    Notes: Summary: This study was designed to investigate the role of vasopressin and angiotensin II in the pathogenesis of focal glomerulosclerosis (FGS). A non-peptide vasopressin VI antagonist (OPC-21268) and an angiotensin converting enzyme inhibitor (ACE-I) were administered either alone or in combination for 15 weeks to FGS, spontaneously hypercholesterolaemic rats. Treatment with the V1 antagonist (1% OPC-21268) suppressed the rise in systolic blood pressure (SBP), serum triglyceride (TG), blood urea nitrogen (BUN) and serum creatinine (S-Cr) levels, but not the elevations of urinary protein excretion (UPE) or serum total cholesterol (TC) levels. Morphologically, V1 antagonist significantly prevented an increase in the index of glomerular sclerosis (IS) and relative interstitial volume (RIV). In the low dose/high dose of V1 antagonist supplementation, the administration of 0.2% OPC-21268 failed to suppress any increase in the SBP and TG levels, but significantly preserved renal function and attenuated renal lesions. In the combination study, rats were divided into four groups: (i) V1 antagonist (1% OPC-21268); (ii) ACEI (imidapril, 5 mg/kg/per day); (iii) both treated groups; and (iv) an untreated control group. Angiotensin-converting enzyme inhibitor significantly suppressed increases in SBP, UPE, TC, BUN, and S-Cr levels compared with V1 antagonist. the combination therapy significantly enhanced these effects. Both agents significantly reduced IS and RIV, and combination therapy further reduced these levels. the results indicated that vasopressin, as well as angiotensin II, via the V1 receptor cause hypertension and renal injury in FGS rats. Vi antagonist and ACE-I have antihypertensive and renoprotective effects in this FGS model, and enhanced their beneficial effects when used as combination therapy.

    Notes: Summary: Lipoprotein glomerulopathy is a new type of glomerular disease which is thought to be induced by an abnormality of lipoprotein metabolism. Until the end of 1994, 21 cases had been reported from Japan. However, this disease is not restricted to Japan because two cases with similar features have been described in France and the United States recently. Clinically, the presenting feature in all patients is proteinuria, resulting in a nephrotic syndrome in the majority. Renal biopsy specimens reveal that capillary lumina in the glomerulus are markedly dilated with pale-stained and mesh-like substances which are composed of fine granules. Under electron microscopy, it is observed that a combination of granules forms strata as fingerprints. Sudan or oil red-O staining and immunofluorescence study on snap-frozen sections show lipid droplets and apolipoproteins (apos) B and E in the capillary lumina, respectively. Accordingly, the intracapillary substances are thought to be lipoprotein thrombi. Lipid and lipoprotein profiles show type III hyperlipoproteinaemia and high level of apo E in plasma. However, the apo E phenotype is usually the heterozygous E2/3 or E2/4, different from the homozygous E2/2 in familial type III hyperlipoproteinaemia. Systemic manifestations characteristic of the previously-reported lipidoses are not observed.Occurrence in several families and recurrence in two transplanted kidneys suggest that the disease may be induced by the hereditary abnormalities of lipoprotein metabolism. On the other hand, it is likely that in situ mechanisms may mediate development of the disease in the glomerulus.

    Notes: Abstract Transabdominal mucosal transection of the esophagus was applied to 20 patients with esophageal varices. The operative procedure involves a longitudinal incision of the muscular layers which follows transection and reanastomosis of the denuded mucosa. With the preservation the muscular structure, postoperative complications such as anastomotic leakage, stenosis or regurgitation, have been minimized. The effect of this procedure on the varices proved satisfactory.

    Notes: Summary Two cases of Noca dia asteroides infection were encountered out of 55 kidney transplant patients at Chiba University Hospital. One patient developed an extrapleural abscess and the other had a pulmonary infiltration with chest wall abscess. The patients were successfully treated by surgical drainage of the chest wall abscesses and by oral administration of minocycline. No adverse effects caused by minocycline were observed during the therapy. From 1900, when the first case of Nocardia infection was reported in Japan, there have been 60 cases reported in Japanese literature through 1973, including those we observed. This is the first report on nocardiosis in kidney transplant patients and on successful treatment of nocardiosis with minocycline in Japan.

    Notes: Abstract The expression of a new type of matrix metalloproteinase, membrane-type matrix metalloproteinase-1 (MT-MMP-1), was examined in 24 cases of primary pancreatic adenocarcinomas and 9 cases of secondary liver tumors derived from pancreatic adenocarcinomas, using a non-radioactive in situ hybridization and immunohistochemical methods. Out of 24 cases of primary pancreatic adenocarcinomas, 18 showed positive expression of MT-MMP-1 transcripts in cancer cells and 20 of 24 showed positive expression in the tumor stromal cells. The immunoreactivity of the gene products for MT-MMP-1 was demonstrated to be almost the same, as shown by in situ hybridization in these 24 cases. In particular, both the staining intensity for MT-MMP-1 transcripts and the immunoreactivity of the gene products in the tumor stromal cells of mucinous cystadenocarcinomas were significantly weaker than those of common-type ductal adenocarcinomas among the 24 cases. All of the 9 cases of secondary liver tumors derived from pancreatic adenocarcinomas showed positive expression for MT-MMP-1 transcripts but less immunoreactivity for the gene products. These results suggest that MT-MMP-1 is transcribed and translated in both cancer cells and the tumor stromal cells in human pancreatic adenocarcinomas. Furthermore, considering that common-type ductal adenocarcinoma of the pancreas usually shows a strong desmoplastic reaction, while mucinous cystadenocarcinoma typically does not, MT-MMP-1 expressed in the tumor stromal cells of common-type adenocarcinomas may be involved in processes leading to the desmoplastic reaction.

    Notes: Abstract Onshore tsunami deposits resulting from the 1993 Southwest Hokkaido and 1983 Japan Sea earthquakes were described to evaluate the feasibility of tsunami deposits for inferring paleoseismic events along submarine faults. Tsunami deposits were divided into three types, based on their composition and aerial distribution: (A) deposits consisting only of floating materials, (B) locally distributed siliclastic deposits, and (C) widespread siliclastic deposits. The most widely distributed tsunami deposits consist of the first two types. Type C deposits are mostly limited to areas where the higher tsunami runup was observed. The scale of tsunami represented by vertical tsunami runup is an important factor controlling the volume of tsunami deposits. The thickest deposits, about 10 cm, occur behind coastal dunes. To produce thick siliclastic tsunami deposits, a suitable source area, such as sand bar or dune, must be available in addition to sufficient vertical tsunami runup. Estimation of the amounts of erosion and deposition indicates that tsunami deposits were derived from both onshore and shoreface regions. The composition and grain size of the tsunami deposits strongly reflect the nature of the sedimentary materials of their source area. Sedimentary structures of the tsunami deposits suggest both low and high flow régimes. Consequently, it seems very difficult to identify tsunami deposits based only on grain size distribution or sedimentary structure of a single site in ancient successions.
