    Notes: Four hundred and twenty patients with moderate to severe acne vulgaris were treated with oral erythromycin and topical benzoyl peroxide to determine the optimum dose regimes. Our results show that the response was significantly less in patients with a greater severity of acne, with truncal acne and in those with a higher sebum excretion rate. There was a significantly better clinical result in patients given ig erythromycin daily than in those given 0.5 g daily (plus topical therapy in both groups). The relapse rate on stopping antibiotics is also significantly less in patients given i g daily and this dosage did not produce any increase in side effects. We suggest that any patient requiring oral antibiotics should initially be given i g daily of erythromycin (or tetracycline) for up to six months, plus topical therapy.

    Notes: Twenty-five patients with immunologically classified B cell chronic lymphocytic leukaemia (CLL) were examined for skin lesions. Six showed evidence of cutaneous involvement, and histological examination of skin biopsies form these patients revealed perivascular lymphocytic infiltration with some diffuse involvement in three. Immunohistological analysis with a range of B and T cell specific monoclonal antibodies revealed that in all cases the infiltrate was predominantly T cell in origin and that epidermotropism in three cases was also associated with T cell components. Six control patients with macroscopically normal skin were also biopsied and these biopsies subjected to the same assessment. All were normal.

    Notes: Summary The effect of high-dose cyproterone acetateethinyl estradiol treatment on rates of sebum secretion and on the proportions of linoleic acid (18∶2Δ9,12) and sebaleic acid (18∶2Δ5,8) in the skin surface lipids of three female acne patients was examined. Changes in rates of sebum secretion were evaluated indirectly by measuring the ratio of wax esters/(cholesterol +cholesterol esters) in the subjects' skin surface lipid. In two of the subjects, this ratio indicated a reduction of sebum secretion rates to the childhood range. Concomitantly, there was an increase in linoleic acid and a decrease in sebaleic acid in all lipid classes. In the third subject, in whom there was only a small reduction in sebum secretion rate, the proportion of linoleic acid in the cholesterol esters more than doubled, but the changes in the other lipid classes were small or nonexistent. The results indicate that the proportions of linoleic acid and sebaleic acid in sebum are influenced by sebum secretion rates.

    Notes: Abstract An optical blood hydration monitor for use with extracorporeal haemodialysis circuits is described. Studies in five stable haemodialysis patients demonstrate good correlations between measured blood water, haemoglobin concentration and optical attenuation during ultrafiltration. There are no significant errors introduced as a result of a changing extracorporeal circuit pressure and errors resulting from a changing state of oxygenation of haemoglobin are small. Changes in blood volume effected by ultrafiltration may be deduced if it is assumed that these are due solely to shifts of water into or out of the vascular compartment.
