    Notes: Abstract The levels and cellular localization of mRNA for complement C1q and C3 were examined by RNA gel blot and nonradioactive in situ hybridization in the frontal cortex of patients with Alzheimer's disease (AD) and age-matched controls. We found that the hybridization signal for C1q mRNA was markedly increased (approx. 3.5-fold) in the frontal cortex of AD patients compared to that in age-matched controls. In contrast to previous reports we also found that the levels of C3 mRNA, although well expressed, did not differ significantly between AD cases and age-matched controls. Nonradioactive in situ hybridization using digoxigenin-labeled riboprobes revealed that transcripts coding for both C1q and C3 were closely associated with neurons. These results support the hypothesis that complement could play a role in neuronal degeneration which has been observed in the brain of AD patients.

    Notes: Abstract Tumors of the ovary in girls represent about 80% of pediatric genital tumors; approximately 30% of these tumors are malignant. The risk of malignancy increases with decreasing age. The most frequent finding is a teratoma; other tumors are rare. Small-cell carcinoma (SCCO) of the ovary is extremely rare, occurring mostly in young women. We present an 8-year-old girl with a SCCO of the hypercalcemic type. The findings and treatment are discussed with emphasis on the poor prognosis in these patients, even in stage 1 disease. The current literature is reviewed.

    Notes: Summary A positive result of re-operation in patients with recurrent symptoms after lumbar disc surgery is likely only if a new disc herniation is present. An improved ability to differentiate between recurrent disc herniation and scar tissue by contrast enhanced CT and MRI is suggested in earlier studies. In a prospective study 29 patients were selected for operation for suspected recurrent disc herniation. The inclusion of the patients was based on clinical symptoms and signs and myelography or non-enhanced CT. All patients were examined by CT and MRI both with and without intravenous contrast pre-operatively. The examinations were evaluated blind on a five point scale and statistical analysed by a regret function. Intravenous contrast improved the diagnostic power of both CT and MRI. MRI was superior to CT in both non-enhanced and enhanced examinations. MRI with intravenous contrast enhancement is proposed as the primary examination in patients with suspected recurrent disc herniation.

    Description / Table of Contents: Zusammenfassung Es wird über ein Fehlbildungssyndrom mit Hypo- und Aplasie zahlreicher Phalangen an Fingern und Zehen, Hypo- und Aplasie zahlreicher Interphalangalgelenke, Synostosierung im Hand- und Fußwurzelbereich und Dysplasie im Bereich der Ellenbogengelenke berichtet, das in einer Sippe in drei Generationen in konstanter Ausprägung beobachtet werden konnte. Besonderheiten im Papillarleistenmuster von Fingerbeeren, Handflächen, Zehenbeeren und Fußflächen werden den von Degenhardt u. Geipel bei einem ähnlichen Fehlbildungssyndrom diskutierten gegenübergestellt. Die Möglichkeit einer Zuordnung der hier beobachteten Sonderform zu bereits bekannten Syndromen und die Vorstellungen über die Erbgrundlagen dieser Fehlbildungen werden diskutiert.
    Notes: Abstract Report of a syndrome consisting of hypo- and aplasia of several phalanges of fingers and toes, hypo- and aplasia of interphalangeal joints, synostoses of carpal and tarsal bones, and dysplasia of the cubital joints. The anomalies could be observed through three generations with regular manifestation. Specific patterns of dermatoglyphics were examined and compared with those described by Degenhardt and Geipel in a similar malformation syndrome and with observations in children with exogenous skeletal defects. The relationship of the malformations seen in this family to other known syndromes and our knowledge of the inheritance of such malformations are discussed.

    Notes: Abstract Objective: To evaluate the influence of pentoxifylline (PTX), a phosphodiesterase inhibitor, on cytokines and inflammatory proteins in patients suffering from septic shock. Design: Prospective study comparing a therapy group to a matched control group. Setting: Medical intensive care unit at a university hospital. Patients: Twenty four patients fulfilling the criteria of septic shock were included in this study. Twelve patients received PTX (therapy group) and 12 patients matched for diagnosis, age and gender served as the control group. Interventions: Pentoxifylline at 1 mg/kg per hour over 24 h in the therapy group. Measurements ad results: Cytokine levels [tumor necrosis factor-α (TNF)], soluble TNF receptor [TNF-R], and interleukin-6 [IL-6] and inflammatory proteins [C-reactive protein, α-1-antitrypsin (AAT), fibronectin, and haptoglobin], as well as hemodynamic parameters and the APACHE III score were evaluated before initiation of therapy and 24 h later. After 24 h, TNF levels were significantly lower in the therapy group (p=0.013), while IL-6 levels were significantly higher in the therapy group (p=0.030). Within the 24 h TNF declined significantly in the therapy group (p=0.006), while IL-6 showed a significant increase (p=0.043). AAT and the APACHE III score tended to differ significantly after 24 h between the groups [AAT levels higher in the therapy group (p=0.05), APACHE III score lower (p=0.05)]. In the therapy group, the systemic vascular resistance index was significantly higher after 24 h (p=0.0026) whereas the cardiac index declined (p=0.035). Conclusions: PTX does influence TNF levels in septic shock patients. Nevertheless, inhibiting a single mediator in severe septic shock cannot stop the inflammatory overreaction.

    Notes: Zusammenfassung Bei dem beschriebenen Fall wurde durch die Kombination aus einem hereditären Protein C-Mangel und einer Heparin-induzierten Thrombozytopenie Typ II mit ungewöhnlichem Verlauf eine erhebliche Thrombophilie verursacht. Ebenso wie Heparin-assoziierte Thrombosen auch ohne Thrombozytopenie auftreten können, scheint ein Wiederanstieg der Thrombozyten trotz Fortsetzung der Heparintherapie bei persistierender Thrombophilie im Rahmen dieses Krankheitsbildes möglich zu sein.
