    Abstract: Ceftriaxone is used in wide range of day to day microbial infections in clinicalpractice3. Despite the incumbent drug regulating authority in Pakistan, there is scanty literaturecomparing the anti-microbial efficacy of different available brands of ceftriaxone. Objectives:To know the in-vitro activity of various brands of ceftriaxone against bacteria most commonlyisolated from surgical site infection (SSI). A comparison of five days cost of these brands willalso be done. Design: Experimental study. Period: Feb 2013 to Aug 2013 Setting: Surgical“C” unit Lady Reading Hospital (LRH) in collaboration with departments of pharmacologyKhyber Girls Medical College (KGMC) and microbiology department of Lady Reading HospitalPeshawar. Material & Methods: Isolates of five bacteria i.e. Staphylococcus aureus, Proteusmirabilis, Escherischia coli, enterobacter Spp, and Klebsiella pneumoniae, found sensitive toceftriaxone were grown on 50 slops each and the zone of inhibition was checked for each ofthe ten brands of ceftriaxone. Results: The zones of inhibitions of different brands of ceftriaxoneagainst the above mentioned bacteria were not significantly different. The cost of therapy wassignificantly different for ten brands. Conclusions: Various brands of ceftriaxone of variablecost had no influence on their activity against bacteria involved in SSI.

    Abstract: Introduction: Laparoscopic cholecystectomy is the treatment of choice forsymptomatic cholelithiasis. Intraperitoneal instillation of bupivacaine is one of the methods usedto improve pain relief after laparoscopic cholecystectomy. Objective: To compare the mean painscore after intraperitoneal instillation of bupivacaine with placebo during laparoscopiccholecystectomy. Study Design: Randomized Control trial. Setting: This study was carried out asurgical unit PGMI Lady Reading Hospital and Hayatabad Medical Complex, Peshawar.Duration: The duration of study was 6 months from 15th May to 15th December, 2013. Subjectsand Methods: 92 patients in each group were included in study to compare the mean pain scoreof intraperitoneal instillation of bupivacaine (Group A; study group) with 0.9% normal salinesolution (Group B; placebo group) using visual analogue scale after laparoscopiccholecystectomy at 12th hour after surgery. Data was entered in software SPSS version 16.0. Ttest was used to compare the mean pain scores. Results: The mean age of patients in Group Aand B was 41.82 ± 7.34 and 40.95 ± 9.24 respectively (p=0.483). Group A has low mean painscore (3.619 ± 0.676) according to Visual Analogue Scale then Group B (3.837 ± 0.667) with astatistically significant p value (p=0.036). A t test failed to reveal a statistically reliable differencebetween gender (p=0.513) and age (p=0.767) wise distribution of mean pain between group Aand B. Conclusions: Mean pain score of intraperitoneal instillation of bupivacaine is significantlyless than 0.9% normal saline solution at 12th hour after laparoscopic cholecystectomy.

    Abstract: Introduction: Colorectal cancer is a potentially fatal gastrointestinal disease andhas been studied extensively. In an effort to decrease the morbidity and mortality associatedwith this disease, studies have been performed to gain insight into the anatomic distribution,average age at presentation, mean age at presentation for different segments of colon involvedand intersex differences. Objective: The objective of this study is to determine the frequencyof sites of colorectal cancer involvement. Material and methods: Study design: Study wasdescriptive case series. Setting: General surgical department post graduate Lady ReadingHospital Peshawar. Period: From 01/01/2011 to 30/06/2012. Sample size: Sample size was416 using 3.57% proportion of descending colon5, 95% confidence level and 1.785% margin oferror under WHO software for sample size determination. Sampling technique: Consecutivenon probability sampling. Results: This study was carried out in 416 consecutive patients.These patients included 233 men (56 %) and 183 women (44%). Age varied from twelve yearsto seventy years. The commonest age group in the study at the time of presentation was63-72 years. Commonest site involved was rectum (26%) followed by sigmoid colon (16%).Bleeding per rectum was the commonest symptom (62.05%) followed by altered bowel habits(35.71%). Twelve patients (21.43%) presented with intestinal obstruction. Histopathologically,twenty patients had well differentiated adeno-carcinoma (35.72%) whereas eighteen patientshad anaplastic tumour (32.14%) and mucinous adeno-carcinoma was found in five patients(8.92%).Conclusion: Rectum is the most common site of tumour followed by left, right andtransverse colon respectively. The site of involvement affects the surgical procedure required.In conclusion the symptoms of colorectal cancer may not be representative of any anatomicalsite, by the time symptoms appear the lesion may have become invasive.
