    Notes: Abstract Many pathogens must surmount an epithelial cell barrier in order to establish an infection. While much has been learned about the interaction of bacterial pathogens with cultured epithelial cells, the influence of cell polarity on these events has only recently been appreciated. This review outlines bacterial-host epithelial cell interactions in the context of the distinct apical and basolateral surfaces of the polarized epithelium that lines the lumens of our organs.

    Description / Table of Contents: Abstract Introduction: The influence of kolloids on the immune system is not well documented. In this study we investigated the effects of gelatine, hydroxyethylstarch (HES), human albumine, and dextrane on neutrophil function and receptor expression by flow cytometry. Methods: Whole blood of healthy volunteers was incubated for 30 minutes with either gelatine, HES (6% and 10%), dextrane 40 and 60, or human albumin 20%. Phagocytic capacity was determined by uptake of fluorescein-isothiocyanate labeled bacteria, the conversion of dihydrorhodamine 123 into fluorescent rhodamine 123 was used for oxidative burst measurements. Expression of complement receptors CD 11b and CD35 was investigated using fluorescein-isothiocyanate labeled antibodies. Results: Incubation with gelatine significantly increased expression of complement receptors and oxidative burst. Dextranes and HES had no influence on neutrophil function. Human albumin reduced the oxidative burst, whereas CD 35 expression was increased. Conclusion: The physiological significance of these changes in a range of 10% has to be clarified in further investigations.
    Notes: Zusammenfassung Fragestellung: Der Einfluss kolloidaler Volumenersatzmittel auf die Immunfunktion ist unzureichend belegt. In dieser Studie wurden die Auswirkungen von Gelatinelösungen, Hydroxyäthylstärke (HAES), Humanalbumin und Dextranen auf Expression von Oberflächenrezeptoren und Funktion polymorphkerniger neutrophiler Granulozyten durchflusszytometrisch untersucht. Methodik: Vollblut von gesunden Probanden wurde 30 min mit folgenden Volumenersatzmitteln inkubiert: HAES (6% und 10%), Dextran 40, Dextran 60, Gelatinelösung (3%), Humanalbumin 20%. Die Phagozytoseleistung wurde mittels Fluoreszeinisothiozyanat-markierten Bakterien ermittelt. Der Oxidative Burst wurde anhand der Umwandlung von Dihydrorhodamin 123 in grün-fluoreszierendes Rhodamin 123 bestimmt. Die Expression der Komplementrezeptoren CD11b und CD 35 wurde mit Fluoreszeinisothiozyanat-markierten Antikörpern untersucht. Ergebnisse: Nach Inkubation mit Gelatinelösung waren Expression von Komplementrezeptoren und Oxidativer Burst signifikant erhöht. Der Median der Fluoreszenzintensität stieg für CD35 von 531±42 auf 565±51, für CD 11b von 416±24 auf 446±19 und für den Oxidativen Burst von 490±52 auf 516±51. HAES und Dextrane beeinflussten die Granulozytenfunktion nicht. Humanalbumin 20% steigerte die Expression von CD 35, reduzierte jedoch den Oxidativen Burst. Schlussfolgerung: Die klinische Bedeutung dieser in vitro-Veränderungen in einer Größenordnung von maximal 10% des Ausgangswerts sollte in in vivo-Studien näher untersucht werden.

    Notes: Abstract  Rationale: Prepulse inhibition of acoustic startle is the reduction in startle response to an intense auditory stimulus when this stimulus is immediately preceded by a weaker prestimulus. Prepulse inhibition occurs normally in humans and experimental animals, but schizophrenic persons often exhibit a marked impairment in this measure. Previous studies have shown that dopamine (DA)-dependent neuronal mechanisms are involved in the modulation of prepulse inhibition. Objective: Experiments were conducted in rats to elucidate further the involvement of DA-ergic mechanisms in prepulse inhibition. Results: In line with previous studies, the indirect DA agonist, amphetamine, was shown to decrease prepulse inhibition. A close reverse relationship over time between DA overflow in the nucleus accumbens and prepulse inhibition was obtained using a technique allowing concomitant measurement of these parameters in awake, freely moving rats. This effect was more pronounced in amphetamine-treated rats compared to rats treated with equimolar doses of cocaine, which increased DA overflow without affecting prepulse inhibition. In other experiments, the combined treatment with subthreshold doses of the selective DA D1 agonist, SKF 38393, and the selective DA D2 agonist, quinpirole, was also shown to decrease prepulse inhibition. Finally, the selective DA D2 antagonist, raclopride, was shown to enhance prepulse inhibition. Conclusions: In line with previous studies, it is concluded that DA neurotransmission is involved in the modulation of prepulse inhibition and that the ventral part of the mesostriatal DA system may serve an important role in this modulation. Furthermore, the possibility is discussed that the discrepant results on prepulse inhibition obtained with amphetamine and cocaine may disclose functionally relevant differences in their mechanisms of action, and that the enhancement of prepulse inhibition induced by some antipsychotics in rats may reflect their propensity to induce adverse mental effects in humans.

    Description / Table of Contents: Summary In Germany there is still an urgent need for action in regard to screening for breast cancer. The announcement of research projects for mammography screening proves this. The following reports the results of the Munich field study. In 2 years, 2489 breast cancer cases were registered (status 6/98). Screening data were available for 1319 patients. The reduction in mortality, found in randomized studies, due to mammography in women over 50 years old was confirmed. A relative reduction of 44.8% in 10-year mortality could be estimated through mammography and 25.1% through palpation, in comparison with ”doing nothing”. We could also estimate the participation of screening and the used methods in the Munich region. If the mammographies carried out today were used at 2-year intervals for women aged between 50 and 70 years, then the mammography screening could be performed with no additional costs for 70% of the women. The known facts regarding the population-based mortality and regarding the acceptance of the palpation screening and frequency of mammography are additional aspects for inclusion in the discussion of what research projects in Germany are and what they should fulfill.
    Notes: Zusammenfassung Hintergrund: Bei der Früherkennung des Mammakarzinoms besteht in Deutschland nach wie vor ein dringender Handlungsbedarf. Die Ausschreibung von Modellprojekten zur Einführung des Mammographiescreenings belegt dies. Im folgenden wird – als Diskussionsgrundlage –über erste Ergebnisse der Feldstudie München berichtet. Ergebnisse: In einem Zeitraum von 2 Jahren wurden 2489 Mammakarzinome registriert (Stand Juni 1998). Zu 1319 Patientinnen sind Angaben zur Früherkennung verfügbar. Die durch randomisierte Studien gesicherte Senkung der Mortalität durch die Mammographie im Alter 〉50 Jahre konnte bestätigt werden. Aufgrund der dokumentierten Stadienverteilung beträgt die Schätzung für die relative Senkung der 10-Jahres-Mortalität durch die Mammographie 44,8% und durch die Palpation 25,1% im Vergleich zum „Nichtstun”. Auch die Inanspruchnahme der Früherkennung und die in der Region München eingesetzten Methoden konnten abgeschätzt werden. Schlussfolgerungen: Würden die heute durchgeführten Mammographien bei Frauen im Alter zwischen 50 und 70 Jahren mit einem Abstand von 2 Jahren eingesetzt, so könnte das Mammographiescreening bei etwa 82% der Frauen kostenneutral durchgeführt werden. Die bekannten Fakten zur Mortalität und zur bundesweiten Inanspruchnahme des heutigen Früherkennungsangebots sind weitere Grundlagen für die Diskussion, was Modellprojekte in Deutschland sein und was sie wann leisten sollten.

    Notes: Abstract To determine the value of different otoscopic findings in diagnosing otitis media with effusion, 3,780 otological examinations of 250 infants at ages 0–2 years were compared with tympanometric findings. Otoscopy was performed by an experienced otologist and research-audiologist prior to tympanometry. The following success rates for these procedures were found: otoscopy, 85–91%, tympanometry, 74–94%, either otoscopy or tympanometry, 89– 99%. “Glue” was noted as the most frequent abnormal otoscopic feature, i.e., depending on age, 6–32%. Fluid line/ bubbles, bulging/hyperaemia or otorrhoea were rarely (0– 5%) observed and contributed little to the diagnosis of otitis media with effusion. No structural changes of the tympanic membrane were observed. Analysis of tympanometric data revealed that the prevalence of tympanogram types B (21–44%), C1 (0.5–16%) and C2 (0–8%) was age-dependent. Comparison of otoscopic and tympanometric findings indicated sufficient diagnostic agreement (Cohen’s kappa between 0.41 and 0.74) at age 6–24 months. In conclusion, otoscopy appears to be a valid instrument for diagnosing otitis media with effusion, at least from 6 to 24 months of age. In addition, the results of our study suggest that tuba-dysfunction and otitis media with effusion are age-dependent disorders during infancy, without structural effects on the tympanic membrane.
