiding baseline, 2 to 5 sessions of biofeedback-assisted bladder retraining, and 6- to 12-month follow-up. Training sessions included stepwise filling of the bladder and manometric feedback display of bladder pressure, abdominal pressure, and external anal sphincter pressure. Training procedures were designed to teach subjects to attend to bladder sensations, inhibit bladder contractions, and improve voluntary sphincter muscle control. All four subjects achieved and maintained continence regardless of substantial differences in subject characteristics, including laterality of stroke, degree of sensory impairment, and independence in daily activities.

    Notes: Abstract A pattern-recognition program was developed which emulates visual scoring of colonic myoelectric and pressure recordings. It smoothes digitized data with a moving average filter, computes difference scores between successive groups of three data points, and uses the signs of these difference scores to detect the beginning and end of waves. Adjacent waves are merged if their means are closer than 1.67 times the sum of their standard deviations, and amplitude and duration criteria are used to exclude nonsignificant waves. When compared to four experienced human scorers on randomly selected records, the program agreed as well with the human scorers as they agreed with each other, and it approached the level of agreement of these observers with themselves when they were asked to rescore the same records blindly four to six weeks later. Human scorers agreed with themselves on 36–71% of myoelectric slow waves and on 42–88% of pressure waves, compared to 100% test-retest reliability for the pattern-recognition program. Frequency histograms of the duration of waves detected by the pattern-recognition program differed from the spectra generated by the fast Fourier transform (FFT) method. This pattern-recognition program provides an alternative to spectral analysis for the reliable and objective quantification of colonic myoelectric slow waves and pressure waves.
