    Notes: Abstract A one-bolus (dose) of 1 g of methylprednisolone was administered intravenously to patients undergoing facial surgery or craniofacial surgery, before the termination of the operative procedure. The degree of facial edema noted was reduced, and when it occurred, it was mild and of shorter duration. These observations were made on the experimental design first, and later in the clinical setting. No adverse effects were noted, and patients given this treatment required less pain medication in the immediate postoperative period. The mechanism of action of the steroids is multifactorial, related to decrease in the accumulation of fluid at the capillary level, and reduction of flow at the venoarterial sphincters. The use of steroids is safe when used with caution in selected patients, and by experienced surgeons.

    Notes: Abstract Augmentation mammoplasty by the transaxillary approach is an acceptable and safe technique with few complications. Proper preoperative planning and attention to surgical detail result in symmetrical and natural appearing breasts. The major advantage is related to the present female attire and desires to have a concealed scar. In about 3 months the incision is hardly noticeable and the woman is very pleased.

    Notes: Abstract Twenty-four patients with severe anophthalmic orbit syndrome were treated with a combination of techniques utilizing autogenous tissue. Autogenous corticocancellous bone grafts were used in the orbit as a volume filler. The temporalis muscle and a fasciae flap with a pericranial flap were used as a curtain to cover the bone grafts and to give a softening effect. The prosthesis used was a regular eye shell. With this method it can be smaller, thinner, and lighter to produce less deformation of the lower lid, particularly when used for a long period of time. In children who have this deformity compounded by small orbits after oncologic treatment, three-wall orbital enlargement is done to achieve a larger orbit, so as to match the normal unaffected socket and produce symmetry.

    Notes: Abstract The “forceful” and macho look of a prototypical man may not be unduly appealing to others whom he meets. This “forceful” look might not even appeal to the individual himself. In order to soften this appearance, a series of operative procedures has been devised for use on the craniofacial skeleton. These surgical steps can be done in a single operation or as a series of multiple operative procedures. Moreover, the needs of some patients may require that only special segments of these procedures be performed. This article presents these operative procedures and describes the feasibility with which they can be performed. Complications and unfavorable outcomes, when they occur, are usually related to unrealistic expectations on the part of the patient. The surgical steps routinely performed are those that contour the forehead, orbits, malar eminence, cheeks, chin, angle of the mandible, and larynx. Three categories of patients are described: the female with a male face; the male with a “forceful look”; and the patient requesting a gender identity change. The psychosocial, psychological, and behavioral problems leading to the decision for surgery will be the basis of final patient selection.

    Notes: Success in the use of custom-fabricated Silastic implants in patients with facial contour defects led us to utilize this method in reconstructing mandibular defects. However, mandibular reconstruction with alloplastic material is quite difficult because of the presence of continuous stress and motion in that region. The evolution in the use of the mandibular implant from the commercially available one to the K-wire reinforced, prefabricated, silicone implant was largely dependent on the availability of silicone and the ultimate result obtained.In the past few years, about 30 subdermal implants were inserted. Some of those implants were placed immediately into patients undergoing resection for tumor. The implant was prefabricated prior to surgery from x-ray measurements of the anticipated segment to be resected; it was then used to reconstruct the defect prior to the final closure of the operative wound. Other implants were manufactured 6-18 months after resection for tumor or for deformity as a result of trauma. The implants were used to reconstruct the facial defects and to correct the functional abnormality, such as drooling or deviation of the affected side by the unstable mandibular segment. Patients with severe congenital malformation have soft tissue deficiency in association with their bony malformation. Therefore, implants in such conditions were doomed to failure.Patient selection is the key to successful implantation.

    Notes: An alloplastic tray made of poly(ethylene terephthalate) cloth mesh and poly(ether urethane) used in conjunction with cancellous bone-chip grafts is the basis for this new method for reconstructing facial osseous contour defects. The tray can be prefabricated in quantity and at low cost for standard defects, such as discontinuity defects of the mandible or for chin augmentation. It can be custom-fabricated for specific defects in individual patients as, for example, gunshot wounds to the forehead. The materials were chosen because of their known histocompatibility, the ease of fabrication, and because the materials can be molded to form esthetic contours. They can be easily modified at the time of surgery. Sterilization is by autoclaving.Initial studies were undertaken in mongrel dogs where mandibular discontinuity defects in the range of 2-4 cm were reconstructed. In man, the technique has been used to reconstruct discontinuity defects of the mandible ranging in size from 1.5-12 cm in 16 patients. Atrophic mandibular alveolar ridges have been augmented using this technique in 7 patients, thus providing an adequate alveolar ridge for dentures. Two patients with defects of the skull secondary to gunshot wounds have had frontal bone restoration using this method.Reconstructions of the ear, the chin, and the orbital floor are currently being evaluated. This technique overcomes many problems of esthetic contour reconstruction and combines the use of autogenous cancellous pmarticulate bone and a tissue-compatible synthetic composite material.
