    Notes: Skin-prick testing (SPT) with allergen is widely used in the investigation of respiratory disease, yet its relationship to the results of provocation testing of the nose or bronchus remains obscure. We have studied this relationship by determining the concentration of Dermatophagoides pteronyssinus extract and the associated weal size which resulted in the lowest ‘false positive’ (FP) and highest ‘true positive’ (TP) rates when compared to the results of bronchial and nasal provocation testing. Subjects studied included those with both positive and negative bronchial or nasal provocation tests. For each of a series of concentrations of D. pteronyssinus extract, standardized in relation to content of Der pI antigen, receiver operating characteristic (ROC) curves of TP against FP rates were constructed. Optimal diagnostic concentrations for SPT were 0.112–1.122 ± 106 IU of Der pI ml-1. However, at the lower end of this range the optimum weal areas for a positive test with the D. pteronyssinus extract were very small (〈5 mm2). More appropriate concentrations for use in clinical practice were 0.56–1.122 × 106 IU Der pI ml-1 since at these concentrations more easily measurable weal areas of 5–10 mm2 (2.6–3.6 mm diameter) were almost equally predictive of a positive provocation test.

    Notes: The efficacy of hyposensitization with standardised extract of Dermatophagoides pteronyssinus (D. pteronyssinus) conjugated to alginate and containing known amounts of antigen P1 (Conjuvac®) was tested in a double blind, placebo controlled, multi-centre study in 66 adult patients with perennial rhinitis. Patients received 11 weekly injections of increasing concentrations of Conjuvac containing from 56 × 101 to 448 × 103 IU D. pteronyssinus or placebo injections of the alginate diluent to some of which 5 μg of histamine has been randomly added This was followed by 15 monthly injections of Conjuvac or placebo. The severity of nasal blockage, sneezing and rhinorrhoea was recorded twice daily in a diary and visual analogue assessments (VAS) made at each clinic visit. Nasal provocation testing (NPT) was performed with increasing concentrations of the same extract of D. pteronyssinus as used in the hyposensitization injections, and changes in nasal airways resistance measured by passive anterior rhinomanometry. VAS was recorded and NPT was performed on entry to the study and after the fifth, ninth and final monthly injection. Conjuvac injections were well tolerated. Large local reactions (〉 5 cm) occurred within 30 min in only 1% of patients but later in 23%. No systemic reactions or anaphylaxis occurred within 30 min of injections, but urticaria or worsening of asthma and rhinitis was reported later in 3% of patients. A significant improvement in nasal obstruction occurred in the Conjuvac compared to the placebo treated group (P 〈 0.01) and there was significant increase in the percentage of patients able to tolerate provocation with the highest concentrations of D. pteronyssinus extract after nine and 15 maintenance injections of Conjuvac compared to the placebo (P 0.02, P 〈 0.001). Patient use of additional therapy decreased sooner in the Conjuvac treated group but was minimal in both groups after 5 months of the study.

    Notes: Abstract Plasma levels of histamine and Nt-methylhistamine were measured simultaneously by high performance liquid chromatography during the intravenous infusion of histamine acid phosphate in six normal volunteers. Progressive, dose-related increases in plasma histamine were noted, reaching a maximum value of 3.1±0.14 ng ml−1 corresponding to a maximum infusion rate of 180 ng kg−1 min−1 (means±SEM). Increases in plasma histamine were accompanied by a significant dose-related fall in mean diastolic blood pressure (baseline 74.0±4.4 mm Hg falling to 60.0±3.3 mm Hg at maximum infusion rate,p〈0.001) and an increase in pulse rate (baseline 76.3±2.8 beats min−1 rising to 89.24 beats min−1 at maximum infusion rate,p〈0.05). All subjects exhibited facial flushing, the threshold plasma histamine level for this effect being 1.3±0.15 ng ml−1 corresponding to an infusion rate of 60 ng kg−1 min−1. Elevation of plasma Nt-methylhistamine was seen in only one subject, who exhibited a level of 0.5 ng ml−1 at the highest infusion rate. These results suggest that measurements of plasma Nt-methylhistamine are unlikely to provide a useful index of histamine release into the circulation.

    Notes: Abstract Previous in vivo studies in rat and man indicate that chronic renal insufficiency leads to an increase in the capacity of the large intestine for K secretion. The present studies were performed in isolated rat distal colon with conventional and K-sensitive microelectrodes to determine the cellular basis for enhanced colonic K secretion after 70% nephrectomy. The data revealed that in animals fed a regular diet, nephrectomy had no effect on the Na or K conductance of the apical membrane, or the kinetics of the basolateral membrane Na-K pump, but intracellular K activity decreased from 70±4 mmol/l to 58±4 mmol/l (P〈0.005). In control (non-nephrectomised) animals, feeding a diet modestly (4-fold) enriched with K resulted in small but significant increases in the Na and K conductance of the apical membrane, no change in the kinetics of the basolateral membrane Na-K pump, and a rise in intracellular K activity from 70±4 mmol/l to 94±7 mmol/l (P〈0.005). In contrast, in animals fed the K enriched diet, nephrectomy resulted in (i) large, amiloride-sensitive increases in transepithelial voltage and total tissue conductance (consistent with an appreciable degree of secondary hyperaldosteronism), (ii) marked increases in the Na and K conductance of the apical membrane, (iii) significant hyperpolarisation of the basolateral membrane, (iv) a 100% increase (P〈0.02) in the maximum activity of the basolateral membrane Na-K pump, and (v) a rise in intracellular K activity from 94±7 mmol/l to 129±7 mmol/l (P〈0.0025). These data suggest that the combination of modest dietary K enrichment and 70% nephrectomy stimulated an active K secretory process which reflected an increase in the K excretory load applied to the colonic mucosa, and the effects of aldosterone. In this model of renal insufficiency, enhanced K secretion by the transcellular and paracellular (potential-dependent) pathways results in a marked rise in the K excretory capacity of the colon.
