Abstract: Bowel loop anastomosis is considered as major part of elective gastrointestinal surgeries. The anastomotic procedures being used now a days include hand sewn and stapled anastomosis. For appropriate gastrointestinal anastomosis, many factors should be considered such as intraoperative duration, restoration of blood supply, restoration of normal function of gastrointestinal tract and decrease tissue damage. Objective: The objective of the study was to compare the outcome of stapler and hand sewn anastomosis in elective gastrointestinal surgeries. Methods: Cross-sectional observational study conducted in department of surgery, Sheikh Zayed Hospital, Rahim Yar Khan from May 1, 2021 to August 31, 2022. Total 60 patients included in study and outcome variables such as anastomotic integrity, duration of procedure, post-operative hospital stay and return of bowel activity compared in hand sewn and stapled anastomosis. Results: The study included total 60 patients out of which 38 (63.33%) underwent stapled anastomosis and 22 (36.66%) underwent hand sewn anastomosis. Age (P value: 0.373), gender (p value: 0.372), anastomotic site (p value: 0.284) and return of bowel activity (p value: 0.331) did not show statistically significant difference between two groups. Anastomotic integrity (p value: 0.025), duration of procedure (p value: 0.002), post-operative hospital stay (p value: 0.037) show statistically significant difference between hand sewn and stapled anastomosis. Conclusions: Stapled anastomosis has better anastomotic integrity, reduced duration of procedure and decreased post-operative hospital stay as compared to hand sewn anastomosis with statistically significant difference between two groups.

Abstract: Objective: This study was conducted to find the best treatment in treating the patients of myofascial syndrome for this purpose two treatment options were compared to rule out the best option of treatment in minimizing the pain, enhancing the cervical movement and in determining the pressure pain threshold of MTrp. Material and methodology: It was a comparative study. Total 60 patients were randomly allocated to each group of treatment. We compared two treatments, treatment A of 30 patients was physical therapy and 30 patients of treatment B was diclofenac patch. For seven days, these patches were applied three times per day to the MTrP area of the upper trapezius muscle. The fix’s sufficiency and security boundaries were evaluated before it was used (day 0), three days later (day 3), and six days later (day 6). Chi square test was used for checking association of treatments and different attributes of patients. Results: There is significance association was found between demographic characteristics and treatments groups with p-values less than 0.05. There is significance difference effect between both treatments using VAS pain scale and cervical active Rom and PPT of MTRP using p-value less 0.05. These results shows that physical therapy treatment effects are batter as compare to diclofenac patch. Conclusion: Physical therapy techniques and the diclofenac patch have both been shown to be helpful but significant results are shown by Group A in treating trigger points of upper trapezius by normalizing the pathophysiological reasons of trigger points of this region. Physical therapy techniques are more helpful in reducing pain and in achieving cervical movement.

Abstract: Background: Hepatitis C is the leading cause of liver damage that lead to death if left untreated. Sofosbuvir has recently attained enormous success due to high rate of virological response. Recent studies have shown that addition of vitamin D to antiviral therapy improves the efficacy of treatment in patients with Hepatitis C. Objective: To compare the frequency of virological response in hepatitis C patients treated with vitamin D along with sofosbuvir plus ribavirin to those treated with sofosbuvir plus ribavirin only Study Design: Randomized-controlled-trial Place and Duration of Study: Department of Medicine, Pak Emirates Military Hospital, Rawalpindi from 1st November 2017 to 30th April 2018. Methodology: One hundred patients of chronic hepatitis C were included. Patients were randomly divided into two groups; group A receiving vitamin D along with sofosbuvir plus ribavirin and group B receiving sofosbuvir plus ribavirin therapy only. Patients were treated for 6 months and their RVR was monitored. Results: The mean age of patients in combination group was 39.0±12.92 years and in control group were 36.66±13.80years. After 4 weeks, RVR (no HCV RNA detected on PCR) was achieved in 31 (62%) cases in combination group while in 19 (38%) cases there was still HCV RNA present. In control group, RVR could be achieved in 18 (36%) cases while in 32 (64%) cases, RVR could not be achieved. The difference was significant between both groups (p 〈 0.05). Conclusion: The addition of vitamin D can help in achieving RVR in 〉 50% cases within 4 weeks. So addition of vitamin D in standard therapy may help to improve the quality of treatment. Abstract: Artificial intelligence (AI) has provided significant help in many fields of life. This study proposed a framework that helped in understanding customers’ attitudes about the adoption of Robo-advisors. The role of the Technology Readiness Index moderated as one of the primary relationships. A total of 208 potential users of Robo-advisor services provided the data that confirmed the validity of the model. This model provided the input for structural equation modeling and analysis of the study hypotheses. The results indicated that consumers showed positive attitudes about Robo-advisor services, with the moderating effect of Technology Readiness Index dimensions, namely, contributors and inhibitors. Perceived ease of use, perceived usefulness, and perceived convenience influenced consumers in developing positive attitudes about this service. Financial businesses can design better AI Robo-advisor services to fulfill the requirements of a wide range of consumers. This proposed framework contributes to the consumers’ understanding of behavioral intentions for the use of Robo-advisors in FinTech.
