    Abstract: Data collected from a national survey of economic development professionals are used in this study to examine the perceived role of Leadership in Energy and Environmental Design–certified and Energy Star–certified buildings in the business recruitment process. A series of ordinal logistic regression models are estimated to examine economic developers’ attitudes toward these buildings after controlling for their personal characteristics and the characteristics of the communities where they work. The results suggest less favorable perceptions about this form of sustainable real estate development in smaller communities and those with weaker economic fundamentals. Furthermore, women employed in economic development are found to have more favorable attitudes toward these buildings than their male counterparts. Increasing support for sustainable real estate development among economic developers working in local government may therefore require a combination of thoughtful policy making, executive education, and greater gender diversity in the profession.

    Abstract: Texturing of breast implants is done to decrease the risk of associated complications. Each manufacturer utilizes unique and at times proprietary techniques to texture the surface of their implants. Little is known about the integrity of this surface structure texturing or the propensity for the surfaces to shed particulate matter. This study aimed to determine the extent of surface particulate shedding from 3 textured implants approved by the US Food and Drug Administration (FDA), which are manufactured by Allergan, Mentor, and Sientra. Methods: Control images of each of the 3 textured breast implants were obtained with scanning electron microscopy (SEM). A liquid adhesive, ethylene vinyl acetate (EVA) copolymer was then applied to the external shell of the implants, allowed to cool, and peeled from the surface. Images of the EVA copolymer were taken with SEM to qualitatively analyze displacement of surface particulate debris. Scanning electron microscopy imaging of the implants was repeated for qualitative comparisons with the control images. Results: The peeled copolymer of the 3 implants exhibited surface shedding. Comparison of the 3 breast implants showed the shedding to be greatest for the Allergan implant. Conclusions: This study highlights the dynamic surface material properties of the 3 FDA-approved breast implants. Shedding of particulate matter from the implant surfaces can be precipitated by moderate adhesion. Our qualitative examination of SEM findings showed more debris shed from the Allergan breast implants than from the Mentor or Sientra implants.

    Abstract: Surgical resection of metastases to the adrenal gland can improve overall survival of patients with stage IV melanoma, but its relative value with respect to current nonsurgical therapies is unknown. We hypothesized that surgery remains an optimal first-line treatment approach for resectable adrenal metastases. A search of our institution's prospectively collected melanoma database identified stage IV patients treated for adrenal metastases between January 1, 2000, and August 11, 2014. The 91 study patients had a mean age of 60.3 years at diagnosis of adrenal metastasis and 24 had undergone adrenalectomy. Improved survival was associated with an unknown primary lesion, surgical resection, and nonsurgical therapies. Median overall survival from diagnosis of adrenal metastases was 29.2 months with adrenalectomy versus 9.4 months with nonoperative treatment. Adrenalectomy, either as complete metastasectomy or targeted to lesions resistant to systemic therapy, is associated with improved long-term survival in metastatic melanoma.

    Abstract: Stairways into buildings present a significant environmental barrier for those with mobility impairments. A number of home access solutions that allow users to safely enter and exit the home exist; however, these all have some limitations. The purpose of this work was to develop a novel, inclusive home access solution that integrates a staircase and a lift into one device. Method We developed an integrated staircase lift following a structured protocol with stakeholders providing feedback at various stages in the design process, consistent with rehabilitation engineering design methods. Results A novel home access device was developed. The integrated staircase-lift has the following features: inclusivity, by a design that provides an option for either use of stairs or a lift; constant availability, with a lift platform always ready for use on either level; and potential aesthetic advantages when integrating the device into an existing home. The potential also exists for emergency descent during a power outage, and self-powered versions. Conclusions By engaging stakeholders in a user-centred design process, we were able to gain insight into the limitations of existing home access solutions and get specific feedback on our lift concept. This information strengthened the development of a novel home access device.
