    Notes: Transmission spectra of monodispersed particles exhibit pronounced resonance features from which particle properties can be deduced. ZnS powders were used as model materials since they can be synthesized by homogeneous precipitation with a narrow size distribution. The optical extinction spectra can be simulated using Mie scattering theory, and can be used as an in situ diagnostic tool for following the growth of particles during precipitation reactions. Using results of particle diameter measurements by other methods, information on the refractive index of the particles can be retrieved, indicating a highly porous internal structure of the spheres. Distinct features in the experimental and simulated spectra have been interpreted as to arise from circumference and diameter resonance effects in the particles. The simultaneous observation of these two effects allows one to estimate size and solid volume fraction from simple transmission spectra by application of an effective-medium model for the simulation parameters. The potential of application of Mie resonance effects for selective light screening or as pigments are discussed. © 1998 American Institute of Physics.

    Notes: Antagonists at the ionotropic non-NMDA [AMPA (amino-methyl proprionic acid)/kainate] type of glutamate receptors have been suggested to possess several advantages compared to NMDA (N-methyl-d-aspartate) receptor antagonists, particularly in terms of risk/benefit ratio, but the non-NMDA receptor antagonists available so far have not fulfilled this promise. From a large series of pyrrolyl-quinoxalinedione derivatives, we selected six new competitive non-NMDA receptor antagonists. The basis of selection was high potency and selectivity for AMPA and/or kainate receptors, high in vivo potency after systemic administration, and an acceptable ratio between neuroprotective or anticonvulsant effects and adverse effects. Pharmacological characteristics of these novel compounds are described in this study with special emphasis on their effects in the kindling model of temporal lobe epilepsy, the most common type of epilepsy in humans. In most experiments, NBQX and the major antiepileptic drug valproate were used for comparison with the novel compounds. The novel non-NMDA receptor antagonists markedly differed in their AMPA and kainate receptor affinities from NBQX. Thus, while NBQX essentially did not bind to kainate receptors at relevant concentrations, several of the novel compounds exhibited affinity to rat brain kainate receptors or recombinant kainate receptor subtypes in addition to AMPA receptors. One compound, LU 97175, bound to native high affinity kainate receptors and rat GluR5–GluR7 subunits, i.e. low affinity kainate binding sites, with much higher affinities than to AMPA receptors. All compounds potently blocked AMPA-induced cell death in vitro and, except LU 97175, AMPA-induced convulsions in vivo. In the kindling model, compounds with a high affinity for GluR7 (LU 97175) or compounds (LU 115455, LU 136541) which potently bind to AMPA receptors and low affinity kainate receptor subunits were potent anticonvulsants in the kindling model, whereas the AMPA receptor-selective LU 112313 was the least selective compound in this model, indicating that non-NMDA antagonists acting at both AMPA and kainate receptors are more effective in this model than AMPA receptor-selective drugs. Three of the novel compounds, i.e. LU 97175, LU 115455 and LU 136541, exerted potent anticonvulsant effects without inducing motor impairment in the rotarod test. This combination of actions is thought to be a prerequisite for selective anticonvulsant drug action.

    Notes: Abstract PURPOSE: This study was designed to evaluate the influence of intraoperative intermittent sequential compression (ISC) on venous blood return from the lower limbs during laparoscopic and conventional colorectal colectomy. METHODS: Fifty patients undergoing laparoscopic (n=25) or conventional (n=25) colorectal surgery were included in a prospective study. Peak venous flow (PFV) and the cross-sectional area (CSA) of the femoral vein were assessed by Doppler ultrasound examination intraoperatively. RESULTS: Age, gender, and body mass index were comparable between both groups. Baseline PFV was 21±6.6 cm/s in the conventional and 18.4±6.4 cm/s in the laparoscopic group (P=0.2). ISC increased PFV to 156±29 percent of the baseline value in the conventional group and to 161±29 percent in the laparoscopic group. PFV decreased after abdominal insufflation to 127±19 percent of the baseline value in the laparoscopic group and after laparotomy to 134±27 percent in the conventional group (P=0.3). PFV decreased slightly in both groups during surgery but remained well above the baseline value. Baseline CSA was 1.02±0.17 cm 2 in the conventional group and 1±0.23 cm 2 in the laparoscopic group. ISC decreased CSA to 0.91±0.18 cm 2 (conventional) and 0.85±0.18 cm 2 (laparoscopic) after initiation of ISC. CSA was 0.92±0.18 cm 2 after abdominal insufflation in the laparoscopic group, and it was 0.93±0.18 cm 2 after laparotomy in the conventional group (P=0.4). During surgery, there were no differences in absolute CSA or CSA changes compared with the baseline value in both groups. Postoperative circumference of the calf and thigh were not different between both groups. Postoperative thromboembolic complications did not occur. CONCLUSION: ISC effectively increases venous blood flow from the lower limbs during conventional and laparoscopic colorectal resections and may decrease the risk of postoperative deep vein thrombosis. Therefore, ISC is strongly recommended in every prolonged laparoscopic procedure.

The atypical segmental localization first led to the diagnosis of herpes zoster and an antiviral therapy. Regarding the unusual course of the complication following an intramuscular injection in this case, the question for the real pathomechanisms still remains.
    Notes: Zusammenfassung Wir berichten über den Fall eines 35jährigen Bürokaufmannes, der an einer ausgesprochen schmerzhaften Embolia cutis medicamentosa litt. Als Besonderheit ist die untypische segmentale Lokalisation im Versorgungsgebiet L1 zu werten, die zunächst zu der klinischen Diagnose eines nekrotisierenden Zosters und einer konservativen antiviralen Therapie führte sowie der ungewöhnliche Verlauf der subjektiven Beschwerden und das Intervall bis zum Auftreten der charakteristischen Hauterscheinungen, wodurch sich erneut die Frage nach den zugrundeliegenden pathogenetischen Vorgängen stellt.

    Notes: Abstract  The development of photoreceptors in the mammalian retina is thought to be controlled by extrinsic signals. We have shown previously that ciliary neurotrophic factor (CNTF) potently inhibits photoreceptor differentiation in cultures of rat retina. The present study analyzes which developmental processes are affected by CNTF. Rod differentiation as determined by opsin and recoverin immunocytochemistry was effectively blocked by CNTF and leukemia inhibitory factor, but not by other neurotrophic agents tested. CNTF did not influence proliferation, cell death, or survival, and had no effect on the downregulation of nestin immunoreactivity in progenitor cells. Opsin-positive rods could be reverted to an opsin-negative state initially, but became unresponsive to CNTF later. No compensatory increase in the number of other cell types was observed. Application of neutralizing antibodies against CNTF revealed that rod development was partially blocked by an endogenous CNTF-like molecule in control cultures. Our results suggest that CNTF can act as a specific negative regulator of rod differentiation. Its action on photoreceptor precursor cells could serve to synchronize the maturation of photoreceptors, which are born over an extended period of time. Together with other stimulatory signals, CNTF may thus control the temporally and numerically correct integration of photoreceptors into the retinal network.

    Description / Table of Contents: Zusammenfassung In Sera von Hepatitis C Patienten wurde die Höhe der Virämie mittels PCR sowie der Genotyp des infizierenden HCV bestimmt. Bei insgesamt 123 untersuchten Personen handelte es sich um 39 Dialysepatienten, 19 Hämophile, 13 Drogenabhängige sowie 52 weitere Patienten ohne einen dieser Risikofaktoren (chronische Hepatitis-Gruppe). In Österreich fanden wir den Genotyp 1b vorherrschend, gefolgt von 3a und 1a. Allerdings waren Infektionen mit 1b bei Hämophilen und Drogenabhängigen seltener als bei Dialysepatienten und der Patientengruppe mit chronischer Hepatitis. Hämophile wurden möglicherweise mit antihämophilem Plasma infiziert, dessen Spender aus Südamerika bzw. Asien stammten. Die höchsten Virusmengen im Blut fanden wir bei Personen mit Infektionen durch Genotyp 3a. Die Bestimmung der Virämiehöhe, sowie des infizierten Genotyps könnte für die Indikation einer Interferontherapie bei Hepatitis C von entscheidender Bedeutung sein.
    Notes: Summary Virus load was tested by means of PCR calibrated with standards and HCV genotypes were determined by the LIPA-technique using sera from 123 HCV patients. Of these 39 were on renal hemodialysis treatment, 19 suffered from hemophilia, 13 were i.v. drug users and the remaining 52 had none of these risk factors (chronic hepatitis group). The most prevalent subtype in Austria was 1b followed by 3a and 1a. However, genotype 1b infections were found relatively less often in hemophilia patients and drug users than in the other groups, indicating that hemophiliacs probably had been infected by an antihemophilic plasma coming from South American or Asian donors. The highest amounts of virus were found in patients infected with genotype 3a. Determination of the patient's virus load and of the infecting subtype of HCV may be helpful in planning interferon alpha therapy.

    Description / Table of Contents: Zusammenfassung Solange die Methoden zum serologischen Nachweis von IgG- und IgM-Antikörpern bei Lyme Borreliose nicht standardisiert sind, müssen die Ergebnisse mit Vorsicht interpretiert und stets im Zusammenhang mit der klinischen Symptomatik gesehen werden. Falsch negative Ergebnisse treten vor allem in den ersten Wochen nach Infektion auf. 50% der Patienten mit einem Erythema migrans, das kürzer als 4 Wochen besteht, haben keine nachweisbar erhöhten Borrelienantikörper. Die Entscheidung zur Therapie muß nach klinischen Kriterien gefällt werden. Häufig entwickeln sich IgM- und/oder IgG-Antikörper erst während der antibiotischen Therapie. Nach 4 Wochen sind bei 80% der Patienten erhöhte Antikörper mit den neuentwickelten ELISAs nachweisbar. Positive und grenzwertige Ergebnisse müssen mit dem Westernblot bestätigt werden. Falsch positive Ergebnisse, vor allem geringfügig erhöhte IgM Antikörper, können bei verschiedenen anderen Erkrankungen auftreten. Ein weiteres Problem nach Therapie einer Lyme Borreliose ist die lange Persistenz von borrelienspezifischen Antikörpern. Serologische Verlaufskontrollen können nur mit derselben Methode durchgeführt werden. Eine erneute antibiotische Behandlung ist zu erwägen, wenn IgM-Antikörper ansteigen und neue Symptome auftreten.
    Notes: Summary As long as test procedures are not standardized, the serological results of IgM-and IgG-antibodies in Lyme borreliosis must be interpreted with caution and always in the context of clinical signs and symptoms. False negative results occur primarily during the first weeks of infection. In erythema migrans of less than 4 weeks' duration, 50% of patients are seronegative even with newly designed ELISAs. At this early stage of the infection the therapeutic decision has to be established on the basis of clinical criteria. Frequently IgM-and/or IgG-antibodies develop during antibiotic therapy. After 4 weeks' duration 80% of patients have elevated borrelial antibodies detectable with recently developed ELISAs. Positive and borderline results should be confirmed by Western blot. False positive results, particularly slightly elevated IgM, may occur in a variety of other diseases. Another problem is the persistence ofBorrelia-specific IgM antibodies after therapy. Serological follow-up can only be carried out with the same methods in the same laboratory. Retreatment should be considered if IgM antibodies are increasing significantly and new symptoms are occurring.

    Notes: Abstract  An amphiphilic poly(acrylic acid)/polystyrene graft copolymer (PAA-g-PS) has been used to form “nanoreactors” for the synthesis of gold clusters. Such copolymers tend to form stable micelles in non-polar organic solvents where the poly(acrylic acid) chains constitute the core, and the polystyrene chains, the shell. In the present study, the micellar structure of PAA-g-PS in toluene has been demonstrated by dynamic light scattering and transmission electron microscopy (TEM). The subsequent preparation of gold-graft copolymer composites involved the introduction of gold chloride (AuCl3), either in powder form or previously dissolved in ether, into the micellar cores of the PAA-g-PS in toluene. The gold salt was then reduced by ultraviolet (UV) irradiation of the emulsion, or of dried cast films. TEM and ultraviolet-visible (UV/Vis) spectroscopy were used to characterize the resulting composites. Gold particles of less than 5 nm in diameter were observed in all cases, but the size distribution and the spatial arrangement of the clusters in the cast films were modified when diethyl ether was used to introduce AuCl3 into the PAA-g-PS micellar cores. This was thought to be due to enhanced nucleation of the gold particles and partial disruption of the micellar cores in the presence of diethyl ether.
