    Abstract: Genome-wide association studies (GWAS) have determined a new single nucleotide polymorphism (SNP) called VTI1A (rs7086803) that induces lung cancer susceptibility in nonsmoking women in Asia. This study aimed to evaluate the association between the VTI1A gene and the susceptibility of Chinese patients to lung cancer; it was also conducted to investigate the relationship between VTI1A SNP and adiponectin receptor 1 expression. Methods A total of 887 subjects were enrolled in this study. VTI1A (rs7086803) genotypes were determined by genotyping. Overall survival (OS) was evaluated using Kaplan-Meier analysis with a log-rank test. Results Multivariate regression analysis results indicated that the AA genotype of VTI1A (rs7086803) polymorphism was associated with an increased risk of developing non-small cell lung carcinoma (NSCLC) compared with the GG genotype (AA vs. GG: odds ratio [OR] = 2.020; 95% confidence interval [95% CI] , 1.033-3.949, p = 0.037). The AA genotype of VTI1A (rs7086803) in smokers predicted significantly shorter OS (median survival time [MST]: AA 9.8 months, AG 19.3 months, GG 12.2 months, p = 0.017). Adiponectin receptor 1 expression in tumor tissues with the AA genotype was significantly lower than that for other genotypes (mean rank: AA 18.55, AG 25, GG 45.76, p = 0.001). Conclusions The presence of the allele A of VTI1A (rs7086803) may be the allele contributing to the risk of lung cancer susceptibility in Chinese population. Smoking lung cancer patients with the AA genotype of VTI1A gene (rs7086803) had a poor survival rate. Adiponectin receptor 1 expression may be correlated with the susceptibility of the allele A of VTI1A.

    Abstract: This study investigates the effectiveness of information technology–based 2-way referral in the diagnosis and management of cervical lesions in Minhang District, Shanghai, China. Women who underwent screening for cervical lesions in 4 community health centers constituted the intervention group, whereas women from the other 9 community health centers were included as controls. The diagnosis rate of cervical lesions was higher in the intervention group (7.61%) than in the control group (0.36%; P = .000). The diagnosis rate of early cervical lesion was 97.11% in the intervention group and 85.71% in the control group, showing no statistically significant differences ( P = .080). However, early diagnosis rate of precancerous lesion was much higher in the intervention group (95.45%) compared with the control group (64.29%; P = .001). In conclusion, higher effectiveness of early diagnosis and management of cervical lesions was observed in the intervention group compared with the control group.

    Abstract: A polymer coating applied to the wet surface of a ship’s hull can reduce some types of damage to a ship subjected to a far-field underwater explosion. Results from numerical simulations are compared with full-scale underwater explosion test data for a ship with and without polymer coating to study the effectiveness of such coatings in blast-resistant design. The polymer coating is a thin wall structure applied to the wet surface of the ship hull, while the ship is a relatively large structure. Hence, an effective approximate simulation method for the full-scale ship with polymer coating is developed. Based on the homogenization theory of polymer coating, a continuum model that takes into account the strain rate dependence is introduced. The shock response subjected to underwater explosion of the finite element ship model is investigated. The results obtained from underwater explosion tests and numerical simulation are compared and discussed. The blast-resistant property of polymer coating is elaborated. Mesh sensitivity and the validity of the polymer coating equivalent continuum model are studied.

    Abstract: Portal vein thrombosis (PVT) is a common complication in cirrhosis. The aim of this study was to determine risk factors for PVT, assess the efficacy of anticoagulant therapy, and evaluate the effects of PVT on patients with cirrhosis undergoing elective transjugular intrahepatic portosystemic shunt (TIPSS). A total of 101 patients with cirrhosis undergoing elective TIPSS were prospectively studied. After TIPSS, all patients received preventive therapy for PVT and were followed up at 3, 6, 12, and 24 months. Clinical outcomes were compared between patients who developed PVT after TIPSS and those who did not. Multivariate analysis showed that white blood cell count (relative risk [RR]: 0.377; 95% confidence interval [CI] : 0.132-0.579; P = .001), Child-Turcotte-Pugh score (RR: 1.547; 95% CI: 1.029-2.365; P = .032), and ascites (RR: 1.264; 95% CI: 1.019-1.742; P = .040) were independent predictors for PVT. Warfarin treatment within 12 months achieved significantly higher rates of complete recanalization than aspirin or clopidogrel in patients with PVT (54.5% vs 31.3%; P = .013), although adverse events were similar between the 2 groups ( P 〉 .05). Patients without PVT had significantly lower 2-year cumulative rates of variceal rebleeding (15.9% vs 36.6%; P = .023), shunt dysfunction (27.0% vs 46.8%; P = .039), hepatic encephalopathy (24.1% vs 42.6%; P = .045), and hepatocellular carcinoma (11.4% vs 31.2%; P = .024) and markedly higher 2-year cumulative survival rates (89.8% vs 72.9%; P = .041) than those with PVT. The PVT is associated with poorer clinical outcomes in TIPSS-treated patients, and warfarin is both safe and more effective in recanalizing PVT than aspirin or clopidogrel.

    Abstract: Myelolipomas are benign tumors, consisting of hematopoietic cells and mature adipose tissue, which mainly occur within the adrenal gland. Extra-adrenal myelolipomas are rare, and fewer than 60 cases have been reported in the literature. Here, we report a case of intrasplenic myelolipoma in a 42-year-old man with more than 1 month of abdominal pain. Computed tomography scanning revealed a giant, heterogeneous, well-demarcated mass in the spleen. Splenectomy was performed, and an intrasplenic giant mass was completely excised. The diagnosis of myelolipoma was made based on morphological examination. To the best of our knowledge, this is the third reported case of myelolipoma in the human spleen.

    Abstract: High-speed grinding experiments of particle reinforcing titanium matrix composites ((TiC p  + TiB w )/Ti-6Al-4V) were carried out with vitrified cubic boron nitride wheel and electroplated cubic boron nitride wheel, at the wheel speed ranged from 80 to 140 m/s. The cutting behavior, that is, grinding force, grinding temperature, specific grinding energy, and ground surface morphology, and grinding chips are analyzed. The results indicate that compared to the workpiece speed and the depth of cut, the wheel speed has a more significant influence on the grinding forces. The grinding temperature and specific grinding energy obtained with the vitrified cubic boron nitride wheel are always larger than those with the electroplated cubic boron nitride wheel. Based on the comprehensive consideration in terms of grinding force, grinding temperature, and specific grinding energy, the electroplated cubic boron nitride wheel is more suitable than vitrified cubic boron nitride wheel for high-speed grinding particle reinforcing titanium matrix composites. The removal of the reinforcements of particle reinforcing titanium matrix composites is mainly by means of pullout, fracture or crushing, micro-cracks, voids, and smearing. The segment chips containing the reinforcements are formed during high-speed grinding of particle reinforcing titanium matrix composites.

    Abstract: We investigated the preventive effect of alprostadil on contrast-induced nephropathy (CIN) in patients with renal insufficiency undergoing percutaneous coronary intervention (PCI). A total of 300 patients with creatinine clearance (crCl) ≤60 mL/min undergoing PCI were randomly assigned to alprostadil or a control group. The primary end point was the incidence of CIN defined as an increase in serum creatinine (Scr) levels by ≥0.5 mg/dL or≥ 25% after administration of the contrast media within 72 hours. The secondary end points were (1) changes in Scr and crCl within 72 hours and (2) the incidence of major adverse events during hospitalization. The incidence of CIN was 2.7% (4/150) in the alprostadil group, and 8.7% (13/150) in the control group (χ 2 = 5.05, P = .043).There was no difference regarding the incidence of major adverse events during hospitalization between the alprostadil group and control groups (2.7% vs 4.0%, P = .750). Multivariate logistic regression analysis showed that alprostadil was an independent protective factor for CIN (odds ratio = 0.136, 95% confidence interval: 0.020-0.944, P = .044). Prophylactic administration of alprostadil may prevent CIN in patients with renal insufficiency undergoing PCI.

    Abstract: Progression of coronary artery calcification (CAC) may be more predictive of future coronary heart disease events than a baseline CAC score. We determined whether serum gamma-glutamyltransferase (GGT) activity can independently predict the progression of CAC in adults with type 2 diabetes mellitus (T2DM). Patients (n = 326) without symptomatic cardiovascular (CV) disease were evaluated by CAC imaging. The CAC scores were assessed at baseline and after 20 ± 4 months. Serum GGT activities were significantly higher in progressors compared with nonprogressors (39 ± 16 vs 27 ± 11 U/L, P 〈 .001). Multivariable analyses demonstrated that GGT activity retained a strong association with CAC progression after adjustment for CV risk factors. Additionally, there was a graded association between GGT activity quartile and annualized CAC progression. In asymptomatic patients with T2DM, we prospectively found that serum GGT activity may be an independent predictor of CAC progression but not a predictor of CAC incidence.

    Abstract: Battle damage survivability is the prime design criterion of naval ships. Composites and hybrid structures such as polymer coating attract much attention to increase the ship survivability. Aiming to develop the parameter optimization design of the coating and to assess the underwater explosion shock environment and the blast-resistant effect of polymer coating efficiently, an approximate numerical method was performed in this preliminary study. An equivalent continuum model based on the homogenization theory was introduced to simulate the dynamic behavior of the coating. The test data of full-scale underwater explosion tests and the numerical simulation results were investigated. The blast-resistant property of the polymer coating was elaborated. The core strength had an important influence on the ship dynamic response: under low-intensity underwater explosion shock load, “soft” core had greater blast-resistant effect than the other ones, while under high-intensity underwater explosion shock load, “ordinary” core and “hard” core had better blast-resistant effect than the “soft” core.

    Abstract: A low-speed wind tunnel study of active flow control for improving aerodynamic performance of delta wings at high angles of attack is presented. A dielectric barrier discharge actuator driven by pulsed nanosecond voltage is used to create periodic perturbations along the leading edge of delta wing. Four wings with sweep back angles of Λ = 30°, 47°, and 60° are tested. Data obtained by means of force measurements show that the effectiveness of the actuation strongly depends on the wing’s sweep angles and the actuation frequency. For Λ = 30°, 47° delta wings, the pulsed actuation could increase the lift at post stall regions. For Λ = 60° delta wing, the pulsed actuation at the leading edge almost has no effect on its aerodynamic performance. At last, the control mechanism is discussed.
