    Notes: Summary The antihypertensive efficacy of minoxidil and captopril was compared in 23 males with essential or renal parenchymatous hypertension refractory to conventional antihypertensive drug therapy. Following a pretreatment period the patients were randomly assigned to receive either minoxidil, 2.5 mg twice daily (n=12), or captopril, 25 mg twice daily (n=11). In patients with diastolic blood pressure 〉95 mmHg, doses of minoxidil and captopril were increased in 2-week intervals. Patients who maintained diastolic pressure 〉95 mmHg and/or those with intolerable side effects were switched over to the alternative substance. After a mean observation period of 12 weeks a significant decrease in systolic and diastolic blood pressure was observed (179/114 vs 148/92 mmHg in the minoxidil group; 176/111 vs 158/97 mmHg in the captopril group). The primary response rate was 75% in patients treated with minoxidil and 55% in those with captopril (not significant). After the change to the alternative substance two of the four non-responders on captopril and one of the two non-responders on minoxidil became responders. Side effects occurred significantly more often during minoxidil than captopril (p〈0.05). The high efficacy of minoxidil and captopril in the treatment of severe hypertension refractory to conventional drugs was confirmed. Minoxidil lowered blood pressure slightly more than captopril, but it had a higher incidence of side effects than captopril.

    Notes: Summary In three patients with primary aldosteronism, the acute effect of a single dose of captopril on the elevated mean arterial blood pressure (MAP) was studied before and after 4 weeks of treatment with spironolactone. Before spironolactone therapy, captopril did not cause any drop in MAP. Four weeks later, after an oral daily dose of 400 mg spironolactone, MAP was still elevated in all three patients, though electrolyte abnormalities were fully corrected. Since plasma renin activity (PRA) was increased to values above the normal range, the acute effect of captopril on MAP was tested again. A single dose of 25 mg captopril then caused a fall in MAP to normal. These data reveal the conversion from a renin-independent to a renindependent kind of hypertension after spironolactone therapy in three patients with primary aldosteronism syndrome. This might be of pathogenetic and therapeutic interest.

    Notes: Summary The effect of insulin on skeletal growth was examined by (1) systemic injection, (2) local administration into the tibia growth plate and (3) in vitro by use of chondrocytes in culture. (1) Male rats, body weight 60–75 g, were hypophysectomised. One week after the operation, the animals were divided into three groups. Group A received intraperitoneally saline, group B insulin (5–30 U·kg−1·day−1) and group C human growth hormone (250 μg/kg/day) for the following 10 days. In addition, on day 10 the rats were injected with 10 μCi 35-S-sulfate intraperitoneally. Twenty-four h later in the non-fasting state plasma glucose, insulin, somatomedin activity (porcine assay), body weight, nose-rump length, width of the tibia growth plate, and the 35-S-sulfate incorporation into rib cartilage were determined. Compared to saline, growth hormone and insulin treatment significantly enhanced body weights, nose-rumb lengths, the widths of the proximal tibia growth plates and the incorporation of sulfate into rib cartilage. For the three skeletal growth parameters, growth hormone was more effective than insulin, while body weights did not differ after insulin or growth hormone treatment. So matomedin activity (U/ml) was low in group A (0.39±0.04, n=9, Mean±SEM) and group B (0.34±0.08, n=8) and high in the growth hormone treated group C (0.90±0.09, n=7; p〈0.002). (2) To test the possibility that insulin might directly augment skeletal growth, insulin (80 mU) was injected into the proximal tibia growth plate of one leg and saline into the cartilage zone of the other leg. Insulin treatment significantly increased the width of the cartilage zones. Insulin: 211±22 μm, saline 200±22 μm, (Mean±SD, n=6, p〈0.05). (3) Addition of human biosynthetic insulin and growth hormone to the culture medium increased colony formation of chondrocytes in a bell-shaped fashion. A plateau in colony formation was reached with 3.1–6.25 ng/ml insulin and 25–50 ng/ml growth hormone, but with larger dosages of both hormones, the effect was diminished. The results suggest that insulin might stimulate postnatal skeletal growth by a direct effect on the target cells.

    Notes: Abstract Three infants with severe combined immunodeficiency and adenosine deaminase (ADA) deficiency were treated by T-cell depleted bone marrow transplantation (BMT), using human leukocyte antigen (HLA)-haploidentical parents as donors. In the first patient, two initial transplants failed to engraft and no change of the immunodeficiency was observed. In order to overcome this graft resistance, cytoreductive conditioning was used prior to a third transplant. In the other two patients, similar conditioning was used prior to initial transplants. In all three patients, complete and permanent immunological reconstitution was observed and they survive from 3.5 to 5 years after transplantation. In biopsies obtained from iliac bones prior to BMT, osteochondral abnormalities characteristic of ADA-deficiency were noted in all three patients. After successful transplantation, these abnormalities had completely resolved. Our results demonstrate that cytoreductive conditioning prior to HLA-haploidentical BMT is useful in order to obtain stable engraftment and reversal of abnormalities associated with ADA deficiency.

    Notes: Summary In the newborn several situations of hyperinsulinism can be associated with myocardial hypertrophy and increased contractility. Insulin and the insulin-like growth factors (IGF) are derived from a common ancestral molecule. Insulin exerts mainly metabolic action, whereas the IGFs promote cell multiplication and differentiation. Using an assay system of cultured neonatal myocardial cells the stimulatory action of insulin and the insulin-like growth factors I and II on myocardial cell contractility was investigated. Spontaneously beating aggregates of myocardial cells were synchronized by an electric impulse generator. Contractility was measured via the amplitude of contraction by an optoelectronic system. Insulin at a concentration of 6,250 and 12,500 μU/ml increased the contractility by 11 and 18%; IGF-I at a concentration of 12 and 25 ng/ml, and IGF-II at a concentration of 25 and 50 ng/ml increased the contractility by 16 and 22%, and 13 and 18%, respectively. Lower concentrations did not provoke a significant increase in contractility. Insulin only in supraphysiological doses increases the contractility of neonatal myocardial rat cells, whereas both insulin-like growth factors act in physiological concentrations. Therefore, during hyperinsulinism insulin may increase myocardial contractility via the IGF receptor and not via the insulin receptor.

    Description / Table of Contents: Synthesis of [2]-Catenanes from [2]-RotaxanesStarting from the rotaxane 18a a mixture of catenane 21 and rotaxane 23 is obtained by attaching a polymethylene bridge between the methylene groups in α-position of the sulfonyl groups. The isocyclic catenane 28 containing a 28- and a 46-membered ring is obtained from the catenane 21 by splitting the bonds between the bridgehead atoms and the sulfonyl groups. The 1H-, 13C-NMR, and mass spectra of this compounds are discussed.
    Notes: Ausgehend von dem Rotaxan 18a wird durch Angliederung einer Polymethylenbrücke zwischen den beiden zu den Sulfonylgruppen α-ständigen Methylengruppen ein Gemisch von Catenan 21 und Rotaxan 23 synthetisiert. Aus 21 wird nach Spaltung der Bindungen zwischen den Brückenkopfatomen und den Sulfonylgruppen das isocyclische Catenan 28, bestehend aus einem 28-und einem 46-gliedrigen Ring, erhalten. Die 1H-, 13C-NMR-Spektren und Massenspektren dieser Verbindung werden diskutiert.

    Description / Table of Contents: Directed Synthesis of Translationally Isomeric [3]-Catenanes1)In a multi-step reaction sequence the tetrahydroxymetacyclophane 12c is synthesized. Acetalisation with 1,25-dichloro-13-pentacosanone followed by nitration and reduction afforded the diamine 13c. By cyclization of this compound in 2-pentanol with sodium carbonate and sodium iodide under high dilution conditions the monomeric products 16, 17, and 18 are obtained in yields of 21.4, 7.7, and 0.9%, respectively. On the basis of mass, 13C NMR and 1H NMR spectra the structure of these compounds is discussed. Starting from the precatenane 16 the [3]-catenanes 25a, b,c and 26a, b, c are obtained in a multi-step reaction sequence. The structure of these compounds is confirmed by mass, 13C NMR, and 1H NMR spectroscopic investigations.
    Notes: In einer vielstufigen Synthese wird das Tetrahydroxymetacyclophan 12c synthetisiert. Durch Acetalisierung mit 1,25-Dichlor-13-Pentacosanon, nachfolgender Nitrierung und Reduktion wird das Diamin 13c erhalten. Dessen Cyclisierung in 2-Pentanol in Gegenwart von Natriumcarbonat und Natriumiodid unter Verdünnungsbedingungen ergibt die monomeren Cyclisierungsprodukte 16, 17 und 18 in Ausbeuten von 21.4. 7.7 und 0.9%. Die Struktur dieser Verbindungen wird anhand der Massen-, 13C- und 1H-NMR-Spektren diskutiert. In einer mehrstufigen Reaktionsfolge werden aus dem Prä-[3]-catenan 16 die [3]-Catenane 25a, b, c und 26a, b, c erhalten. Die Struktur dieser Verbindungen wird durch 13C-NMR, 1H-NMR- und massenspektroskopische Untersuchungen bewiesen.

Structure of Bis-diansa CompoundsCyclization of diamine 1 under high dilution conditions afforded compound 2 with bis-diansa structure as the main product. The two bridged cis-trans isomeric compounds 8 and 13 could be isolated as byproducts. The structure of these compounds was determined by mass spectrometry and confirmed by preparing the tetraacetates 4 and 10. Rupture of the bonds between the aromatic nuclei and the bridged system in compound 2 resulted in the formation of the 22-membered macroheterocycle 6. In an analogous manner the 44-membered macroheterocycle 12 was prepared from one of the bridged compounds (8 or 13).
    Notes: Die Cyclisierung des Diamins 1 unter Verdünnungsbedingungen liefert als Hauptprodukt die Bis-diansa-Verbindung 2. Als Nebenprodukte werden die überbrückten cis-trans-isomeren Verbindungen 8 und 13 isoliert. Die massenspektrometrisch getroffenen Strukturzuordnungen wurden durch Darstellung der Tetraacetate 4 und 10 bestätigt. Nach Lösen der Bindungen zwischen den aromatischen Kernen und den doppelhenkligen Systemen konnte aus 2 weiterhin der 22-gliedrige Makroheterocyclus 6 erhalten werden, aus der überbrückten Verbindung (8 oder 13) dagegen der 44-gliedrige Makroheterocyclus 12.

    Description / Table of Contents: Thermolysis of Acetoxy-alkylbenzene and Hydroxy-alkylbenzene DerivativesThe thermolysis of long chain acetoxy-alkylbenzene and hydroxy-alkylbenzene derivatives at 340-350°C affords, probably via a radical reaction, products in which acetoxy or hydroxy groups and/or alkyl residues are substituted by hydrogen (tables 1, 2). The yields vary with type, number, and position of substituents on the benzene ring. The thermolysis of hydroxydialkylbenzene derivatives 14 and 20 gives in addition hydroxy-triaklylbenzene derivatives formed by transfer of alkyl radicals.
    Notes: Die Thermolyse von langkettigen Acetoxy-alkylbenzolen und Hydroxy-alkylbenzolen bei 340-350°C liefert in einer vermutlich radikalisch ablaufenden Reaktion Produkte, in denen Acetoxy-bzw. Hydroxygruppen und/oder Alkylreste durch Wasserstoff ersetzt sind (Tab. 1, 2). Die Ausbeuten sind von der Art, der Anzahl und der Stellung der Substituenten am Benzolkern abhängig. Die Thermolyse der Dialkyl-hydroxybenzole 14 und 20 ergibt zusätzlich Trialkyl-hydroxybenzole, die durch Übertragung von Alkyl-Radikalen entstehen.
