    Kurzfassung: Although several human clinical trials using various bone marrow-derived cell types for cirrhotic or decompensated patients have reported a short-term benefit, long-term follow-up data are limited. We analyzed the long-term clinical outcomes of autologous bone marrow cell infusion (ABMI) for decompensated liver cirrhosis (LC). Patients enrolled in a pilot single-armed ABMI study were followed up more than 5 years. Bone marrow-derived mononuclear cells (BM-MNCs) from decompensated LC were harvested and after processing were infused into a peripheral vein. The laboratory test results and long-term clinical course including liver transplantation (LT), development of cancer, cause of death, and survival after ABMI were analyzed. Nineteen patients were followed up for a median of 66 months after ABMI. Liver function, including serum levels of albumin and Child–Pugh (CP) score, was improved at the 1-year follow-up. Liver volume was significantly greater, cirrhosis was sustained, and collagen content was decreased at the 6-month follow-up. Five years after ABMI, five patients (26.3%) maintained CP class A without LT or death, and five patients (26.3%) had undergone elective LT. Hepatocellular carcinoma (HCC) occurred in five patients (26.3%), and lymphoma and colon cancer occurred in one patient each. Three patients (15.8%) were lost to follow-up at months 22, 31, and 33, respectively, but maintained CP class A until their last follow-up. Five patients expired due to infection. While improved liver function was maintained in some patients for more than 5 years after ABMI, other patients developed HCC. Further studies of long-term follow-up cohorts after cell therapy for LC are warranted.

    Kurzfassung: Interest in health information exchanges (HIEs) is increasing. Several countries have adopted core health data standards with appropriate strategies. Objective: This study was conducted to determine the feasibility of a continuity of care record (CCR) as the standard for an electronic version of the official transfer note and the HIE in Korean healthcare. Method: A technical review of the CCR standard and analysis of stakeholders' views were undertaken. Transfer notes were reviewed and matched with CCR standard categories. The standard for the Korean coding system was selected. Stakeholder analysis included an online survey of members of the Korean Society of Medical Informatics, a public hearing to derive opinions of consumers, doctors, vendors, academic societies and policy makers about the policy process, and a focus group meeting with EMR vendors to determine which HIE objects were technically applicable. Results: Data objects in the official transfer note form matched CCR standards. Korean Classification of Diseases, Korean Standard Terminology of Medicine, Electronic Data Interchange code (EDI code), Logical Observation Identifiers Names and Codes, and Korean drug codes (KD code) were recommended as the Korean coding standard. ‘Social history’, ‘payers’, and ‘encounters’ were mostly marked as optional or unnecessary sections, and ‘allergies’, ‘alerts’, ‘medication list’, ‘problems/diagnoses’, ‘results’, and ‘procedures’ as mandatory. Unlike the US, ‘social history’ was considered optional and ‘advance directives’ mandatory. At the public hearing there was some objection from the Korean Medical Association to the HIE on legal grounds in terms of intellectual property and patients’ personal information. Other groups showed positive or neutral responses. Focus group members divided CCR data objects into three phases based on predicted adoption time in CCR: (i) immediate adoption; (ii) short-term adoption (‘alerts’, ‘family history’); and (iii) long-term adoption (‘results’, ‘advanced directives’, ‘functional status’, ‘medical equipment’, ‘vital signs’, ‘plan of care’, ‘social history’). Conclusion: There were no technical problems in generating the CCR standard document from EMRs. Matters of concern that arose from study results should be resolved with time and consultation.

    Kurzfassung: Background. Although Burkitt lymphoma (BL) is the most common childhood lymphoma in sub-Saharan Africa, Hodgkin lymphoma (HL) and other non-Hodgkin lymphomas occur. Diagnosing non-jaw mass presentations is challenging with limited pathology resources. Procedure. We retrospectively analyzed 114 pediatric lymphomas in Lilongwe, Malawi, from December 2011 to June 2013 and compared clinical versus pathology-based diagnoses over two time periods. Access to pathology resources became more consistent in 2013 compared with 2011-2012; pathology interpretations were based on morphology only. Results. Median age was 8.4 years (2.1-16.3). The most common anatomical sites of presentation were palpable abdominal mass 51%, peripheral lymphadenopathy 35%, and jaw mass 34%. There were 51% jaw masses among clinical diagnoses versus 11% in the pathology-based group ( P 〈 .01), whereas 62% of pathology diagnoses involved peripheral lymphadenopathy versus 16% in the clinical group ( P 〈 .01). The breakdown of clinical diagnoses included BL 85%, lymphoblastic lymphoma (LBL) 9%, HL 4%, and diffuse large B-cell lymphoma (DLBCL) 1%, whereas pathology-based diagnoses included HL 38%, BL 36%, LBL 15%, and DLBCL 11% ( P 〈 .01). Lymphoma diagnosis was pathology confirmed in 19/66 patients (29%) in 2011-2012 and 28/48 (60%) in 2013 ( P 〈 .01). The percentage of non-BL diagnoses was consistent across time periods (35%); however, 14/23 (61%) non-BL diagnoses were pathology confirmed in 2011-2012 versus 16/17 (94%) in 2013. Conclusions. Lymphomas other than Burkitt accounted for 35% of childhood lymphoma diagnoses. Over-reliance on clinical diagnosis for BL was a limitation, but confidence in non-BL diagnoses improved with time as pathology confirmation became standard. Increased awareness of non-BL lymphomas in equatorial Africa is warranted.

    Kurzfassung: The goal of the present study was to examine factors associated with the attitude toward life-sustaining treatment among older adults in South Korea. Focus was given to sociodemographic characteristics (age, gender, education, financial status, and religious affiliation), family-related variables (presence of a spouse, children, and a living parent [or in-law]), and health-related variables (chronic conditions, functional disability, and self-rated health). Using data from 6,895 participants (aged 65−97) in the 2014 Korean National Elderly Survey, logistic regression model was tested. The proportion of the sample that endorsed a strong disagreement with the provision of life-sustaining treatment was 53.2%. The increased odds of having a strong disagreement was found in those with younger age ( 〈 75), higher education, a living parent (or in-law), and more favorable ratings of health. Findings shed light on programs and services on end-of-life care planning for older adults in South Korea.

    Kurzfassung: The purposes of the present study were (1) to explore the completion rate of advance directives (ADs) in a sample of Asian Americans and (2) to examine the direct and moderating effects of knowledge of AD, education, and acculturation in predicting AD completion. Education and acculturation were conceptualized as moderators in the link between knowledge and completion of ADs. Methods: Using data from 2609 participants in the 2015 Asian American Quality of Life survey (aged 18-98), logistic regression analyses on AD completion were conducted, testing both direct and moderating effects. Results: The overall AD completion rate in sample was about 12%. The AD knowledge and acculturation independently predicted AD completion. No direct effect of education was found; however, it interacted with AD knowledge. The AD knowledge was more likely to be translated into completion in the group with higher education. Conclusions: The AD completion rate observed in the present sample of Asian Americans was much lower than that of the US general population (26%-36%). The interactive role of education helps to explain the gap between AD knowledge and completion and suggests intervention strategies.

    Kurzfassung: Pelargonidin (PEL) is a well-known red pigment found in plants and has important biological activities that are potentially beneficial for human health. The aim of this study was to investigate the effect of PEL on lipopolysaccharide (LPS)-induced liver failure in mice, and to elucidate its underlying molecular mechanisms. Liver failure was induced by LPS (15 mg/kg, i.p) in mice, and 12 h later, they were treated intravenously with PEL. Administration of LPS significantly increased mortality, serum levels of alanine transaminase (ALT), aspartate transaminase (AST), and inflammatory cytokines, and expression of toll-like receptor 4 (TLR4) protein; PEL treatment effectively countered these effects of LPS. Further, LPS treatment markedly increased the expression of myeloid differentiation primary response gene 88 (MyD88), phosphorylation of p38, extracellular signal-regulated kinase (ERK), and c-Jun N-terminal kinase (JNK), and expressions of nuclear proteins, such as nuclear factor (NF)-κB and phosphorylated c-Jun. Additionally, LPS increased the serum levels of tumor necrosis factor (TNF)-α and interleukin (IL)-6. All these effects of LPS were attenuated by PEL. In addition, the LPS-mediated increase in the level of serum interferon (IFN)-β expression of the TLR-associated activator of IFN (TRIF) protein, and phosphorylation of IFN regulator factor 3 (IRF3) were reduced by PEL. Our results suggest that PEL attenuates LPS-induced liver damage by inhibition of the TLR-mediated inflammatory pathway and could be used to treat liver diseases.

    Kurzfassung: Informed by recent developments in the field of evolutionary psychology, we elicit novel insights about the causal relationship between the activation of mating motives and gender-specific preferences for risk taking in tourism. In this endeavor, across four experimental studies, we illustrate that the effect of activating mating motives on the propensity for risk-taking tourism will be greater for men than it is for women. That is, when mating motives were salient, men consistently displayed a greater preference for the choice of high-risk travel activities (studies 1A and 1B), adventurous destination choices (study 2), and adventurous appeals in travel advertising (study 3). Our findings offer important theoretical contributions to research into the role of gender in risk-taking tourism behaviors from the perspectives both of evolution-based theorizing and managerial implications for tourism practice.

    Kurzfassung: Objective: Given the importance of social ties and connectedness in the lives of older ethnic immigrants, the present study examined the prevalence of social isolation and its risk factors in older Korean Americans. Method: Using survey data from 1,301 participants ( M age = 70.5, SD = 7.24), risk groups for marginal social ties with family and friends were identified and predictors of each type of social isolation explored. Results: Male gender and poorer rating of health were identified as common risk factors for marginal ties to both family and friends. Findings also present specific risk factors for each type of social isolation. For example, an increased risk of having marginal ties with friends was observed among individuals with perceived financial strain, greater functional impairment, and a shorter stay in the United States. Discussion: The common and specific risk factors should be incorporated in programs to reduce social isolation in older immigrant populations.

    Kurzfassung: Objective: Ethnic communities often serve as the primary source of emotional support for older immigrants. This study aims to identify individuals who are more likely to be nominated as emotional confidants by age peers in the ethnic community and to examine factors contributing to the likelihood of being a more frequently endorsed confidant. Method: Data were drawn from a survey with 675 older Korean Americans. Using the name-generator approach in Social Network Analysis (SNA), participants were asked to list the names of three emotional confidants among age peers in the community. Results: A higher level of popularity (i.e., in-degree centrality) was predicted by male gender, advanced education, lower functional disability, fewer symptoms of depression, and higher levels of participation in social activities. Discussion: Our findings suggest the value of SNA as a means of identifying the key emotional confidants in the community and utilizing them in community-based interventions.

    Kurzfassung: Future perceptions consist of an individual’s ambition, view of future events, and plan for life domains. Such perceptions can predict and shape a person’s career development and motivate to pursue and achieve goals in different life roles. Research on emerging adults’ perceptions has important implications for career professionals assisting this population. In this study, the authors investigated the future perceptions of 49 U.S. and 39 Israeli emerging adult university males using thematic analysis. Participants’ responses represented an array of topics and were grouped into the following themes: (a) work, (b) family and relationships, (c) education, (d) material and monetary assets, (e) location, (f) leisure, and (g) general quality of life. Results suggested that culture could be important in how emerging adult males envision their future lives and roles. Recommendations for how career educators and counselors can implement the findings in their work with U.S. and Israeli college students are discussed.
