    Notes: Experiments are described in which a high-purity, high-power (0.15 TW, 1 MeV) proton beam is generated from an ion source consisting of H2 gas frozen onto a liquid-helium-cooled copper anode at 4.2 K in a series-field-coil extraction diode on the 0.7 TW HydraMITE-II accelerator. Peak anode proton current densities of 2 kA/cm2 were measured. This current density is a factor of 100 higher than those obtained in previous liquid-helium-cooled cryogenic diode experiments on small accelerators and is in the range required for high-power ion beam applications. Thomson parabola, Faraday cup, and carbon activation measurements indicate an ion beam proton fraction close to 100% for the cryogenic source, compared to 50–70% for the standard hydrocarbon anode tested. The cryogenic proton source is believed to consist of no more than a few monolayers of molecular hydrogen. The hydrogen-coated cryogenic anode shows a faster initial anode turn-on than other materials. However, source-limited emission from the thin hydrogen layer results in a somewhat longer current risetime, reduced ion diode efficiency, lower proton current enhancement over the Child–Langmuir limit, and a proton spectrum of lower average energy than for the hydrocarbon anode. Techniques to overcome these limitations are discussed. Cryogenic ion sources consisting of frozen N2, CH4, and Ne have also been studied. In each case, high intensity beams consisting predominantly of components of the refrigerated gas were produced.

    Notes: Accurate modeling of load behavior in Z-pinch plasma radiation sources driven by high-current generators requires the measurement of fast-rise-time multimegampere currents close to the load. Conventional current diagnostics mounted in inductive cavities (such as B-dot loops and Rogowski coils) fail at small radius because of electrical breakdown produced by high dI/dt. In this paper, we describe the use of large-signal, nanosecond-time-resolution lithium niobate piezoelectric stress gauges to directly measure the magnetic pressure B2/2μ0=μ0I2/8π2r2 generated at radius r by a current I flowing in a radial transmission line. Current measurements have been performed at radius r=2.54×10−2 m on Sandia National Laboratories' Proto-II (10 TW) and SATURN (30 TW) gas puff Z-pinch experiments with maximum currents of 10.1 MA and dI/dt to 2.1×1014 A/s. Comparisons with Faraday rotation and B-dot current diagnostic measurements at large radius are presented. Bremsstrahlung noise problems unique to the SATURN gas puff source are discussed. For a Y-cut lithium niobate stress gauge on a pure tungsten electrode, current densities up to I/2πr=78 MA/m can be measured before the electrode yield strength and the piezoelectric operating stress limit are exceeded. Above the Hugoniot elastic limit of the electrode material, the dynamic range and accuracy of the diagnostic are greatly reduced, but it appears that the technique can be extended to higher current densities using an X-cut quartz piezoelectric element and a tungsten-sapphire electrode impedance stack.

    Notes: Objective To help answer the question: should Doppler ultrasound of the umbilical circulation be made available to all pregnant women as part of their routine antenatal care?Design A randomised trial.Setting St James's University Hospital, Leeds.Subjects 2025 low risk primigravid women.Main outcome measures Obstetric intervention rates and short term neonatal morbidity.Results The incidence of abnormal Doppler was low (1.7%) with complete absence of end diastolic flow in only 0.3% of cases. No significant differences could be demonstrated between control and study women in any of the outcomes measured.Conclusion This study did not demonstrate any benefit or harm from Doppler ultrasound as a routine screening test for all low risk women, whereas our previous studies have suggested that it is useful in high risk pregnancies. Any marginal returns on extending access to Doppler ultrasound from high risk to all women must be small. Since this test has excellent performance characteristics (sensitivity and specificity) for the prediction of fetal hypoxia and acidosis our results call into question the cost to benefit ratio of all tests designed to predict these outcomes in low risk women.

    Notes: Objective–To compare routine versus highly selective use of Doppler ultrasound and biophysical scoring in higher risk pregnancy. Design–A pragmatic randomized trial. Setting–St James's University Hospital, Leeds. Subjects–500 pregnant women at high risk of intrauterine growth retardation or still birth. Interventions–Regular monitoring with biophysical profile assessment and Doppler velocity waveform recording in umbilical and uteroplacental arteries. Results immediately available to clinicians. Main outcome measures–Gestational age al delivery, obstetric intervention rates and short-term neonatal morbidity. Results–Risk factors were distributed very evenly between the 250 patients in the study and control groups respectively. A total of 902 biophysical profile and Doppler assessments were done in the 250 study group patients and only in 12 patients in the control group. In the study group, absent end-diastolic flow was found in only 2.7% of all 902 measurements. A persistently abnormal biophysical score was always associated with absence of end-diastolic flow. The mean gestational age at induction of labour was statistically and clinically similar in the two groups and there was no overall statistically significant difference in intervention rates between the two groups. There was a statistically significant lower frequency of depressed 5-min Apgar scores in the study group. Serious neonatal morbidity was also statistically significantly more common in the control group than in the study group. Conclusions–The use of Doppler ultrasound in higher risk pregnancies does not lead to an increase in iatrogenic preterm delivery. The total rate of positive tests on Doppler ultrasound is very low and persistently abnormal biophysical scores are unlikely to be found in patients where umbilical end-diastolic blood flow is present. Surrogate measures for fetal damage seem to be improved when clinicians have access to Doppler ultrasound assessments.

    Notes: A reliable sodium wire-array z-pinch load has been developed for use on the Saturn accelerator. This load routinely produces implosions with ≥200 GW of power in the heliumlike sodium 1s2–1s2p 1P1 line at 11 A(ring). Our best shots have used a 12.5-mm-diam, 20-mm-long annular array of 16 75-μm-diam pure sodium wires. This load will allow us to test predictions of gain in the sodium/neon resonant photoexcitation x-ray laser system.

    Notes: Summary Prostatic intraepithelial neoplasia (PIN) is a putative premalignant lesion of the prostate gland. PIN has been demonstrated to share morphologic and phenotypic similarities to invasive carcinoma of the prostate. In addition, PIN is spatially related to invasive carcinoma and occurs with greater frequency in men whose prostates harbor carcinoma. Prostate-specific antigen (PSA) is a glycoprotein produced by the prostatic epithelium. For PSA to be detected in the serum, it must traverse several tissue layers to reach the circulatory system. PSA levels associated with PIN are intermediate between those of benign and malignant prostate tissue. Spatially associated occult carcinoma, disruption of the basal cell layer, and increased vascularity may account for elevated PSA values in PIN.
