    Notes: Summary. Ten fetuses, severely affected by Rhesus (D) haemolytic disease, received one to three intravascular blood transfusions at between 18 and 30 weeks gestation, with the use of fetoscopically guided needles into one of the umbilical cord vessels. Although the technique was successfully accomplished in all cases, the fetal response to the procedure was varied. Only two fetuses survived beyond the neonatal period, and one child subsequently died principally because of the problems resulting from premature delivery. The reason for the low rate of survival has been explored and the continued use of the method described is now questioned.

    Notes: Sun protection factors were determined by both an in vitro method which used resin casts taken from replicas of human skin and by an in vivo SPF method. Thirty-eight product development samples were tested for the level of sun protection using both methods and the results were compared. The values obtained showed a positive relationship which was closely approximated by a log-linear regression of in vivo data on in vitro data (regression coefficient, r2= 0.86).It is concluded that the cast technique is quick, convenient, inexpensive and in its present form useful for screening sunscreen products prior to in vivo SPF testing.

Fifty-one pregnancies complicated by rhesus (D) isoimmunization have been managed by serial fetal blood sampling between 17 and 36 weeks gestation as an alternative to amniocentesis for ΔOD453 measurements. In 36 pregnancies where the fetus was shown to be rhesus (D) positive and both measurements were made before any intrauterine fetal transfusions, the ΔOD453 value gave misleading predictions on 13 of 63 occasions (21%). Fetal haematocrit estimations provided a direct assessment of the haemopoietic compensation occurring, but fetal bilirubin and albumin concentrations did not correlate directly with disease severity. It is proposed that pregnancies complicated by severe isoimmunization can be more precisely managed by serial fetal blood sampling for haematocrit estimation than amniocentesis for ΔOD453 measurement thus avoiding unnecessary intervention or delayed treatment.

    Notes: This study has attempted to distinguish between allergic and irritant reactions to patch tests by semiquantitative histological methods. The extent of perivascular chronic inflammatory infiltrate at 72h in irritant patch test reactions to sodium lauryl sulphate was shown to be small and very consistent. whereas in allergic reactions in nickel sulphate it was generally larger and more variable in size (p 〈 0.02). The two major lymphocyte subsets (T4 and T8) were randomly intermixed in both types of reaction and formed the major component of both the perivascular and diffuse dermal infiltrate, without any evidence of selective migration The T4:T8 ratios were similar in focal and diffuse infiltrates. The number of T6 dendritic (putative Langerhans) cells in the epidermis (per mm inner epidermal length) was usually greatly reduced in irritant reactions (5–16 mm −1, mean 10 mm−1) but remained within normal limits in allergic reactions (6–33 mm−;1, mean 21 mm–1) (p 〈0.001). Comparable results were seen with other irritants (mercuric chloride and benzalkonium chloride) and older allergens (neomycin sulphate. ethylene diamine and potassium dichromate).In additional experiments, pairs of biopsies were taken from the reaction and from adjacent unaffected skin. The T6 cell density in the epidermis did not significantly differ between allergic reactions and control skin. By contrast, (lie irritant reactions had fewer T6 cells than the control skin (p 〈 0.001).

    Notes: A 42-year-old female presented with a 2.5 cm symptomless swelling on the left lateral surface of the tongue. Histologically, there were large, often pleomorphic, fibroblast-like and histiocyte-like cells, multinucleate giant cells and cells of variable morphology. Many of the cells had abundant vesicular cytoplasm. There were also aggregations of neutrophilic or eosinophilic leukocytes, or both. The cells in some areas formed a fascicular or storiform pattern, or both. Ultrastructurally, the vesicular appearance was due mostly to markedly dilated endoplasmic reticulum, but many of the neoplastic cells contained some lipid. There were some myofibroblasts and a few xanthofibroblasts. The cells of variable morphology had many of the ultrastructural features of the fibroblast and histiocyte-like cells. There were also some small undifferentiated cells with minimal cytoplasm. These ultrastructural features confirmed the histological diagnosis of malignant fibrous histiocytoma.

    Notes: The clinical features, action spectrum and subsequent course, are described in 33 subjects with thiazide-induced photosensitivity. The reaction appeared to be phototoxic in nature. The wavelengths involved were those of the longer UVA and, not infrequently, also the shorter UVB waveband. In most instances withdrawal of the drug resulted in clearance of the clinical reaction and a return of the action spectrum to normal. In the few subjects in whom the photosensitivity persisted, either in the form of the clinical reaction or an abnormal action spectrum, or both, either a specific photodermatosis was present or another potentially photoactive drug was being taken. In no instance, was there evidence of the induction of long-term chronic photosensitivity (persistent light reaction) following the withdrawal of the thiazide drug.

    Notes: Amiodarone-induced cutaneous photosensitivity was studied in 12 subjects treated with the drug. The action spectrum for the abnormal response to sunlight was shown to be within the range of 335–460 (±30) nm. The clinical features of the photosensitivity response suggested that it was most probably a phototoxic reaction, a conclusion supported by the results in in vitro studies which indicated activity mainly against cell membranes. Of the five in vitro models used, three—namely photohaemolysis, the inhibition of DNA synthesis in PHA stimulated lymphocytes and the killing of mouse peritoneal macrophages—provided unequivocal evidence of the phototoxic potential of both amiodarone and its major metabolite, desethylamiodarone. In each model desethylamiodarone produced a greater effect by a factor of between 2 and 10. In vitro, UV-B wavelengths produced a greater effect than UVA but the difference between the effective wavelengths in vivo and in vitro might be explained by the greater absorption of the shorter wavelength UV-B in the opidermis. Zinc oxide-containing preparations appeared to be the most effective in reducing the cutaneous photosensitivity. It is suggested that the long-term cutaneous pigmentation resulting from oral amiodarone has a significant photosensitivity component.
