    Description / Table of Contents: Summary To investigate the role of the renin angiotensin system in the pathogenesis of hypertension in Cushing's syndrome two patients with hypercorticism were infused with 20 mg saralasin (1-sar-8-alaangiotensin II) over a period of 30 minutes under constant blood pressure control. In addition, one patient with primary aldosteronism, an established form of mineralocorticoid hypertension, served as control. Neither in the two patients with Cushing's syndrome nor in the patient with primary aldosteronism could a blood pressure lowering effect of saralasin be observed. In the two patients with hypercorticism both renin activity and plasma aldosterone increased during saralasin infusion. The patient with primary aldosteronism only showed a weak increase in plasma aldosterone concentration. These results seem to exclude an important role of the renin angiotensin system in the pathogenesis of hypertension in Cushing's syndrome. The unresponsiveness of elevated blood pressure to saralasin in the two patients with hypercorticism and in the patient with primary aldosteronism indirectly supports the assumption that in patients with Cushing's syndrome increased mineralocorticoid activity may be the main factor in the pathogenesis of hypertension.
    Notes: Zusammenfassung Um die Bedeutung des Renin-Angiotensin Systems in der Pathogenese der Hypertonie bei Cushing-Syndrom zu untersuchen, wurden bei 2 Patienten mit Hyperkortizismus 20 mg Saralasin (1-Sar-8-Ala-Angiotensin II) über einen Zeitraum von 30 min unter ständiger Blutdruckkontrolle infundiert. Zusätzlich diente ein Patient mit primärem Aldosteronismus, einer etablierten Form von Mineralokortikoidhochdruck, als Kontrolle. Weder bei den 2 Patienten mit Cushing-Syndrom noch bei dem Patienten mit primärem Aldosteronismus ließ sich ein blutdrucksenkender Effekt des Saralasins nachweisen. Die beiden Patienten mit Hyperkortizismus zeigten unter Saralasin sowohl einen Anstieg der Renin-Aktivität als auch des Plasmaaldosterons. Bei dem Patienten mit primärem Aldosteronismus ließ sich nur ein geringgradiger Anstieg der Plasmaaldosteronkonzentration nachweisen. Diese Ergebnisse sprechen gegen eine wichtige Rolle des Renin-Angiotensin Systems in der Pathogenese der Hypertonie bei Cushing-Syndrom. Die Unbeeinflußbarkeit des Hochdrucks durch Saralasin bei den beiden Patienten mit Hyperkortizismus und dem Patienten mit primärem Aldosteronismus stützen indirekt die Annahme, daß bei Patienten mit Cushing-Syndrom eine erhöhte Mineralokortikoidaktivität der Hauptfaktor in der Pathogenese der Hypertonie ist.

    Notes: Summary We evaluated the usefulness of the basal urinary 24-h excretion rates of free cortisol versus 17-hydroxycorticosteroids in the diagnosis of Cushing's syndrome. On an outpatient basis, both urinary free cortisol and 17-hydroxycorticosteroids levels were determined in 48 patients with Cushing's syndrome, as well as in 95 obese and 94 healthy control persons of normal weight. Determination of the urinary free cortisol content allowed a clear-cut distinction between the patients with hypercortisolism and the controls, resulting in a sensitivity of 100% and specificity of 98% for the diagnosis of Cushing's syndrome. The diagnostic accuracy of urinary free cortisol was distinctly superior to that of 17-hydroxycorticosteroids, which showed a wide overlap of values between the groups, with a sensitivity of 73% and a specificity of 94%. In conclusion, the measurement of basal urinary free cortisol provided an excellent diagnostic sensitivity and specificity in the assessment of adrenocortical function. This simple and accurate test thus seems to be particularly useful in the outpatient evaluation of patients with suspected Cushing's syndrome.

    Description / Table of Contents: Zusammenfassung Wiederholte ambulante Plasmacortisolbestimmungen bei einer Patientin mit deutlichen klinischen Zeichen des Hyperkortizismus ergaben mit 16–24 µg/100 ml normale Werte. Erst ein unter standardisierten Bedingungen erstelltes Plasmacortisol-Tagesprofil erbrachte den eindeutigen Nachweis der Hypercortisolämie. Im Vergleich zu gesunden Normalpersonen liegen die Plasmacortisolspiegel der Cushing-Patientin auf einer höheren Ebene. Die physiologischen Phasen ruhender Cortisolsekretion in den Abend- und Nachtstunden fehlen. Wie bei Gesunden können wir bei diesem Fall von Hypercortic ismus, beruhend auf bilateraler Nebennierenrinden-Hyperplasie, große Schwankungen des Plasmacortisolspiegels beobachten. Der Wert wiederholter Plasmacortisolbestimmungen in der Abklärung des Cushing-Syndroms wird diskutiert.
    Notes: Summary In a patient with typical features of Cushing's disease, repeated ambulant determinations of plasma cortisol showed normal values with 16 to 24 µg/100 ml. The entire analysis of the circadian variations of plasma cortisol under standardized conditions led to the diagnosis of hypercorticism. Compared with 2 healthy subjects the patient's curve is set at a higher level. Physiological, quiet periods of cortisol secretion in late day-time do not occur. In accord to the normal, we find great variations of plasma cortisol in this case of Cushing's syndrome due to bilateral adrenal hyperplasia. The diagnostic value of plasma cortisol determinations at short term intervals in differentiating the various forms of hypercorticism is discussed.

    Description / Table of Contents: Zusammenfassung Arterieller Blutdruck, Plasma-Reninaktivität (PRA) und die Konzentration von Aldosteron (PA) und Cortisol (PC) im Plasma wurden bei 8 männlichen Patienten mit essentieller Hypertension vor und nach β-Receptorenblockade mit Propranolol und Prindolol bestimmt. Die Blutentnahme zur Bestimmung von PRA, PA und PC erfolgten halbstündlich zwischen 22.00 und 6.00 Uhr. Vor der β-Receptorenblockade fand sich ein typisches rhythmisches Verhalten von PRA, PA und PC mit relativ niedrigen Werten zwischen 22.00 und 24.00 Uhr und höheren Werten zwischen 1.00 und 4.00 Uhr. Unter einer Dosis von 3×50 mg Propranolol täglich über 4 Wochen kam es bei allen Patienten zu einer signifikanten Senkung des arteriellen Blutdruckes sowie der Pulsfrequenz. PRA und PA fielen signifikant ab. Der typische Rhythmus von PRA war nicht mehr nachweisbar. Dagegen wiesen PA und PC eine qualitativ unveränderte Rhythmik auf. Prindolol führte in einer Dosis von 3×10 mg zu einer vergleichbaren Blutdrucksenkung. Die mittlere PRA und PA blieben unverändert. Die typische Rhythmik der PRA wurde durch Prindolol ebenfalls aufgehoben, während der PA- und PC-Rhythmus unbeeinflußt blieb. Die hypotensive Wirkung von Prindolol kann nicht über eine Hemmung der PRA erklärt werden. Die Dissoziation des PRA- und des PA-Rhythmus weist darauf hin, daß unter β-Receptorenblockade die zirkadiane Rhythmik der Aldosteronsekretion nicht durch das Renin-angiotensin-System reguliert wird. Die normale Cortisolrhythmik ist mit einem unveränderten ACTH-Rhythmus vereinbar, der für die qualitativ normale zirkadiane Aldosteronsekretion unter β-Receptorenblokkade verantwortlich zu sein scheint.
    Notes: Summary Blood pressure (BP), plasma renin activity (PRA) and plasma concentration of aldosterone (PA) and cortisol (PC) were determined in essential hypertensive patients before and after β-adrenergic blockade with propranolol and prindolol. Serial measurements of PRA, PA and PC at 30 min intervals (8 p.m. to 6 a.m.) were performed in 8 patients. Administration of propranolol (50 mg three times daily) over a period of 4 weeks was followed by a significant reduction in BP, PRA and PA. PC remained unaltered. The pattern of rhythmic secretion of renin was abolished whereas that of aldosterone persisted at a lower level. Prindolol (10 mg three times daily) had a similar effect on BP as propranolol but failed to lower PRA and PA. Rhythmic secretion of renin was also markedly altered with prindolol in the absence of any change in rhythmicity of PA and PC. The hypotensive action of prindolol was not mediated via inhibition of renin release. The dissociation between PRA and PA rhythmicity indicates that during β-adrenergic blokkade diurnal rhythm of aldosterone secretion is not regulated by the renin-angiotensin-system. Rhythmicity of PC was normal, indicating that also ACTH-secretion was unaltered. Thus, under β-blockade unchanged diurnal rhythm of aldosterone may be due to normal ACTH-secretion.

    Description / Table of Contents: Zusammenfassung Bei 19 männlichen Patienten mit essentieller Hypertonie und 8 normotensiven Probanden wurde die Plasma-Prolactin-Konzentration halbstündlich zwischen 22.00 und 6.00 Uhr bestimmt. Die hypertensiven Patienten wiesen signifikant höhere Plasma-Prolactin-Konzentrationen auf als die normotensiven Kontrollen. Patienten mit niedriger Plasma-Renin-Aktivität und nur geringgradig erhöhten Plasma-Prolactin-Konzentrationen hatten eine geringere Natriumausscheidung, aber eine stärkere 24 h-Natriurese nach Furosemid, als die Patienten mit normalem Renin und sehr hohen Plasma-Prolactinwerten. 6 Patienten wurden mit dem dopaminergen Agonisten Bromocriptine über einen Zeitraum von 3–6 Wochen behandelt. Es kam in 5 Fällen zu einer signifikanten Senkung des systolischen und diastolischen Blutdrucks, bei 2 Patienten zu einer Blutdrucknormalisierung. Die Ergebnisse weisen darauf hin, daß der beschriebenen Natrium- und Wasser-retinierenden Wirkung von Prolactin für die Aufrechterhaltung einer chronischen Blutdrucksteigerung wahrscheinlich keine entscheidende Bedeutung zukommt. Es ist denkbar, daß die erhöhte Plasma-Prolactin-Konzentration bei den hypertensiven Patienten einen Index für eine veränderte zentralnervöse Funktion darstellt, die durch eine verminderte hypothalamische dopaminerge Aktivität charakterisiert ist. Die blutdrucksenkende Wirkung des dopaminergen Agonisten Bromocriptine könnte darauf hinweisen, daß eine Abnahme des dopaminergen Tonus im Hypothalamus einen Faktor in der Pathogenese der essentiellen Hypertension darstellt.
    Notes: Summary Serial measurements of plasma-prolactin concentration (HPr) and plasma-renin activity (PRA) at 30-min intervals were made in 19 male patients with essential hypertension and in 8 normotensive subjects. HPr was markedly higher in the hypertensive patients than in the normotensive controls. Patients with reduced plasma-renin activity and only slightly elevated HPr-levels showed lower urinary sodium excretion, but a more pronounced 24-h natriuretic response to i.v. furosemide than patients with normal renin and very high HPr-levels. Six patients were treated with the dopaminergic agonist bromocriptine. The drug induced a significant blood pressure reduction in five patients and normalised pressure in two patients. The data do not indicate a role for prolactin in sustaining hypertension via renal salt retaining mechanisms. It is suggested that the raised HPr-levels represent an index of altered central nervous function, characterized by reduced hypothalamic activity. The blood pressure-lowering effect of the dopaminergic agonist bromocriptine fits with the hypothesis that reduced hypothalamic dopaminergic activity might be a factor in the pathogenesis of essential hypertension.

    Notes: Abstract The behavioral and electrophysiological activity of a mimic [(Z,E)7,9,11-dodecatrienyl formate] of the major sex pheromone component [(Z,E) 9,11,13-tetradecatrienal] of carob moth was assessed. Wind-tunnel bioassays demonstrated that the formate was as effective as natural gland extracts, and significantly more effective than the trienal alone or than the trienal blended with two minor pheromone components, in evoking source contact. Dispensers containing the formate were as effective as trienal-containing blend lures in attracting males when placed at the same dosage in traps in date gardens. Single-cell recordings showed that at least two olfactory neurons, differentiated by spike amplitude, are located in the long trichoid hairs on male carob moth antennae. Dose-response relationships indicated that puffs from cartridges loaded with at least 0.1 μg of the formate or of the trienal were necessary to elicit spiking by either the small or the large-spiking cell within a sensillum. Cross-adaptation studies demonstrated that both compounds stimulated the same large-spiking cell. The frequencies of spikes evoked from the large cell when stimulated by emissions from 0.1-μg, 1-μg, or 10-μg cartridges of either the formate or the trienal were not significantly different, suggesting that the formate is an effective mimic of the trienal at the antennal receptor cell level.

    Notes: Abstract Three sex pheromone components of the carob moth were isolated and identified from the extract of female pheromone glands, using a variety of techniques including coupled gas chromatographic-electroantennographic recordings, coupled gas chromatographic-mass spectrometric analysis, microozonolysis, electroantennographic assays of monounsaturated standards, wind-tunnel bioassays, and field trials. The major component was identified as (Z,E)-9,11,13-tetradecatrienal, a novel lepidopterous pheromone component structure. Two minor components, either one of which improves the upwind flight response of males when blended with the major component, were identified as (Z,E)-9,11-tetradecadienal, and (Z)-9-tetra-decenal.
