    Notes: Abstract Pure autonomic failure (PAF; also known as idiopathic orthostatic hypotension or Bradbury-Eggleston syndrome) is an uncommon sporadic disorder, characterized by autonomic failure without other neurological deficits and histopathologically by cell loss in intermediolateral columns and sympathetic ganglia. Few postmortem studies of patients with PAF have been reported in the literature, and none have demonstrated Lewy bodies in distal axons, although this has been described as a feature in Parkinson’s disease with autonomic failure. We report a patient with PAF who had orthostatic hypotension and urinary symptoms for 15 years prior to death at the age of 63 years. Postmortem findings included typical and atypical Lewy bodies in the substantia nigra, locus ceruleus, substantia innominata, and sympathetic ganglia, as well as in autonomic axons in the epicardial fat, autonomic nerve fascicles in periadrenal adipose tissue, and autonomic nerves in the muscularis of the urinary bladder. Sites of autonomic nerve involvement correlated with clinical symptomatology, and thus were a valuable observation in the complete autopsy. Systemic autopsy results should be reviewed carefully in patients with PAF, as Lewy bodies in this disease may be seen in distal axons at a great length from their primary cell bodies.

    Notes: Abstract From April 1992 to May 1994, 780 patients aged from 1 day to 8 years were examined. Sedation of these patients was conducted by giving chlorprothixene orally and, in some cases, chloral hydrate had to be added. The patients were monitored with a pulse oxymeter. Investigations could begin after 50–120 min. In 710 patients (91 %) the first attempt to perform the examination was successful; 70 patients required one or two further attempts. Only two of the 780 patients (0.5 %) showed evidence of respiratory depression. The total number of pediatric MRI examinations performed in 1 year is almost 1000. In the hands of an experienced pediatric radiologist these examinations can be performed entirely without anesthesia.

    Notes: Abstract  · Objective: Lea symbols can be used for measuring visual acuity in childhood. Therefore, these symbols might be useful for early detection of amblyopia. We evaluated whether the visual acuity determined with Lea symbols (LS) corresponds to the visual acuity determined with the Landolt-C (LC). · Patients and methods: In 55 strabismic amblyopic volunteers aged 5–59 years, the monocular visual acuity of both eyes was determined using LC and LS. For comparison, the right eye of 20 healthy volunteers was examined. Single optotypes (LC, LS) were used in 55 amblyopes and crowded optotypes (LC17.2, LC2.6, CLS) in 40 amblyopes. The luminance of the test charts was 180–200 cd/m2, with a contrast 〉85%. The refraction of the subjects was corrected beforehand. · Results: In the 40 amblyopic eyes tested under each condition, LS exceeded CLS and LC by about 1 line (dB), LC17.2 by 2 lines and LC2.6 by 3 lines (mean values ± SD: LS 0.62±1.8 dB, CLS 0.46±1.7 dB, LC 0.5±2.0 dB, LC17.2 0.41±2.3 dB, LC2.6 0.29±2.3 dB). The non-amblyopic fellow eyes and healthy eyes showed smaller differences (fellow eyes LS 1.32±1.1 dB, CLS 1.17±1.1 dB, LC 1.15±0.9 dB, LC17.2 1.05±0.9 dB, LC2.6 0.93±1.1 dB; healthy eyes LS 1.74±0.9 dB, CLS 1.58±0.8 dB, LC 1.48±0.6 dB, LC17.2 1.41±0.7 dB, LC2.6 1.32±1.1 dB). In the amblyopic eyes, the reduction of LC was more distinct than the reduction of LS. Fifty-two amblyopes had an amblyopia 〉1 dB of LC, LC17.2 and LC2.6, while 50 had an interocular acuity difference 〉1 dB CLS. · Conclusions: Using Lea symbols, a recognition acuity can be determined and amblyopia can reliably be detected. Due to their design, the Lea symbols are particularly suitable and recommendable for application in young children. However a slight systematic difference between LS and LC has to be considered.

    Notes: Abstract Observed optical reflectivity in the infrared spectral region is compared with theoretical predictions in a strongly coupled electron–phonon system. Starting from a Fröhlich Hamiltonian, the spectral functions and their temperature dependence are derived. A full analysis including vertex corrections leads to an expression for the optical conductivity σ(ω) that can be formulated in terms of the well-known optical conductivity for a quasi-isotropic system without vertex corrections. A numerical comparison between the full result and the so-called extended Drude formula, its weak coupling expansion, shows little difference over a wide range of coupling constants. Normal-state optical spectra for the high-T c superconductors YBa2Cu3O7 and La2 − x Sr x CuO4 at optimal doping are compared with the results of model calculations. Taking the plasma frequency and ∈∞ from band structure calculations, the model has only one free parameter, the electron–phonon coupling constant λ. In both materials the overall behavior of the reflectivity can be well accounted for over a wide frequency range. Systematic differences exist only in the mid-infrared region. They become more pronounced with increasing frequency, which indicates that a detailed model for the optical response should include temperature-dependent mid-infrared bands.

    Notes: Abstract Some YBCO ceramics have been found to exhibit hysteresis of the current/voltage, I/V, characteristics, which reveals at a certain current If greater than critical current Ic a transition with switch from one branch of the I/V curve to another. In present work, the influence of an electric field at 77 K on hysteresis of I/V curves of ceramic YBCO/Ag and YBCO after hydrogen treatment has been studied. It was established that in strong electric field (120 MV/m) the conductivity increases at current I〉Ic, the field effect being observed only in the upper branch of hysteresis loop. The electric field effect in YBCO after hydrogen treatment is reversible. The observed effects are assumed to be associated with a change of grain boundary weak links from S/I/S-to S/N/S-type in the course of hydrogen treatment or addition of silver into ceramics.

    Notes: Abstract Anemia is a common complication of autonomic failure and reduced red blood cell mass may contribute to the orthostatic hypotension of these patients. We investigated whether treatment with recombinant erythropoietin improves anemia and increases blood pressure in patients with primary autonomic failure. Three patients with multiple system atrophy and autonomic failure and one with pure autonomic failure were studied. All patients had normocytic normochromic anemia and low (n = 2) or normal (n = 2) serum levels of erythropoietin. Treatment with erythropoietin, 4000 U subcutaneously biweekly for 6 weeks, increased hematocrit and blood pressure in all patients. Hematocrit increased from 33.9 ± 0.7 to 44.3 ± 1.4%, blood pressure in supine position increased from 150 ± 8/87 ± 8 (systolic/diastolic; mean ± SD) to 166 ± 25/92 ± 12 mmHg, and after 3 min in the head-up tilt position from 86 ± 21/47 ± 15 to 102 ± 23/63 ± 12 mmHg, (p 〈 0.05). All patients reported improvement in orthostatic symptoms and increased tolerance to standing. The study shows that treatment with erythropoietin improves anemia, increases blood pressure and ameliorates orthostatic hypotension in patients with primary autonomic failure.

    Notes: Abstract Repetitive DNA sequences present in the genome of Dicrocoelium dendriticum were identified by hybridization of genomic DNA that had been digested with different restriction enzymes with 32P-labeled genomic D. dendriticum DNA. DNA fragments containing repetitive sequences were isolated from PstI-digested D.␣dendriticum DNA and were subcloned into a plasmid vector. Plasmids containing repetitive sequences were identified by colony hybridization. One of these plasmids, designated Ddr-IV, was isolated and used as a probe in further studies. Ddr-IV is specific for D. dendriticum since it does not hybridize to DNA isolated from other trematodes. In addition, Ddr-IV was capable of detecting D. dendriticum metacercariae in ants (Formica cunicularia, F. rufibarbis, and Lasius sp.), which act as second intermediate hosts in the parasite's life cycle. Since metacercariae constitute the infectious stage of the parasite for grazing animals, Ddr-IV will provide a useful tool for epidemiology studies of dicrocoeliosis.

    Description / Table of Contents: Abstract Background. It has long been a goal to develop a means to measure squint angles in three dimensions simultaneously. A three-dimensional binocular method is naturally needed especially with surgical procedures that affect three dimensions of ocular motility. Methods. Special modifications of a videographic method provide simultaneous online data on the three-dimensional position of the eyes. The interocular difference is equal to the amount of squint, depending on the direction of gaze, head-tilt/direction of gravity, and convergence. Results. The method introduced provides three-dimensional and binocular documentation of ocular movements. This could lead to improve pre- and postsurgical examination, especially of rotational disturbances, and it could also offer an objective means for comparing the results of various surgical procedures.
    Notes: Zusammenfassung Hintergrund. Die dreidimensionale Videookulographie wird für den strabologischen Bereich modifiziert und als binokulare Methode vorgestellt. Methode/Ergebnisse. Durch die Videoanalyse eines Pupullenbildes kann die horizontale und vertikale Position des Auges bestimmt werden. Rotatorische Bewegungen um die Blicklinie werden mit einem speziellen Überlagerungsverfahren von Irismerkmalen gemessen. Neue Prozessoren ermöglichen eine alternierende Positionsbestimmung mit 25 Hz und ebenso schneller Berechnung der interokulären Positionsdifferenz. Das Bezugssystem in Form einer Art Taucherbrille mit Videokameras ist an den Kopf des Probanden gebunden. Semitransparente Spiegel erlauben u. a. 9 Blickpositionen analog zur Tangententafel. Die freie Beweglichkeit des Kopfes dient zur Messung von Innenohreinflüssen bei okulärer Gegenrollung. Schlussfolgerung. Die vorgestellte Methode ermöglicht die objektive, simultane Untersuchung beider Augen in 3 Dimensionen in Echtzeit und mit Angabe der Schielwinkel in den 3 Dimensionen.

    Notes: Abstract  · Objective: The purpose of this study was to evaluate the antiemetic effect of prophylactic dimenhydrinate application prior to Faden operation and to compare the incidence of PONV between bimedial Faden operation (BMF) and horizontal recess-resect procedure (R&R). · Methods: Ninety-nine children (4–10 years) scheduled for BMF were included in this prospective double-blind study. Midazolam (0.5 mg/kg body weight, BW) was administered orally for premedication 30 min before induction of anesthesia. Additionally, children weighing ≥23 kg received either dimenhydrinate suppositories or placebo. The placebo group was compared with 148 children who underwent R&R surgery without antiemetic prophylaxis during the same period. Anesthesia was induced with thiopentone (5–10 mg/kg BW) and vecuronium (0.1 mg/kg BW) bromide and maintained with halothane (1–2 vol%) in N2O/O2 (65/35 vol%). Age, height, weight, and incidence of oculocardiac reflex were documented. PONV was classified into ”no vomiting”, ”vomiting without therapy”, and ”vomiting requiring rescue medication”. In the latter case dimenhydrinate was given again. The chi-square test was used for statistical analysis.  · Results: Forty-eight patients received placebo, while 51 received dimenhydrinate. No differences between any groups were observed concerning age, height, weight, and incidence of oculocardiac reflex. Compared to R&R surgery, PONV requiring rescue medication occurred significantly more frequently after BMF (45% vs 23% after R&R). PONV after BMF was significantly less severe in the dimenhydrinate group than in the placebo group. The total incidence of PONV after BMF, however, was not significantly reduced. · Conclusion: The high incidence of PONV after BMF can be explained by the greater invasiveness of BMF than R&R surgery. PONV requiring antiemetic rescue medication can be reduced by preoperative administration of dimenhydrinate suppositories.
