    Notes: Rosacea fulminans is a rare disease with female predominance characterized by abrupt onset of pustules, papules, and confluent nodules on the face. The conventional treatment consists of systemic glucocorticoids and isotretinoin. We present the case of a 56-year-old woman with a marked facial papulopustular eruption that had followed an initial period of severe seborrhoea. Conventional treatment produced no clear improvement. Dapsone treatment achieved complete healing in 5 weeks.

    Notes: Background Granulomatous mycosis fungoides is an extremely rare type of cutaneous T-cell lymphoma. Two cases are described and checked for clinical, histological and therapeutic differences to other variants of mycosis fungoides.Case reports Case 1: a 52-year-old patient with livid plaques covering the whole integument. Histological findings showed granulomas with multinuclear giant cells in addition to a malignant lymphohistiocytic infiltrate (monoclonal T-cell receptor (TCR) -γ rearrangement). Despite various chemotherapeutic regimens, progression to tumour stage was observed. Case 2: an 88-year-old man with plaques and ulcerating tumours on the trunk and head. Histological findings showed malignant T-lymphocyte infiltrate (monoclonal TCR-γ rearrangement) and granulomas with multinuclear foreign-body giant cells. Complete regression of all lesions was achieved using both local psoralen-ultraviolet A and electron radiotherapy.Conclusions The diagnosis of a granulomatous mycosis fungoides depends exclusively on the histological demonstration of granulomas. Distinct clinical characteristics are not present. Apart from granuloma formation, no other noticeable histological features are evident. The presence of granulomas in mycosis fungoides does not have prognostic implications, as cases with aggressive, but also with a prolonged course have been described.

    Notes: Abstract The biosynthesis of one riboflavin molecule requires one molecule of GTP and two molecules of ribulose 5-phosphate as substrates. The imidazole ring of GTP is hydrolytically opened, yielding a 4,5-diaminopyrimidine which is converted to 5-amino-6-ribitylamino-2,4(1H,3H)-pyrimidinedione by a sequence of deamination, side chain reduction and dephosphorylation. Condensation of 5-amino-6-ribitylamino-2,4(1H,3H)-pyrimidinedione with 3,4-dihydroxy-2-butanone 4-phosphate obtained from ribulose 5-phosphate affords 6,7-dimethyl-8-ribityllumazine. Dismutation of the lumazine derivative yields riboflavin and 5-amino-6-ribitylamino-2,4(1H,3H)-pyrimidinedione, which is recycled in the biosynthetic pathway. The structure of the biosynthetic enzyme, 6,7-dimethyl-8-ribityllumazine synthase, has been studied in considerable detail.

    Notes: The continuity and momentum equations do not imply a Reynolds number dependence of turbulence data when wall variables are used for normalization. However, experimental and numerical results show a Reynolds number dependence of turbulence intensity very close to the wall. The cause of this is explained. It results from the behavior of a sink term in the dissipation rate equation which shows a Reynolds number dependence in the limit of two-component two-dimensional turbulence as it exists close to walls. Away from the near-wall region the Reynolds number dependence originates from the streamwise pressure gradient which enters into the equations for the turbulent kinetic energy and turbulent dissipation rate through the gradient production processes. The low-Reynolds number effects in turbulent channel flow were investigated experimentally using the laser Doppler anemometry (LDA) measuring technique. A new method was used to eliminate the influence of the limited spatial resolution of the LDA measuring control volume. Results are presented for the limiting behavior of the turbulent intensity near the wall and its variation with the Reynolds number. The present LDA measurements confirm the trend in the data of direct numerical simulations. © 2001 American Institute of Physics.

    Notes: Ionotropic glutamate receptors (ligand-gated, ion-channel proteins) of the N-methyl-d-aspartate (NMDA) receptor type could enable a transmembranous calcium influx from the extracellular space. Though ionotropic glutamate receptors are predominantly neuronal receptors, they are also expressed in non-neuronal tissues like keratinocytes. Therefore, investigations were performed to study the function of NMDA receptors in HaCaT cells. The intracellular calcium concentration of HaCaT cells was studied under the influence of the selective receptor agonist NMDA and the selective NMDA antagonist MK-801. The proliferation of HaCaT cells was investigated using the crystal-violet method. Furthermore, the expression of Cytokeratin 10 and Filaggrin was examined in HaCaT cells after blocking NMDA receptors with MK-801. Using NMDA, there was a significant increase in the number of HaCaT cells showing elevated intracellular calcium concentration, at a dose between 25 µm and 1 mm (up to 84.6% of cells). The NMDA-associated calcium influx could be significantly suppressed by prior application of MK-801. There was no influence of NMDA on the proliferation of HaCaT cells. There was also no cytotoxic effect of NMDA (up to 1 mm). The expression of Cytokeratin 10 and Filaggrin could be suppressed by blocking NMDA receptors with MK-801. The investigations show that glutamate receptors of the NMDA-type play a role in the differentiation of HaCaT cells by regulating their intracellular calcium concentration.

    Notes: Background Sneddon's syndrome is defined as a combination of idiopathic livedo racemosa generalisata and symptoms of cerebrovascular defect. The disease usually starts with vascular symptoms in the epidermis, with neurological deficits becoming evident later. For this reason, histological examination of skin biopsies and determination of arteriolar occlusion is of particular importance for reliable categorization and early diagnosis. To date, these methods have been considered to be too insensitive. Objectives To evaluate the sensitivity of skin biopsies in Sneddon's syndrome. Methods We took a total of five deep punch biopsies (4 mm) from different areas of the livedo (three from white and two from red areas) in 15 patients. Present knowledge of the pathogenic relationships and the particular anatomical features of the skin were taken into account. Results The method had a sensitivity of 27% with one biopsy, 53% with two biopsies and 80% with three biopsies taken from white areas in all cases. Conclusions Skin biopsies using the method presented achieved a high sensitivity, suggesting that the diagnosis in clinically suspected cases could be confirmed in the majority of cases with this technique.
