    Abstract: There is scarce information regarding the complete renin-angiotensin-aldosterone system (RAAS) and its changes after bariatric surgery (BS) in patients with morbid obesity (MO). Objective: to analyze changes (Δ) in RAAS components in MO after BS at several follow-up time points. Methods: Fifty-five MO patients (78% women, 40% hypertensives, age 43.4±8.9yr, BMI 42.4±5.3Kg/m 2 ) were evaluated prospectively before and 1,3,6 and 12 months(m) after BS, with assessment of anthropometrics, RAAS components and 24h-systolic BP (SBP). Results: at 12m body weight (-35.1±9.7 Kg) and waist circumference (-27±9.2 cm) decreased (p 〈 0.001). At 1,3 and 6m, there was a reduction in angiotensin-converting enzyme (ACE) and ACE-2 activity, as well as a decrease in plasma renin activity (PRA) and aldosterone from 3m on (Table). The decrease in 24h-SBP at 1m was 6.2 mmHg (3.6 to 8.8), p 〈 0.001. There were correlations (p 〈 0.05) between ΔPRA/Δ24h-SBP at 1m (r=-0.332), 3m (r=-0.330) and 12m (r=-0.322). ΔPRA and Δbody weight correlated only at 1m (r=-0.427). Δaldosterone and Δwaist circumference correlated at 12m (r=-0.372). ΔACE and Δ24h-SBP correlated at 6 and 12m (r=0.451 and r=0.635, respectively). Correlations between Δ24h-SBP and Δbody weight were statistically significant only at 6m (r=0,366; p=0,016). Conclusions: there are sequential changes in several components of the RASS after BS, with an increase in ACE and ACE2 activity as early as 1m after BS that lasts until 6m, and a decrease in PRA and plasma Aldosterone that initiates at 3m and remains 12m after BS. These changes correlate with the decrease of 24h-SBP. RAAS components, but not body weight, appear to correlate with changes in BP after BS.

    Abstract: The purpose of this paper is to explore, through the analysis case, how the revitalization of a historic centre has been carried out and the role of tourism in this process. Design/methodology/approach This case study area is well-documented as there has been extensive fieldwork on the transformation of the commercial sector in the Monumental Zone of Pontevedra in the last three decades. In order to prepare this paper, a bibliographic review, in-depth interviews, premises registration data and population data have been used. Findings The findings show that the processes of change and revitalization do not conform to a single reality common to all historical centres, although similar strategies are developed, the role of the participating actors and, especially, the idiosyncrasy of these spaces change the outcomes. On the one hand, gentrification does not occur and the increase of residential uses is still a goal. On the other hand, the tourism strategy brings more visitors and complements the commerce activity and attraction. Social implications The new challenge of these spaces, and the urban contribution from this research, is that in the appropriation of space by citizens, tourism may be a complement for commerce, and shops and hospitality (facilities) make these spaces more livable. Although tourism does not necessarily increase the number of residents, the revenue from tourism may prevent the reoccurrence of abandonment. Originality/value The paper focusses on both gentrification and touristification; processes that have led to the substitution of residents and activities and the conflict with the local population and the normalization of urban life. This case has been selected because despite a seemingly successful revitalization process, recently some old threats seem to be returning.

    Abstract: Adipose tissue is a higly active endocrine tissue and produce adipokines, peptides produced by adipose tissue and exerting autocrine, paracrine and endocrine function. Objective: to analyze changes (Δ) in serum levels of several adipokines and inflammatory markers in morbidly obese patients (MO) after bariatric surgery (BS) at several follow-up time points. Methods: Fifty-two MO (81% women, 40% hypertensives, age 43.9±8.9 yr, BMI 42.2±5.4 Kg/m 2 , waist circumference 131.6±12.1 cm) were evaluated prospectively before and 1,3,6 and 12 months (m) after BS, with assessment of anthropometric data and serum concentrations of several adipokines and inflammatory markers. BS technique: laparoscopic gastric by-pass (28 of 52) and sleeve gastrectomy (24 of 52). Results: at 12m body weight (-35.7±10.2 Kg) and waist circumference (-27.2±9.4 cm) decreased (p 〈 0.001). Two inflammatory markers, leptin and hsCRP, decreased at 1,3,6 and 12m (p 〈 0,005 for all). Adiponectin and angiopoetin-2 increased from month 3 on. (Table). These Δ were statistically significant when examined by an ANOVA with repeated measures, excepte for adiponectin (p=0.065). After adjusting for age, sex, and BS technique, the BS technique was statistically significant only for ΔhsCRP (p=0.035). Pearson correlations were explored. There were no statistically significant correlation between Δ body weight or Δ waist circumference with changes of any of these markers at any follow-up time point. Conclusions: there is a decrease in inflammatory markers 1 m after BS in MO patients with progressive decrease throughout 12 m. These changes are independent of the BS technique and of Δ in body weight and waist circumference.
