    Notes: Abstract Two neonates, one with complete and one with incomplete birth-related transection of the cervico-thoracic spinal cord, form the basis of this report. Ultrasound and MRI findings in primary diagnosis and follow-up are described. The aim of this contribution is to bring this serious birth complication to the attention of the reader, to present the obstetrical risk factors, to describe the clinical presentation of the newborns and to make suggestions to expand the field of indications for spinal sonography. The value of spinal ultrasound in the first six months of life is stressed, especially in comparison to MRI. For definitive assessment of the lesion (transectional or nontransectional) follow-up ultrasound studies for several weeks are required.

    Notes: Abstract Electromechanical and pharmacomechanical coupling was investigated in human ciliary muscle by measuring the intracellular free calcium in single cultured ciliary muscle cells and the contractility in meridional ciliary muscle strips. The basal resting calcium concentration was 75±8.7 nmol/l, n=23. Application of acetylcholine (0.1 mmol/l) and carbachol (0.1 mmol/l) resulted in an initial [Ca2+]i peak followed by a recovery phase and a [Ca2+]i plateau. The initial [Ca2+]i peak was still observed in the absence of extracellular calcium and in the presence of verapamil (0.1 mmol/l). During its plateau [Ca2+]i was decreased by withdrawal of extracellular calcium or application of verapamil (0.1 mmol/l). Depolarization induced by a high level of extracellular potassium yielded only a small transient [Ca2+]i peak without a [Ca2+]i plateau. In isolated ciliary muscle strips, muscarinic stimulation (carbachol 0.1 mmol/l) resulted in an initial phasic and a subsequent tonic contraction. Removal of external calcium reduced the phasic contraction to 30.6±4.4% (n=8) and completely abolished the tonic one. Verapamil (0.1 mmol/l) had only a slight relaxing effect when applied during the tonic contraction. We conclude that human ciliary muscle contraction is mediated by calcium release from intracellular stores and calcium entry through calcium channels, which are most probably receptor-operated. Depolarization of the muscle cell membrane and calcium entry through voltage-operated calcium channels do not contribute significantly to human ciliary muscle contraction.

    Description / Table of Contents: Résumé Nous avons utilisé une nouvelle technique d'injection de résine époxy pour étudier la vascularisation du tendon d'Achille. La partie distale postérieure est pauvrement vascularisée, ainsi que la partie moyenne du tendon. Les ruptures s'observent dans cette partie moyenne, mais exceptionnellement au niveau de la partie distale. Nous en concluons qu'il n'y a pas de relation directe entre la vascularisation et la fréquence des ruptures.
    Notes: Summary We have used a new technique of epoxy resin injection to examine the blood supply of the Achilles tendon. The posterior distal part showed poor vascularisation, as did the middle part of the tendon. Ruptures occur in the middle part, but only rarely in the distal part. We therefore suggest that there is no direct relationship between blood supply and the frequency of rupture.

    Description / Table of Contents: Résumé Cet article se propose d'analyser la participation à des campagnes initiées par le DHP (Etude allemande de la Prévention cardiovasculaire). La base de données est constituée par deux enquêtes représentatives, effectuées en 1988 dans la ville de Brême. Au total, 2360 personnes ont été interrogées; 149 d'entre elles ont indiqué avoir pris part aux campagnes du DHP. Une étude statistique selon plusieurs critères a mis en évidence la relation existant entre l'appartenance sociale, l'engagement en matière de santé et la probabilité de participation. Il apparaît que les diverses campagnes du DHP ont touché essentiellement des groupes sociaux à statut élevé et/ou manifestant déjà un intérêt pour les thèmes de santé. La critique consécutive à ce rapport pose le problème du facteur local dans le concept d'action au niveau de la collectivité.
    Abstract: Summary The subject of this article is an analysis of participation in campaigns of the German Cardio-Vascular-Prevention Study (GCP). In 1988 two representative surveys were carried out in the City of Bremen. Altogether, 2360 persons were questioned. Of these, 149 stated that they had participated in campaigns of the GCP. A multivariate analysis shows that the probability of participation was highest for people with an interest in health-related issues and for members of higher social classes. The discussion of these result emphasizes some problem areas in the use of such community-based intervention concepts.
    Notes: Zusammenfassung Gegenstand dieses Artikels ist eine Analyse der Beteiligung an Gesundheitsaktionen der Deutschen Herz-Kreislauf-Präventionsstudie (DHP). Datenbasis sind zwei 1988 in Bremen (Stadt) durchgeführte repräsentative Surveys mit insgesamt 2360 Befragten, von denen 149 angaben, an Aktionen der DHP teilgenommen zu haben. Eine multivariate Analyse zeigte eindeutige Zusammenhänge zwischen der Schichtzugehörigkeit, gesundheitsbezogenem Engagement und der Teilnahmewahrscheinlichkeit. Offenbar sprach die DHP mit vielen Aktionen im wesentlichen statushöhere Bevölkerungsgruppen bzw. solche mit bereits vorhandenem Interesse an Gesundheitsthemen an. Die anschliessende Diskussion dieses Befundes problematisiert die im Gemeindeinterventionskonzept enthaltene Annahme lokaler Bindungen.

    Notes: Abstract Conformations, acid-base and supramolecular properties of phenolic metacyclophanes obtained from the condensation of resorcinol with aldehydes are discussed, including the mechanisms involved in the formation of these macrocycles. The strong binding of choline-type compounds and the inhibition of acetylcholine hydrolysis with therccc stereoisomers is mechanistically evaluated; arctt isomer shows strong conformational coupling for, e.g., choline binding and simultaneous proton release. The presence of larger alkyl residues at the bottom of therccc macrocycle leads to an additional binding site for small lipophilic substrates, which is independent of the upper complexation center for positively charged substrates. Substitution at the upper rim by carboxylic groups at the 2-position of the phenyl rings yields receptors for, e.g., α, ω-diammonium ions with alternate equatorial and axial arylunits. Positively charged substituents at the upper rim, introduced by aminoalkylation, lead to little change of complexation as a result from their orientation away from the binding center. Aminoacid substituents, for the same reason, do not lead to enantioselective complexation, but allow particularly for strong binding of transition metal ions. Preliminary studies show that resorcinarenes bearing a wide array of positive charges are potent groove binders to ds-DNA without intercalative contributions.
