    Notes: In 1990 we reported the trends in presentation of malignant melanoma (MM) from 1986 to 1988 in a defined population in South Wales. Ten years after this initial study we reassessed the incidence of invasive MM in this defined population over a similar 3 year period. Using the local skin cancer registry we identified 186 cases of MM representing a 74% increase. Age standardization to the world population gives an overall incidence of 11.8 per 100 000 population per annum. This is the highest published standard incidence of MM to date from the UK and certainly one of the highest in Europe. We also have observed a continuing trend for diagnosis of thinner lesions with a better prognosis. Information about mortality for the whole of Wales over the corresponding period is also presented.

    Notes: Cancer registries are used to compare incidences between regions, plan for service provision, and to assess the impact of health interventions. Significant variation in data capture for skin cancers and reporting of results was evident between regional cancer registries in the UK when assessed in 1991. Using a postal questionnaire we sought to document methods of recording skin cancer incidence in the UK in 2000, and to assess if practice has changed from 1991. All UK cancer registries were asked for details of their method of skin cancer case registration and latest available incidence figures. Methodology was assessed against recently implemented national standards. All registries responded to the survey. Sources of data were more uniform than was the case 9 years ago. All registries except one attained national standards for basal cell carcinoma data collection, but only half of the registries attained standards for squamous cell carcinoma. Ten of the 12 correctly recorded numbers of malignant melanomas, but three still failed to record the Breslow thickness or Clarke's level. Wide variation is evident in the recorded incidences for each of the malignancies, and the efficiency at which figures are made available. Thus, although there has been improvement since 1991, variability still exists between UK registries in methods of data capture, the data recorded, and efficiency of data processing in skin cancer registration.

    Notes: The biological activity of skin-sensitizing chemicals is related to their ability to react, either directly or after metabolic activation, with appropriate skin proteins. For direct acting electrophilic compounds, this ability can be modelled, using the RAI (relative alkylation index) approach, by a combination of electrophilicity and hydrophobicity parameters. The development of predictive quantitative structure-activity relationships (QSAR) models of skin sensitization, using mechanism-based physicochemical parameters, has been greatly facilitated by the introduction of the murine local lymph node assay (LLNA), which is able to describe the extent of the biological response in objective and quantitative terms. In the present work, sensitization response data in the LLNA is generated for a series of 6 sulfonate esters. An RAI-based hybrid QSAR/dose-response relationship is derived using a negative hydrophobicity coefficient in the RAI expression, to model the effect of retention of the hydrophobic test chemicals in the stratum corneum. Dose-response analyses are used to estimate EC3 and EC20 values as quantitative indices of skin sensitization potential for each compound, and regression analysis is applied to develop QSARs correlating these EC3 and EC20 values with an RAI-based parameter. The high statistical quality of these QSARs demonstrates both the consistency of the LLNA method for generating high quality skin sensitization data, and the value of the RAI approach in development of mathematical models for skin sensitization.

    Notes: In one experiment twenty-four Holstein Friesian cows, average 43 d post-partum, were used in a changeover design experiment to evaluate the replacement of a cereal-based concentrate supplement (C) by an ensiled mixture (MGBP) of malt distillers’ grains and molassed sugar beet pellets. The cows were offered grass silage ad libitum [dry matter (DM) content 170 g kg−1, crude protein (CP) concentration 160 g kg DM−1, metabolizable energy (ME) concentration 10·9 MJ kg DM−1] and either C or MGBP at one of three levels (3, 6, 9 kg DM d−1). The composition of C and MGBP were DM content: 853 and 296 g kg−1, CP concentration: 202 and 187 g kg DM−1, ME concentration: 12·6 and 10·8 MJ kg DM−1 respectively. The cows ate all the C supplement but the intakes of MGBP were 2·7, 4·9 and 6·4 kg DM d−1 for the 3, 6 and 9 kg DM d−1 levels of MGBP respectively. Total DM intakes (kg d−1) were 12·5, 15·6, 18·2 for treatments 3-C, 6-C and 9-C and 13·1, 14·4 and 15·9 (s.e., 0·90) for treatments 3-MGBP, 6-MGBP and 9-MGBP respectively. Milk yields (kg d−1) for treatments 3-C, 6-C and 9-C were 19·9, 23·2 and 24·2, respectively, and for treatments 3-MGBP, 6-MGBP and 9-MGBP were, 20·3, 21·3 and 23·0 respectively (s.e., 1·05). Milk fat contents (g kg−1) for treatments 3-C, 6-C and 9-C were 42·8, 42·3, 43·5 respectively and for treatments 3-MGBP, 6-MGBP and 9-MGBP were 39·5, 38·7 and 38·2 (s.e, 1·86), respectively, and milk protein contents (g kg−1) for treatments 3-C, 6-C and 9-C were 30·5, 30·6, 31·8, respectively, and for 3-MGBP, 6-MGBP and 9-MGBP were 30·0, 30·8 and 31·2 (s.e., 0·66) respectively. Milk yield and milk protein contents were significantly higher for the higher levels of supplementary feeding but there was no difference between the types of supplement. The milk fat contents were significantly lower on the MGBP than C supplements.In a second experiment fifteen Holstein Friesian cows, average 126 d post-partum, were used in a changeover experiment to evaluate the replacement of all (treatment M) or half (treatment MS) of the grass silage (S) in their diet by a mixture of MGBP and straw. All cows received 5·1 kg DM d−1 of concentrate feed. Forage DM intakes were 8·3, 11·2 and 14·2 kg DM d−1 for the S, MS and M treatments respectively. Milk yields (kg d−1) for S, MS and M treatments were 17·0, 19·4 and 20·0 (s.e., 0·56) respectively. Corresponding contents of milk fat and protein (g kg−1) were 42·0, 41·4, 38·6 (s.e., 0·37) and 33·8, 34·1, 34·2 (s.e., 0·42).Ensiled mixtures of malt distillers’ grains and molassed sugar beet pellets can be used to replace some of the conventional concentrates or grass silage for dairy cows giving moderate yields without a loss of production.

    Notes: The paper presents an ecological risk-assessment methodology which (a) addresses surface-water exposures to aquatic mammals and birds, and (b) accounts for long-term accumulation and biomagnification of pollutants. The methodology has been adapted from those developed in the USA for compliance with US Environmental Protection Agency requirements, and combines predicted or measured surface-water concentration data with information on the natural history of wildlife species and chemical-specific bioaccumulation factors, to determine whether estimated exposures would exceed toxicological criteria. Whilst the methodology is applied to metals in leachate discharges from landfill sites, the general approach is applicable to other industrial sites. The approach could provide the basis for a shift towards more numerical risk-based ecological assessments in a wide variety of contexts.

    Notes: Abstract  Cerebral cavernous malformations (CM) are well-circumscribed vascular malformations that often present with epileptic seizures. Although patients may initially benefit from antiepileptic drugs, surgical treatment may become necessary due to medically intractable seizures. However, it is unclear whether lesionectomy alone or tailored epilepsy surgery with previous invasive monitoring is the optimal strategy in such cases. We report two patients with epileptic seizures due to CM. One patient with few seizures prior to surgery became seizure-free following resection of the CM and the surrounding tissue. In the second patient with long-lasting epilepsy, lesionectomy was performed because of the proximity to a functioning left hippocampus. This limited resection failed and the patient still had seizures. Subsequently, invasive monitoring with intracranial depth and strip electrodes was performed in order to localize the epileptogenic area and determine whether the left hippocampus could be spared. The invasive study showed the seizure origin in the tissue around the former CM but no epileptic discharges in the hippocampus. In a second operation, an anterior temporal resection was performed with removal of the epileptogenic surrounding tissue and the patient became seizure-free without cognitive deficits. The optimal surgical strategy for CM presenting with epileptic seizures must take into account various factors such as underlying mechanisms and duration of epilepsy, and location of the lesion.

    Notes: Abstract  Rationale: Recent reports have indicated that the γ-aminobutyric acid (GABA)B agonist baclofen attenuates the reinforcing effects of cocaine. Objectives: To further evaluate the effect of baclofen on cocaine self-administration under a fixed ratio (FR) and progressive ratio (PR) schedule of reinforcement. Methods: In the first series of experiments, three dose–response curves were generated that examined the effect of three doses of baclofen (1.8, 3.2, or 5.6 mg/kg, i.p.) against four unit-injection doses of cocaine (0.19, 0.38, 0.75, and 1.5 mg/kg per injection) reinforced under a FR1 schedule. For comparison, an additional group of rats was pretreated with haloperidol (32, 56, or 100 µg/kg, i.p.). A separate experiment examined the effect of baclofen (1.8, 3.2, or 5.6 mg/kg, i.p.) on responding for concurrently available cocaine or food reinforcement. Results: Under the FR1 schedule, baclofen suppressed intake of low but not high unit injection doses of cocaine. In contrast to haloperidol, baclofen had no effect on the distribution of inter-injection intervals and, instead, produced long pauses in cocaine self-administration. Baclofen dose dependently reduced cocaine- reinforced responding on a PR schedule; concurrent access to a food-reinforced lever demonstrated that the animals retained the capacity to respond at high rates. Conclusion: The effect of baclofen pretreatment on cocaine self-administration is dependent on the unit injection dose of cocaine and on the response requirements of the schedule.
